FOREWORD

The Scope of Practices for Public Health Officers and Technicians clarifies the professional role and

responsibilities of registered professionals in Kenya. The full scope of Public Health practice defines

the boundaries within which professional practice is pegged.

The document sets the outer borders of practice for all members and encompasses all that PHOs

and PHTs are trained and legally mandated undertake as outlined in Public Health Officers

(Training, Registration and Licensing) Act No. 12 of 2013.

The document also forms the basis of professional indemnity that is guaranteed to a Public Health

officer and Technician while undertaking their professional duties and responsibilities. It similarly

outlines the basis of sanctions in case of professional malpractice or practice outside the

established limits as per the established areas of competence.

The scope of practice therefore provides registered Public Health Officers and Technicians,

employers, clients and the general public with a deeper understanding of professional

engagements necessary for the delivery of public health services in Kenya.

It defines to the professionals while providing public health care delivery overlaps at every level of

exit for distinction and excellence in pursuit of quality standards of professional practice, ethics

and competence as well as areas of continuing professional development.

This document outlines the core professional competences or each exit levels that determine the

scopes of practice. The basic premise behind these exit levels include: putting client needs first; a

commitment to professionalism, lifelong learning and the recognition of the need for legislative

and regulatory changes to support evolving scopes of practice; and that health professionals

should be involved in decision making regarding scopes of practice.

GLOSSARY OF TERMS

“Analysis” means the testing or examination of any matter, substance or process for the

purpose of determining its composition or qualities or its effect (whether physical, chemical or

biological) on any segment of the environment

“Communicable disease” means an infectious disease transmissible (as from person to person)

by direct contact with an affected individual or the individual's discharges or by

indirect means (as by a vector)

“Competence” means the ability of a registered Public Health Officer to integrate and apply the

knowledge, skills, judgment and interpersonal attributes3 required to practice safely and

ethically in a designated role and setting

“County health department” means any regional department responsible for health as per County

Governments Act

“Environment” means the surroundings within which humans exist and which are made up of: (a)

the land, water and atmosphere of the earth, (b) micro-organisms, plant and animal life, (c) any

part or combination of (a) and (b) and the interrelationships among and between them, and (d)

the physical, chemical, aesthetic and cultural properties and conditions of the foregoing that

influence human health and well-being;

“Public health Officer / Technician” means, any person registered as such with the Public Health

and Technician Council Kenya and includes: (a) Public health Officer, (b) Public health Assistants,

(c) Assistants Public Health Officers , (d) student Public health Officer, (e)Public health practitioner

in private sector

“Environmental health services” mean the assessment, monitoring, correction, control and

prevention of environmental factors that can adversely affect human health. These services

include but are not limited to anticipation and identification of environmental health hazards and

risks

“Hazardous substance” means any chemical, waste, gas, medicine, drug, plant, animal or micro-

organism which is likely to be injurious to human health or the environment

“Health” means a state of complete physical, mental and social well-being, and not merely the

absence of disease or infirmity

“Health nuisance” means a situation or state of affairs that endangers life or health or adversely

affects the well-being of a person or community

“Pollution” means pollution as defined in Environmental Management and Co-Ordination Act,

1999 (ACT NO 8 OF 1999); Any direct or indirect alteration of the physical, thermal, chemical,

biological, or radio-active properties of any part of the environment by discharging, emitting, or

depositing wastes so as to affect any beneficial use adversely, to cause a condition which is

hazardous or potentially hazardous to public health, safety or welfare, or to animals, birds, wildlife,

fish or aquatic life, or to plants or to cause contravention of any condition, limitation, or restriction

“Premises” means any building, structure or tent, together with the land on which it is situated and

the adjoining land used in connection with it, and includes any land without any building, structure

or tent and any vehicle, conveyance or ship;

“Public health risk” means a likelihood of an event that may adversely affect the health of human

populations; with an emphasis on one which may spread internationally or may present a serious

and direct danger

“Regulations” mean regulations made under Public Health related Acts

“Stakeholder” means individuals and groups concerned with or affected by the environmental

performance of an organization

“Waste” includes any matter prescribed to be waste and any matter, whether liquid, solid,

gaseous, or radioactive, which is discharged, emitted, or deposited in the environment in such

volume composition or manner likely to cause an alteration of the environment

“Water” includes drinking water, river, stream, water-course, reservoir, well, dam, canal, channel,

lake, swamp, open drain, or underground water;

“Vector” means an insect or other animal that normally transports an infectious agent that

constitutes a public health risk.

PREAMBLE

Public Health Officers and Technicians Council is a statutory body established by the Public Health

Officers (Training, Registration and Licensing) Act No. 12 of 2013 to regulate the training,

registration and licensing of public health professionals in Kenya.

