Welcome to Sanford Health! You are a very important part of our system and I would like to thank you for all of your efforts on behalf of our patients and staff. While at Sanford, you will have the opportunity to help patients and to assist their family and friends.

A health system is a very dynamic place, which is necessary for total care of our patients’ needs and desires. We have many departments with employees to carry out the functions of our system, and you are an integral part of that mission.

Your efforts help us provide the care necessary to show how much we care. We are concerned about the well being of those who rely on us for their healthcare needs. Without you, it would be next to impossible to accomplish all we do for our patients and their families.

Things You need To Know!

healTh/infecTion conTrolWash hands properly between patients and: • After each patient contact • After unanticipated contact with blood or other body fluids • After removing gloves, gown or protective clothing • After touching potentially contaminated items • Use either soap and water or waterless antisepticRemember, clean hands is the mainstay of preventing the spread of infections. If you think you have an infection that’s contagious, report to your supervisor or the Employee Health office to determine if there are any work restrictions.

isolaTionThere are special isolation precautions for patients with certain conditions. Signs are posted on the door indicating any special protective gear (i.e., mask, gloves, gown). Before entering the patient’s room check with staff for proper instructions. Hand hygiene with antiseptic must be performed upon leaving the patient’s room. If you have questions, contact the Infection Prevention & Control at (605) 333-6478.

fall PrevenTionPatient safety is the heart of the matter of patient care at Sanford. Due to medical conditions, procedures, weakness or medications, our patients are at a greater fall risk while hospitalized. Patients at high risk for falling are identified by a bright yellow or red star “Call Don’t Fall” magnet on a patient’s doorframe or white board at most Sanford facilities. We need your partnership in keeping patients safe. If you see someone getting out of bed or a chair and they are a fall risk: 1. Encourage the patient to stay where they are. 2. Immediately request assistance. 3. If a person is a fall risk do not leave the patient alone if you transport

them into a room. 4. Alert staff if you have concerns about the patient’s safety. injuriesAny injuries sustained while on any Sanford Health campus must be reported to the Employee Health Office. At the time of injury, complete an Exposure/Employee Incident Report and submit it to the Employee Health Office. Please do not think that your injury is inconsequential and does not need to be reported. All injuries must be reported. Your compliance with this policy will be appreciated.

TB TesTingCurrent regulations require that all healthcare workers must have an annual TB skin test. If you have been documented to be PPD positive you will not be TB skin tested but must be assessed yearly for signs and symptoms of active pulmonary TB. Different states may have different requirements for this. Check with your facility’s Employee Health Office.

dress codeCasual business dress attire is required. No suggestive attire, blue denim jeans, shorts, short athletic clothing, sweatshirts, t-shirts, slit skirts, sandals or flip flops are allowed. Nylons or socks must be worn at all times. Body art (tattoos, piercing/jewelry) that may be offensive to patients, family members, guests of patients, other customers or employees, or if deemed inappropriate at the discretion of the director are not allowed to be visible. Examples of inappropriate tattoos include anything of an obscene or sexual nature and/or anything that may be construed to be discriminatory against any sex, race, ethnic background or religion. Examples of inappropriate piercing jewelry include facial jewelry, large ear jewelry or more than 3 pieces of ear jewelry per ear. No use of scented lotions, perfumes or cologne is permitted.

idenTificaTion BadgesIndividuals are required to wear a nametag identifying themselves. Name badges are obtained through Human Resources and must be worn above the waist. In the event that you lose your badge, a replacement badge will be issued for a $10 charge. Students should wear the name badge issued by the school with picture.

volunTeer uniform – if applicableA volunteer uniform will be provided. It must be worn at all times when volunteering. It is the volunteer’s responsibility to keep the uniform laundered. The name badge and uniform must be returned to Volunteer Services when the volunteer discontinues their volunteer job.

ParKingSanford USD Medical Center (SUSDMC): Two ramps are available for parking: one is west of the hospital (enter on 18th street) and the other is located south of Medical Building 3 (enter on 22nd street). We encourage you not to park along the streets around the hospital. Valet parking may be used when it is necessary because of physical limitations. Surface parking is also available on the corner of 19th & Grange or 18th & Lake. All other facilities: Check with your supervisor for instructions.

cell PhonesIn many areas of the hospital cell phones can interrupt the use of monitors and patient devices. For this reason we ask you to turn off your cell phone when in the hospital.

emergencY oPeraTionsCodes are announced on the hospital public address system to indicate an emergency situation. On the SUSDMC campus dial 3-1234 to report any of the following: Fire: Code Red, Building Name and Location. All Clear: Attention, Attention, Code Red, All Clear. Your responsibility during a fire emergency depends upon your location.

Report to your supervisor for instructions. Disaster: Attention, Attention, Code Disaster All Clear: Attention, Attention, Code Disaster All Clear

This announcement indicates that a disaster has happened in our community and the hospital and staff need to prepare for multiple patients. Report to the supervisor in your area for an assignment during a disaster.

Tornado: Attention, Attention, Tornado Watch or Tornado Warning. Take Cover. All Clear: Attention, Attention, Alert Has Been Cancelled. Search & Report: This code is used when a bomb threat has been made to

the hospital. Code Gray, Location: This overhead page signifies an active hostage

situation or shooter has been identified on the campus. It is meant to keep employees, patients, families and providers away.

clinical emergencY evenTsThe following are announced on the hospital public address system to indicate a clinical emergency situation. On the SUSDMC campus dial 3-1234 to report any of the following:Code Blue: Adult/Pediatric/Neonate locationThis code is used when a patient or person is in medical emergency. If you are in the code blue area, please move to another area or part of the department to allow emergency personnel easy and quick access to the patient or person requiring medical attention. Code Blue Adult indicates a person 15 years of age and older; a Code Blue Pediatric indicates a person 14 years of age and younger. Code Blue Neonate indicates an infant in the Intensive Care Nursery. Campus Assessment Team: This code is called when a person has fallen and needs assistance. Check to see if your facility has a response team.

