Overview of the Indian Health Diabetes Network
The Indian health diabetes network includes an Area Diabetes Consultant
in each of the 12 IHS Areas, 19 Model Diabetes Programs, 399 Special
Diabetes Program for Indians grant programs, and other diabetes and
technical experts.
Area Diabetes Consultants
The Indian Health Care Improvement Act, amended in 1987, established
that each IHS Area should have an Area Diabetes Consultant to
support a variety of activities.

1. Serve as Project Officers for the Special Diabetes Program for Indians Community-
Directed Diabetes Program.
2. Serve as a liaison between the Special Diabetes Program for Indians grant
programs and clinical staff at IHS, Tribal, and Urban Indian health care facilities.
3. Play a critical role in the coordination of the extensive Indian health system
diabetes network to facilitate bi-directional information flow from local to national
levels.
4. Provide diabetes orientation, training, and monitoring activities to health care
professionals and paraprofessionals.
5. Work with the IHS Division of Diabetes to translate and disseminate the latest
scientific findings on diabetes treatment and prevention to American Indian and
Alaska Native communities.

1. Serve as Project Officers for the Special Diabetes Program for
Indians Community-Directed Diabetes Program:
Support and advise grantees in the preparation of SDPI grant applications*;
Review and approve qualifications of proposed SDPI Diabetes Coordinators;
Review SDPI grant applications*;
Review and approve carryover requests and budget revisions;
Recommend SDPI grant applications for approval in Grant Solutions;
Create the funding memo to fund grants in Grant Solutions;
Assist grantees with other amendments to their SDPI awards;
Provide grantees with review feedback on SDPI grant applications*;

progress reports and Best Practice required key measure data into SOS;
Support and advise grantees in the preparation and submission of their
Annual Diabetes Data Audit;

2. Serve as a liaison between the Special Diabetes Program for Indians
grant programs and clinical staff at IHS, Tribal, and Urban Indian health
care facilities.
Direct grantees where to find information and the appropriate resource contact.
Coordinate with grantees’ designated specialist at DGM
Liaise with appropriate DDTP support personnel in the Diabetes Audit Support
Team, the SDPI Outcomes System and Mandatory Training Programs
Identify appropriate resources for programs in RPMS, EHR, ICARE, DMS etc.

3. Play a critical role in the coordination of the extensive Indian health
system diabetes network to facilitate bi-directional information flow
from local to national levels.
Help to ensure that grantees are aware of new program requirements, changes,
system updates, deadlines, resources, clinical advances etc;
Ensure that grantees’ concerns, problems, opinions and feedback are directed to
the appropriate national resource at DGM or DDTP, if the issue cannot be
addressed locally
Participate in monthly ADC Calls with DDTP Director and Staff