The Council aspires be a world class public health regulatory body for maintaining standards of

public health training and practice in Kenya by defining the threshold of practice for all the levels

of exit and providing professional indemnity to all registered Public Health Officers and Technicians

practicing in Kenya.

This Scope of practice document therefore provides the basis for determining professional

competence in Public Health practice and forms the foundation of offering a professional

indemnity to all Public Health Officers and Technicians during professional practice.

The document therefore defines the areas of professional practice, the corresponding levels of

exit for each area of practice and the boundaries within which the professionals qualified in

specific areas of practice will be confined for the purposes of quality public health service delivery.

The Scope of practice document spells out, in detail, the services that a qualified Public Health

professional is deemed competent by the Council to perform and is permitted to undertake within

the confines and terms of their professional license as established by relevant law and regulations

guiding such practice.

In addition, the document will guide employment and deployment of all Public Health Officers

and Technicians by various organizations based on the levels of competence to guide efficient

and effective service delivery in all areas of professional practice.

Further, the scope of practice document will define scope of training by guiding curriculum

development for all levels of exit in all the accredited and recognized training institutions offering

Public Health programmes in Kenya.

This document will therefore provide a legal and regulatory framework for registered Public Health

Officers and Public Health Technicians in particular and the public in general for work the areas

of competent practice to the levels of qualifications.

This document will therefore be a critical guide to the National Government, County

Governments, the Non-state actors, NGO sector, private sector and all other relevant

organizations either in the public or private sector in engaging Public Health Officers and

Technicians for efficient and effective public Health service delivery in Kenya.

BACKGROUND

Public Health practice covers the assessment, correction, control and prevention of

environmental factors that can adversely affect human health.

It encompasses measures necessary to deal with issues such as environmental degradation and

climate change and with hazards, including chemical exposure and contamination of air, water

and food.

Public health practice provides opportunities to enhance health by planning and developing

health-promoting environments that contribute to better health outcomes. This include (a) water

quality monitoring, (b) food quality control, (c) waste management, (d) surveillance of premises,

(e) communicable diseases control, (f) vector control, (g) environmental pollution control, (h)

disposal of the dead, (i) chemical safety and noise control, U) port health, (k) malaria control, (I)

hazardous substances control, and (m) air quality management.

Public Health practitioners (Public Health Officers and Technicians) are responsible for carrying out

measures for protecting public health, including administering and enforcing legislation related

to environmental health and providing support to minimize health and safety hazards.

The practitioners licensed under the relevant laws are multi-skilled professionals trained in various

areas of specialties with minimum of certificate level to a maximum post graduate degree levels.

However, the course of training may be organized to professional competency in short courses

required for professional development in existing and emerging areas of practice.

Public Health professionals, upon full implementation of this scope of practice, will be competent

practitioners offering quality public health services for agencies of the National government,

County health authorities and other organizations in the public or private sector.

The Scope of Practice for Environmental Health Services is organized around in four pillars of

practice which are: (a) Public Health Engineering, (b) Community Health, (c) General Public

Health, (d) Food Science and Inspections.

The competency levels (exit levels of qualification) considered in this document includes Post

Graduate Degrees as the highest level, Bachelor’s Degree, Higher National Diploma, Diploma and

Certificate in Environmental Health as the lowest level.

Each level of competency is defined around the core pillars that guide the corresponding scope

of practice to guide service delivery by all Public Health Officers and Technicians in Kenya.

1. Pillar I – Public Health Engineering Practice This pillar of the scope is concerned with the application of scientific and engineering principles for protection of human populations

from the effects of adverse environmental factors; protection of environments, from potentially deleterious effects of natural and

human activities; and improvement of environmental quality. The scope of activities for this section includes; Water, sanitation and

hygiene (WASH), Liquid and Solid waste management, conservancy, drainage and plumbing, Pollution control Climate Change and

ELIGIBULITY FOR REGISTRATION, LICENSURE AND PRACTICE Every Public health officer/Technician practicing in Kenya is required to be registered and

licensed with the Public Health Officers and Technicians’ Council of Kenya in accordance with

the Public Health Officers (Training, registration and Licensing) Act No. 12 of 2013, Laws of Kenya.

Graduates of certificates, diploma and degree programmes in Environmental Health trained

from approved institution that have passed the Council professional licenses and have gone

through a one (1) year PHOTC supervised internship programme from a recognized internship

Centre. The graduates will be awarded practicing licenses in a public graduation ceremony

after reciting Public Health Officers and Technicians professional oath, receiving registration

certificate and a practicing license a copy and signing PHOTC code of ethics document.

CONTINUING PROFESSIONAL DEVELOPMENT (CPD) All professionals shall adhere to the CPD requirements for PHOTC and shall only be maintained in

our register if they met the minimum 60 CPD points per year as prescribe in the CPD guidelines

for Public Health Officers band Technicians Council.
