SECTION 1 - GENERAL INFORMATION

1.1. Introduction. The Decorah Community School District, (the “District”), is requesting proposals (“Proposals” or “Responses”) from qualified proposers in good standing (“Proposers”) to provide proposals on the upgrade and or replacement of the District’s Security and Surveillance camera system along with a proposal of equipment and installation costs (the “Services”)

1.1.1. Intent. It is the intent of this Request for Proposals (this “Request”) to set forth the minimum acceptable requirements for Responses to this Request.

1.2. Background and Detailed Description of Services. The District currently has a security camera system that is in need of upgrading and expansion. It will be the responsibility of each proposer to inspect the District’s current camera system and determine to most cost effective approach to upgrading or replacing the system, and the integration of additional cameras to the security and surveillance system. Locations of cameras and details of the requirements are described throughout this RFP and in Appendix A and accompanying building maps.

1.3. Proposal Requirements. One (1) unbound original copy, and a searchable electronic pdf copy of the Response are required to be submitted to Kurt MJ DeVore, at the address listed below, no later than 12:00 p.m. on Wednesday, March 1, 2017. Guidelines for Responses are described throughout this RFP. Any response, modification, or amendment received after the due date and time is late. No late response, modification, or amendment will be accepted. Proposals must be submitted in a sealed envelope and must be signed by an authorized representative of the Proposer and include the following:

1.3.1. Introductory Letter. An introductory letter expressing an interest in providing the Services should be included. The introductory letter should be addressed to:

1.3.2. Qualifications. Describe, in sufficient detail, the Proposer’s key personnel (including a brief resume) and Proposer’s experience, qualifications and technical support that are relevant to the Services detailed in this Request.

1.3.3. Hardware Features and Configuration. Describe in detail the proposers assessment of the equipment and camera system currently being deployed by the District; and the proposed upgrade and hardware expansion features and configuration of the security/surveillance camera systems.

1.3.4. Full Service. Describe, in sufficient detail, all the Services proposed to be provided, making specific reference to any Services not included in the Proposal, and Services proposed to be provided by subcontractors and/or any additional services proposed to be provided and the cost of such additional services, if any.

1.3.4. Cost and Installation Schedule. Detail the proposed installation schedule and all-inclusive cost, including fully loaded hourly cost of labor, equipment, materials for the Services, and subcontractors’ markup, which includes a commitment and an affirmative statement that all the proposals made in the Responses shall remain valid for a period of 120 days after the due date of the Responses.

1.3.5. References. Include the name, address, and contact person of at least three government agencies with which the Proposer has provided similar type Services. It is the intent of the District to contact such references to assess the level of satisfaction with the equipment and services rendered to the listed references. List all lawsuits and complaints filed against Proposer within the past three years.

1.3.6. Dates. A list of important dates and times is described in Appendix B.

1.4. Identification of Anticipated Potential Problems. Proposals should identify and describe any potential problems with respect to providing the Equipment and Services to the District.

1.5. Evaluation Criteria and Scoring Process. A Review Panel (referred to hereinafter as “Review Panel”) will review all Proposals received. Each of the evaluation criterion has been listed below. The criteria are as follows and the scoring rubric is located in Appendix C:

1.6. Selection. Interviews may be conducted with Proposers determined by the District to be reasonably susceptible of being selected for the award. In addition, one or more Proposers may be invited to make presentations. However, Proposals may be accepted without discussion or interview. It is the intent of the District to use the above criteria in the evaluation of all proposals, however, the District reserves the right to modify the above criteria as deemed appropriate by the Review Panel. If such modification occurs, each Proposer being interviewed will be notified at least twenty-four (24) hours prior to the interview of the revised criteria. Based on the results of discussions, if any, interviews, if any, and proposal scoring, the Proposers will be rated by the Review Panel.

