    Sub: - Dr. NTR UHS - Academic Sending of Schedule for NursingCourses for the academic year 2014-2015 Reg.

    I am by direction, herewith furnishing the SCHEDULE which is to be

    followed by the colleges offering Nursing courses for the academic year 2014-2015so

    as to apply for INSPECTION / AFFILIATIONin the requisite form Annexed to the

    Statutes by remitting the requisite amount of fee for each course to Dr. NTR

    University of Health Sciences, Vijayawada only in the form of Demand Draft in

    favour of the Registrar drawn on any Nationalized bank, payable at Vijayawada.

    It is informed that the application for INSPECTION or AFFILIATION shallbe accompanied with the following details.

    I. FEE PARTICULARS

    a. Inspection for affiliation / continuation of affiliation proposal should be submitted

    in prescribed application form as per Nursing statutes and should pay prescribed

    fee.

    b. The colleges where 1st batch of students completed course, have to apply for

    continuation of affiliation by paying requisite fee.

    c.

    University usually arranges inspection every year for the colleges until the 1

    st

    batch/ Enhancement seats batch completes their course. Such colleges shall apply for

    INSPECTIONand also for continuation of AFFILIATION.

    d. A fee of Rs.50,000/- is prescribed for issue of consent of affiliation which enables an

    institute to submit it before INC for getting permission to start new nursing

    courses.

    e.

    The colleges offering Under graduation and Post graduation courses should

    submit University Inspection / Affiliation proposals and requisite fee separately for

    M.Sc (N), B.Sc (N) & Post Basic B.Sc. Nursing courses.

    f.

    Government Colleges are exempted from payment of Inspection fee and Affiliationfee, however, the colleges shall apply for Inspection and Affiliation every academic

    year in the prescribed proforma as per statutes.P.T.O

    II. STAFF PARTICULARS

    a. Physical facilities, clinical data and Teaching staff particulars of all the departments

    unit wise including Principal, Vice Principal, Professors, Assoc. Profs, Asst.

    Profs., and Tutors / Sr. Residents are to be submitted as per enclosed proforma.b. In addition to the above, should submit teaching faculty ID forms speciality wise

    and also two (2) soft copies of Teaching Faculty Data i.e., scanned data containing

    details of each teacher separately speciality wise including Undergraduate &

    Postgraduate courses. This is mandatory for issue of Affiliation.

    III. PERMISSION LETTERS

    a. Renewal/Permission letter from INC, New Delhi for the year 2014-15.

    b. The colleges who have already applied for enhancement of seats and awaiting for

    INC, New Delhi / Government of A.P permission should also apply for

    University inspection for enhancement of seats along with continuation of

    affiliation.

    c. However, affiliation for enhancement of seats / renewal of permission for M.Sc

    (N), B.Sc (N) and Post Basic B.Sc. Nursing courses will be considered only after

    receipt of letter of permission from Indian Nursing Council / Government of A.P.

    d. Each institute should be running at the address permitted by Government only. If

    any college wishes to shift the college to new premises, they should obtain

    Government permission and submit the same to University. Shifting of college

    premises in between the academic year and without University/Government

    permission will not be entertained.

    IV. CLINICAL TRAINING TO THE STUDENTS

    a. Each institute should send the students for clinical training to the hospitals

    permitted by the Government only.

    b.

    Latest consent letters in original from the concerned hospitals regarding clinical

    training should be submitted along with affiliation proposals.

    c.

    Government permission is necessary for changes in clinical affiliated hospitals.

    V. GENERAL INFORMATION

    a. The University shall have the right to conduct an enquiry or appoint an enquiry

    committee if there are substantial allegations (or) irregularities in administrative,

    financial (or) academic nature, against the colleges.

    b. Should take all the measures to prevent ragging in the institutions. Necessary

    guidelines have already been issued in this regard.

    VI. ENCLOSURES REQUIRED FOR GRANTING AFFILIATION

    a. Faculty ID forms with enclosures like attested copies of qualification,

    appointment orders, joining reports etc,.b.

    Latest consent letters from clinically affiliated hospitals.

    c. Teaching staff details as per PROFORMAenclosed.

    d. CD containing the scanned data of the above.

    I request you to take necessary immediate action to have Inspection /

    Continuation of Affiliation within the dates as Notedabove for the academic year

    2014-15. This should be followed strictly.

    The Colleges who have already submitted the proposals before receipt of this

    intimation are requested to re-submit the proposals as per this pattern.

    Kindly treat the matter as MOST URGENT.
