In-Process Medical History: He arrived at Guantanamo Bay in good health. It is noted on his in-processing physical that he had a contusion above his left brow with a 2 cm abrasion, a 2 cm gunshot wound entrance wound on the medial aspect of his right lower leg, and a 2.5 cm gunshot wound exit wound 1 inch superior to the entrance wound with good healing tissue. His weight was 118 pounds with a BMI of 19.

Detention Medical History: He had access to medical care on a daily basis. He hunger struck on many occasions as above.

Detention Psychiatric History: No such diagnoses or history of self-injurious

behavior.

Detention Inpatient History: None

Detention Surgical History: Pilonidal cyst incision and drainage

Dental History: He had access to routine dental care while in custody.

Eye Exam: He had access to routine eye exams and was prescribed glasses. Current Health: His last clinic appointment was in April 2006. His last BMI was 28 on 21 May 2006 with a 108% Ideal Body Weight.

Medications: None

Immunizations: PPD 8/02, 12 mm; MMR 4/03; Hep A/Hep B (TWINRIX) completed 5/03; Influenza 11/02, 1/06; completed DpT/Tetanus 5/03. Hepatitis A, Hepatitis B, Hepatitis C and HIV screens were negative. He is not G6PD deficient.

In-Process Medical History: He arrived at Guantanamo Bay in good health and without any injuries. It was noted on his entrance physical that he had patchy alopecia, 4 cm scars on his shins bilaterally, and a sunken in chest secondary to being kicked by a donkey in the past. His weight was 114 pounds with a BMI of 17.3,

Detention Medical History: He had access to medical care on a daily basis. He was a hunger striker January, March, and May 2006 and in July, August, September, Nov-December of 2005. He also was a documented hunger striker in July 2002. He was diagnosed with gastroesophageal reflux disease during a hunger strike.

Detention Psychiatric History: He has no known psychiatric diagnosis and no

history of self-injurious behavior.

Detention Inpatient History: None

Detention Surgical History: None Dental History: He had access to routine dental care while in custody and had 2 extractions in December 2004 and was seen in sick call in September 2005.

Eye Exam: He had access to routine eye exams but did not request any appointments.

Current Health: Last seen in clinic on 30 May 2006 for a hunger strike evaluation and intravenous hydration. His last weight was 119 lbs. on 06 June 2006. His calculated BMI is 18 and ideal body weight percentage is 79.1.

Medications: None

Immunizations: PPD 6/04, 0 mm; MMR 4/03; Hep A/Hep B (TWINRIX) completed 10/04; Influenza 10/03; completed DpT/Tetanus 3/03. Hepatitis A, Hepatitis B, Hepatitis C and HIV screens were negative. His malaria smear was negative. He is not G6PD deficient.

Past Medical History: History of nephrolithiasis and urinary tract infections History of peptic ulcer disease Hunger striking 7/02, 7/05, 8/05, 9/05, 10/05, 12/05 Hunger striker - enterally fed from 12/05 until 6/2/06, tube fed 169 days. Chronic knee pain Chronic testicular pain - Varicocele Old 5th metacarpal fracture stable

Past Surgical History: Right Mandibular abscess/cyst incision and drainage in July 2002.

In-Process Medical History: He arrived at Guantanamo Bay in good health. He had a 2cm scar in left frontal, multiple scars on scalp, superficial abrasion on left pinna and right cheek. He was found to have tenderness epigastrically on entrance physical exam. He complained about chronic testicular pain and dysuria on entrance history. His weight was 172 pounds with a BMI of 27.

Detention Medical History: He had access to medical care on a daily basis. In July 2002, he was treated for right mandibular abscess/cyst and required incision and drainage. Treated for a left otitis media in July 2003. He had a contact dermatitis in February 2005 that resolved. Complained about chronic testicular pain and was found to have a varicocele in February 2005 and was given scrotal support. He complained of chronic knee pain and treated appropriately with anti-inflammatory pain medications and exercises. He was a repeated hunger striker during 2005. In December 2005, he was worked up extensively for significant weight loss. After complete work up was negative, detainee admitted to being a covert hunger striker. He had been seen for inflammation of his nasal passages due to prolonged enteral feeding while on hunger strike. Evaluated by ENT in April 2006 and resolved with some topical antibiotics. Detainee was a prolonged hunger striker who was enterally fed for 5 months. Ended last hunger strike on 06 Jun 2006. He was 113.5% of IB W with a weight of 1651bs prior to ending his hunger strike.

Detention Inpatient History: Admitted to detention hospital for weight loss work up in December 2005 and later enterally fed in the hospital before moving to the feeding blocks.

Detention Surgical History: In July 2002, he had incision and drainage of right mandibular abscess/cyst.

Dental History: He had access to routine dental care while in custody and in May 2004 and May 2006 for a routine exam and cleaning.

Eye Exam: He had access to routine eye exams.

Current Health: He is currently in good health on most recent exam with a BMI of 26.

Medications: None

Immunizations: PPD negative; MMR 4/03; Hep A/Hep B (TWINRIX) completed 10/03; Influenza 10/03; completed DpT/Tetanus 4/04. Hepatitis A, Hepatitis B, Hepatitis C and HIV screens were negative. He is not G6PD deficient.
