Scientific Summary Scientific Summary: The summary should provide an abstract that is understandable by a multi-disciplinary group of scientific reviewers. Summarize the objectives and their relevance to the work proposed in the research project providing the reviewers with the central key question being studied and related research activities. If you are submitting an application that is fully focused on KT science, the scientific summary mustreflect the KT science focus. The Scientific Summary may be used to identify appropriate peer reviewers for your applications.

Canadian Common CV Upload
The Canadian Common CV provides information about your academic and research training, mentorship activities, KT plans, and non-academic activities e.g. community work.
• The applicant must complete the Common CV – MSFHR Full Version https://ccv-cvc.ca. The current MSFHR ApplyNet system is not linked with the Common CV system. To complete your requirement, please Submit your CV, provide your consent, confirm your submission, then go to the history tab and save a .pdf copy of the Common CV – MSFHR Full Version.
• In your MSFHR ApplyNet account, go to the Uploads section of your Full Application Form and upload the .pdf copy of the Common CV – MSFHR Full Version.
Publications, Patents & Intellectual Rights Upload
For Publications, Scholars should list all published and accepted publications throughout their career under the following suggested headings in a separate documents. Categories may be added as needed. Combine the publication list into one single .pdf document, and upload to MSFHR ApplyNet. There is no page limit for your publication list.

Directions:
• For each multi-authored publications, definie your role and indicate your percent contribution to the team effort.

contributions). • It is recommended to include the journal impact factor and number of citations for each
publication. • Maintain the author order as it exists in the original citation.
Patents and Intellectual Property Rights: This section should include details of patents and intellectual property rights for technology transfer, products, and services. Where appropriate, include information regarding the title, patents, or intellectual property rights number and date, country(ies) of issue, name of the inventors and assignee(s), as well as relevance of the stated item.

Program of Research Upload
Your Program of Research is limited to 10 pages. If you try to upload a document that is more than 10 pages, you will receive an error message.
Your Program of Research should include the description of a single project and a set of coordinated projects undertaking related research for the five year term. Knowledge translation activities must be embedded within the program of research.
As you prepare your program of research, please consider the following:
• Contact your host institution’s research office to find out when the internal deadline is for submission of your final application. It will be earlier than the MSFHR deadline of January 31, 2019.
• Your host institution’s research office is also an important resource for information on how to write a successful grant. We strongly encourage our applicants to reach out to your research office for support.
• Be aware of MSFHR’s formatting guidelines. Reviewers may mark you down for failing to observe the guidelines.
• You have 10 additional pages for your Appendices, which may include references, tables, charts, figures and photographs. Appendices are uploaded as a separate document. Reviewers are not obliged to review your Appendices. If you have data that is important to your program of research, include it in your Program of Research and ensure it is clearly labeled.
Format your Program of Research to be easy to read. Use headings to show a logical progression, and break up text into paragraphs.
Possible headings you may want to consider for your Program of Research may include, but are not limited to:
• Rationale, background and statement of the problem • Research question or hypothesis, aim and objectives • Study • Methodology • Expected outcomes • Dissemination of results and publication policy • Duration of the project, including a timeline for each activity • Problems anticipated • Ethical considerations
