Pursuant to the Order entered by the Commission in the above-referenced matter on March27, 2015 (the "Final Order"),' please find enclosed the following additional permits and relateddocuments issued in connection with SKRECC's construction of its New Headquarters Facility inSomerset, Kentucky:^

• HVAC Permit # HVAC-16-02-0146-P (Frei Plumbing, Inc./Leonard H. Morrison III);• Plumbing Permit # PL-15-11-00766-P (Frei Plumbing, Inc./Desmond Frei);• Electrical Inspection Report and Permit # 31090 (F.I.E. Electric, Inc.);• Letter from the Kentucky Public Protection Cabinet, Department ofHousing, Buildings

and Construction, reflecting the approval of the Division of Plumbing of plans relatedto the New Headquarters Facility; and

• Letter from MSE of Kentucky, Inc.. SKRECC's architect, advising that a Notice ofIntent (NOI) for Construction Sites and the related Notice of Termination (NOT) neednot be submitted to the Kentucky Division of Water for the Project.

' Specifically, Ordering Paragraph 10 of the Final Order directs South Kentucky Rural Electric CooperativeCorporation ("SKRECC") to tender to the Commission copies of the permits referenced in paragraph 26 ofSKRECC'sApplication filed in this matter on October 20, 2014. This correspondence is intended to address the outstandingpermits and approvals discussed in that paragraph of SKRECC's Application.

In addition to the foregoing, SKRECC has been advised by the City of Somerset PlanningCommission that it need not seek nor obtain approval of a site development plan related to theNew Headquarters Facility. Finally, the portion of the Project related to the New HeadquartersFacility's communications tower remains in progress: SKRECC will keep the Commission abreastof its status and any other relevant permitting and approval activities, as required by the FinalOrder.

This is to advise you that plans covering the above captioned project are approved for the plumbingsystem only by the Division of Plumbingas of the date of this letter. The approval type is as listed above.

Inspections must be requested before any part of this plumbing is covered under the floor or concealedin the walls. A fmal inspection and air test must be requested after all plumbing fixtureshave been set. Ifa private sewage disposal system or water system is used they also must be inspected.

Two sets of approved plans have been sent to the project Plumbing Inspector. One set of the approvedplumbing plans shall be on the constructionsite for the plumbing inspectors review and comments.

RE: KYDOW NCI for new Somerset Office

Dear Mr. Anderson:

This letter is intended to clarify the reasons that forms KYRIO (Notice of Intent (NO!) forConstruction Sit^) and the related NOT (Notice of Termination) are not required for yourproject. I have attached the general information sheet for Form KYRIO StormwaterConstruction. Normally the construction contractor is required to file this form electronicallyupon starting construction on a site. However, this form only is applicable where "a disturbanceof one ore more acres" is planned. In the case of the Somerset office project it has beendetermined that only about 28,000 sq. ft. (0.64 acres) of site area is being disturbed forconstruction of the new office foundations and floor. The remainder of the project constructionarea is paved or covered by existing buildings, and the site area there will not be disturbed.

Consequently, the NOI application is not required. Similarly, because the NOI is not filed, theNotice of Termination (NOT) ofthe site disturbance activity is also not required.
