Self Management Support for Diabetic Patients at the Birmingham Free Clinic Sudipta Mohanty, Lauren Jonkman, Mary Herbert, Thuy Bui Background : Self management support (SMS) programs utilizing various sophisticated communication tools have shown great promise in improving patient self-management of chronic diseases such as diabetes. However, its applicability in low-income communities is questionable because of less access to such tools. Objective : This project aims to evaluate the feasibility of a phone-based SMS program for underserved diabetic patients seen at the Birmingham Free Clinic . Methods : Medical, pharmacy, and translational research students collaborated to survey clinic patients with type 2 diabetes on the following topics: current Results : Discussion : Results from survey data were used to create a new SMS program design tailored to address the specific needs of this patient population at Birmingham. Patients attended informational sessions and expressed strong interest in the new SMS program. Future Direction : We are collaborating with students from Nutrition and Dietetics to create the dietary counseling texting program mentioned above. Patients will receive text messages twice a week for 6 months educating them about ADA diet and The Birmingham Free Clinic A multidisciplinary primary care clinic providing free care to uninsured patients in the greater Pittsburgh area. 80% of patients are below the federal poverty level. 1/3 have no income. 50% are homeless. 15-17% are non citizens. Background: Self management support (SMS) programs utilizing various sophisticated communication tools have shown great promise in improving patient self-management of chronic diseases such as diabetes. Objective: This project aims to evaluate the feasibility of a phone-based SMS program for underserved diabetic patients seen at the Birmingham Free Clinic .

Methods: Medical, pharmacy, and translational research students collaborated to survey clinic patients with type 2 diabetes on the following topics: current diabetic management, diabetes health literacy, familiarity with cell phone use, reliability of cell phone communication, and interest in cell-phone based SMS programs.

Results:

Discussion: Results from survey data were used to create a new SMS program design tailored to address the specific needs of this patient population at Birmingham. Patients attended informational sessions and expressed strong interest in the new SMS program.

Future Direction: We are collaborating with students from Nutrition and Dietetics to create the dietary counseling texting program mentioned above. Patients will receive text messages twice a week for 6 months educating them about ADA diet and guiding them through positive dietary changes. The clinic is also working to recruit dieticians to provide over the phone assistance.