Code Exit: Code Exit, Sex, Age (Code Exit female 2). This code is used for a missing person. All Sanford USD Medical Center employees should respond by securing the nearest exit.Code Man: Code Man, Building, Location. This code is used when there is an unruly visitor or patient creating a disturbance. Trained security and maintenance employees should go to the location requesting the assistance. All other campuses, check with your supervisor.

aBsenTee PolicY – volunTeersIf you are unable to volunteer during your assigned hours, please notify the unit supervisor from your volunteer assignment.

adminisTraTive resTrainTsThe officer/guard must remove all administrative restraint devices (handcuffs, leg irons, etc.) in the event of a medical emergency (heart attacks, seizures, respiratory or cardiac arrest).

smoKing PolicYSanford Health is a tobacco and smoke free institution. This affects all indoor and outdoor property.

PaTienT Bill of righTsA copy of the Patient Bill of Rights is located in all patient areas and patient bedsides. It is expected these rights will be respected at all times. Examples: • All patients have the right to participate in decisions regarding their

healthcare. • All patients have the right to information about the treatment they are

receiving. • All patients have the right to courteous and respectful treatment.

diversiTYWe believe it is critical to treat all individuals fairly, regardless of age, gender, sexual orientation, disability, race, creed, color, national origin, religion, veteran status or any other basis prohibited by applicable federal, state or local law.

non-emPloYee clinical comPeTencY exPecTaTionsTo ensure the safety of all patients, the organization and the individual delivering care, the following competencies and/or policies must be adhered to by all clinical personnel. • Use 2 patient identifiers prior to delivering any clinical care • Use read back of written verbal/telephone orders and critical test results • Accurately and completely document on patient records and adhere to abolished abbreviation list • Adhere to "hand-off" communication policy (SOP H-025) • Label medications, med containers, solutions on and off the sterile field in surgical and procedural settings • When working with surgical cases, use "time-outs" prior to beginning a case. This requires your full attention. • Use clinical alarms on all equipment • Follow CDC Hand Hygiene guidelines • Use correct forms to report unanticipated death/major permanent loss of function associated with healthcare- associated infection • Reconcile medications when appropriatecorPoraTe comPlianceSanford expects everyone to uphold the law as identified by state, national and local governments. If any suspected illegal or unlawful activity is noticed, it must be reported to the Corporate Compliance Help line at 1-800-325-9402 or Compliance Director at (605) 328-6641.

harassmenTIt is Sanford policy that all personnel are responsible for assuring that the workplace is free from any type of harassment. Because of Sanford’s strong disapproval of offensive or inappropriate behavior at work, all personnel must avoid any action or conduct which could be viewed as harassment.

Harassment includes verbal or physical conduct designed to threaten, intimidate or coerce. This includes any actions or behavior which interferes with an individual’s ability to do their job by creating a hostile or intimidating environment based on gender, race, color, religion, national origin, age, disability, military/veteran status, pregnancy or protected activity.

Sexual Harassment: Sexual harassment is unwelcome sexual advances, requests for sexual favors and other verbal or physical conduct of a sexual nature.

drug free worK PlaceSanford is declared a drug free workplace at all locations where company business is conducted. All employees are absolutely prohibited from unlawfully manufacturing, distributing, dispensing, possessing or using controlled substances in the workplace. Any person violating the above is subject to discipline, up to and including termination, for the first offense.

soliciTaTionSanford has a policy against any individual soliciting sales for personal profit or benefit. Sanford Health limits solicitation, distribution and posting of signs on its premises because these activities can interfere with normal operations, reduce employee efficiency and hinder the security of our employees and their property.

healTh insurance PorTaBiliTY and accounTaBiliTY acT of 1996 (hiPaa)Our mission is to provide the highest quality healthcare to patients. As part of this promise to care for them, we keep information about their health and healthcare confidential. Under a national law as of April 2003, it is illegal to violate this code. During your work at Sanford, you may encounter friends and acquaintances hospitalized or visiting for diagnostic tests or treatment. Always remember that the patient has certain rights to privacy while a guest of the facility. Forensic staff, as well as paid staff, must avoid discussion of personal matters relating to patients’ health, diagnoses or conduct at all times, both inside and outside the facility. Avoid talking about patients in public areas such as hallways and elevators where conversations may be overheard. Remember the old adage: What you see here, what you hear here, when you leave here, let it stay here. If you hear, see or otherwise become aware of a situation where health information is being inappropriately used or disclosed, it is your duty to report the incident to your instructor or supervisor who can then bring the concern forward. (Other options are to contact the Privacy Officer or to call the Compliance Helpline. The Privacy Officer and the Compliance Helpline telephone numbers can be located in Sanford Health’s intranet, the telephone directory or from the Helpline posters that are displayed throughout our facilities.)

the sick.

information generated from these encounters is treated as private information.

e Educate and inform. • Part of caring for the sick is to keep them and their loved ones

informed about their conditions and their treatment. • If one of us doesn’t know the answer to a question we can find a

Sanford resource.

c Communication is not done until understanding is reached. • Communication with those we serve and the rest of the medical team

is our job to complete. • We have not communicated until everyone understands.

T Time is valuable, especially yours. • Our time as caregivers is no more important than those we serve.

The time that a patient and family spends in obtaining healthcare advice and treatment will be considered with respect.

• Be respectful of each other’s time and devote time to developing relationships.
