PURPOSE OF THIS PERFORMANCE APPRAISAL
To do a personal inventory highlighting strengths and weaknesses and to agree on a practical improvement programme. Aperformance appraisal shall be conducted annually and serves as a history of development and progress.
GUIDELINES
1.

2. The appraiser shall first complete the entire form with the exception of the Employee Comments and then arrange todiscuss the form in a face-to-face Appraisal Meeting with the employee. The employee should have the opportunity tocomment verbally and in writing on the performance appraisal.

The Appraisal Meeting with the employee shall be conducted in private and allow the employee sufficient time toreflect on the appraisal prior to signing. The purpose of the meeting is twofold: (i) to review past performance; (ii) tolook forward to the year ahead.

The appraiser and employee should mutually agree a set of objectives for the coming year. Achievement of objectivesset for the appraisal period should be reviewed as part of the appraisal process.
5.
Based on the total score, the appraiser shall circle the respective rating according to the score as follows:
Rating Score
Outstanding: Performance is outstanding in all major aspects of the job. 5 85100
Good: Performance exceeds job requirements 4 7084
Satisfactory: Performance meets basic job requirements. 3 5069
Conditional: Performance does not consistently meet job requirements. 2 3149
Unsatisfactory: Performance unsatisfactory or fail to meet job requirements. 1 1230
