General Information

The following information provides an overview of the SeNT referral

system for HNECC PHN and HNE LHD eReferral partners.

SeNT Referral System

The HNECC Primary Health Network, HNE HealthPathways Team and Hunter New England Health are working together to develop a ‘whole-of-health system’ e-referrals solution for local GPs, specialists and other primary care providers with the help of BPAC Clinical Solutions.

The Solution

The SeNT e-referral system allows GPs to send secure referrals electronically to approving private and public clinical specialists and allied health providers. The clinical content, specialist forms, e-referral algorithms and referral pathways are based on HealthPathways (www.hne.healthpathways.org.au).

How it Works?

SeNT uniquely integrates structured HealthPathways and specialist required pre-requisites with GP clinical software such as Medical Director and Best Practice.

The system architecture is based on the selection of; clinical area, condition, urgency, public/private, and location.

After installing the SeNT widget, GPs can seamlessly open the SeNT electronic referral from either inside the patient’s clinical record, or through links within HealthPathways.

Information such as patient details including observations and social history, medical history/investigations, current medications, allergies/adverse reactions all auto-populate from the clinical software.

Implementation of SeNT(Phase 1)

• Initial roll-out to over 20 GP practices in Greater Newcastle region.

- 50% Medical Director

- 50% Best Practice clinical software users

• Design and delivery of e-Referrals for patients initially in seven clinical areas:

Colorectal Genetics Ophthalmology Urology

Diabetes Maternity Paediatrics

• Participating practices are provided with on-site training and support.

• Public specialists receive referrals as norm, with referrals linked and sent through HNELHD VPN

• Private specialists receive referrals through either referral manager online portal, or elect for traditional ‘print only’ version.

• More than 60+ HNE Health Services, including hospital outpatients, emergency departments, community and mental health are participating.
