Pipeline Safety Management System

9/17 Goshen Incident Safety Review

• Thank You

• Mechanical couplers discovered on transmission pipeline excavations must trigger an immediate safety reaction

Customer Threats – In the Field

• Remain calm, polite, and non-threatening• Get to a safe location as quickly as possible• Call 911• Contact supervisor and/or company

representative• Supervisor to contact Corporate Security and

complete a “threat” form

Customer Threats – In the Field

Pipe Loading and Unloading – Safety Policy Updates

• The intention is to minimize hazards to workers – no one shall be permitted to be on the load

• Require lift systems with two points of contact (or vacuum system) for loading/unloading (vs. center of gravity sling)

• Reviewing pipe delivery specifications

Hours of Service / Fatigue Management Safety Guidelines

FR Wear Update - Gaseous Environment

Definition:– 20% LEL, 1% Gas – Company employees

and emergency responders only

FR Wear Update - Non Gaseous Environment

Working Definition:An environment where engineering controls or other processes are in place such that no loss of containment is possible or any containment volume lost (either intentionally or unintentionally) is not considered a hazard to personnel

FR Wear Update - Potentially Gaseous

Working Definition: An environment where the potential exists for a hazardous loss of containment; this loss of containment may result from a catastrophic failure or through a controlled process such as blowdown/purging, tapping, welding, or other interaction with active gas pipelines or services

Potentially Gaseous and Gaseous Environment Notes

• When gas is released, constantly monitor the gas levels in the area; establish a public safety perimeter (1926.651)

• Remove all blended material garments – including gloves (wear leather or FR gloves) and UnderArmour-type thermal wear

• Remove all ignition sources, including cell phones and traffic vests

• Man the fire extinguishers upwind
