Type of incident : Occurrence
Incident Time :15:45 Hours
Description of Incident:
The Top disc insulator of B phase of the hoisted insulator string came out from ball of “Ball and
clevis” and the complete string of B Phase bumped to C Phase insulator string, four insulators
were broken because of the impact and broken pieces of ceramics fell on internal road of L&T.
Root Cause Analysis
Human Factors
Violation by supervisor : The supervisor did not ensured the correct locking of “M”
pin prior installation
Improper decision making or lack of judgment: The supervisor & workmen
assumed that desired locking property will be achieved
Physical Factors
Defective tools / equipment: The distorted “M” pin was used for the joint
Recommendations:
1. Check point for checking (by contractor supervisor & random checking by Tata Power
representative) all the locking pins of disc insulator and other hardware accessories
should be added to conductor stringing checklist.
2. All Contractor workmen and supervisors should be trained on how to check locking pin
of disc insulator and how to install it properly. A technically acceptable Good pin should
be available at site as a sample pin for comparison purpose.
3. Project engineering should be asked to check whether changing specification (for
increasing spring tension of M pin, increase of the thickness of the Cu flat of the pin) of
M pin could avoid such incidents.
3. Safety training

Training on LOTO, Knowledge sharing session on working at height, First aid and fire fighting,
CSM, BCP workshop, DMP Awareness, Gas cylinder safety. Apart from Safety Induction
(picture), Safety training & IMS awareness training, ”, Safety training for unit#5 outage, Working
At Height trainings conducted during the month for Contractor employees.

Conducted Weekly, Monthly & quarterly field safety audit at following sites:
1. DCS (WINGVISHNU SHIVAM TOWER, Thakur Village, Kandiwali East (Central
Zone))
2. Transmission (Dharavi & Mahalaxmi)
3. Transmission Projects (Powai GIS, 220KV Salsette - Saki Cable project & Sahar Sub
Station).

Conducted Train the Trainer Courses for Haldia employees on Critical Safety Procedures.
Web based training on Office safety was configured for Carnac, Dharavi, Technopolis &
Noida office based employees.
Conducted Fire risk assessment at DCS.
Safety Convention was held on 5th March 2015 inaugurated by CEO & MD, COO &
Executive Director, CHRO, Chief Culture Officer. Divisional Heads have participated in the
same.

8. Security Highlights
I. Trombay 26 Mar 2015 – An interactive session on crime against women, domestic violence and
women empowerment was organized at junior club by Tata Colony Residents Association
by Security Trombay. Total 150 colony residents including female members & children.
30 Mar 2015 – Mock drill was carried out (Level III) by fire department with Scenario “Fire in
contractor’s shed near U#7, suspect gas fire”. U#7 MO/SCR regarding fire at contractor’s
shed near U#7.
Mock Drill on security scenario, safety briefing and OJT training in March were conducted in
Trombay.
02 Mar 2015 – BCP Stage II audit in view of testing
readiness of the organization and meeting the
challenges of a continuously changing environment
successfully completed at Trombay for ISO
22301:2012 BCMS Certifications.
II. MPL(Maithon)
Security Review meeting was held on 27.03.2015 with the external customer at site office conference hall with the security related issues.
. A training session on use of wireless hand set and procedure of
transmission was conducted for the security personnel.
Checking of Hyvas in the month of March 732 vehicle was arrested by over speeding and
2878 vehicle was checked without tarpaulin/without seal/helper.