1.7. General Information. The District reserves the right to amend, modify or waive any requirement set forth in this Request. Response to this Request is at the Proposer’s sole risk and expense. All Proposers must comply with applicable Federal, State, and local laws and regulations. Except for written responses provided by the contact person described below, the District has not authorized anyone to make any representations regarding the subject matter of this Request. All requests for clarification or additional information regarding this Request must be submitted in writing to the contact person described below no later than February 15, 2017 for clarification or additional information, and if the contact person deems, in his sole and absolute discretion, that such response is of general applicability, his response, if any, will be posted on the District’s website at http://www.decorah.k12.ia.us/district_info/rfp (which constitutes a written response). Entities responding to this Request are encouraged to review this website frequently. The District anticipates selecting one of the responding Proposers, but there is no guaranty that any responding Proposer will be selected. Responses will be placed in the public domain and become public records subject to examination and review by any interested parties in accordance with the Iowa Code 2017, Section 22. All materials submitted in response to this Request will become the property of the District and will be managed in accordance with Iowa Code. A mandatory pre-proposal conference will be held on Monday, February 6, 2017 at 10:00 a.m. in the Board Room of the District, 510 Winnebago Street, Decorah, IA 52101.

1.8. Special Matters. All Services performed pursuant to this Proposal shall comply with all applicable laws, ordinance, rules, regulations, and applicable standards of performance, including, but not limited to, status verification system requirement for contractors of the State of Iowa.

1.9. Terms of Contract. The successful Proposer will be required to enter into a written agreement with the District to provide the Services. If the selected Proposer and the District are unable to negotiate an acceptable agreement, then another Proposer(s) may be selected and negotiation will continue with such other Proposer(s) until an acceptable agreement is completed.

SECTION 2 - TECHNICAL REQUIREMENTS

General Requirements

The proposed solution will:

2.1. Camera Coverage. Provide full camera coverage of all areas as designated on attached Maps. Note that camera locations in the attachment are [NOT] meant to designate individual camera locations but to serve as a guide. Walkthroughs are required so that proposals adequately provide cameras to match the school’s needs. Specific camera/types ie. Degree of angle, should be suggested by the Proposer based on the location to be covered if different than that listed below.

2.2. Cabling. Include the necessary Berk-Tek LANMark 1000 or better specified cabling to connect the IP cameras back to the appropriate MDF or IDF closet, not to exceed industry standard length. The IP cameras will be integrated into the existing network. All cameras need to be positioned with owner during installation for proper coverage areas. If additional cameras are needed to provide adequate coverage as described in the RFP they will be added at cost. Deviation from Berk-Tek LANMark 1000 cable, without prior written consent from the Director of Technology of the District, will result in all cable being removed and replaced at NO expense to the District.

2.3. Warranty. Include 3-year parts/labor warranty or better on cameras and 20-25 years parts/labor warranty on the cabling inclusive of the manufacturer warranty once cable tests are submitted. Also provide as an alternate, the extension to a 5 year warranty if available for the cameras.

2.4. Camera Specifications. All Interior IP Cameras shall be Axis P3364-LV and Exterior IP Cameras shall be ArecontVision-AV12176DN-28 or P3364-LVE if noted on the plans. There are additional interior cameras which shall be Interior ArecontVision-AV12176DN-28.

2.5. Network Switches. All network switches shall be Brocade with 2 stacking cables and dual power supplies. Any Proposer providing the proposed switch(es) must be able to program them with specifications provided by the District. This may be an unnecessary item and determined at the time of the

Network requirements

2.7. Network Design. The submitted design should avoid any single points of failure in order for the IP camera’s to perform at an optimal level. All TIA/EIA 568 standards shall be followed. The District shall be consulted prior to installation and a final walk through shall be performed with District Director of Technology. Any changes shall be mutually agreed upon.

2.8. Number and Location of Cameras. The purpose of this RFP is to provide the District with full camera coverage. The vendor will perform a site survey to verify the ideal locations for cameras and to insure that all necessary coverage is provided for in the Proposal. General locations for cameras are specified in Attachment A.

2.9. Cabling. All cameras shall be POE and shall be wired to the most convenient MDF or IDF as outlined on Attachment A.

2.9.1. Installation. Cables shall not be tie wrapped or routed along electrical or gas conduit. Horizontal cable run in hallways above a suspended ceiling shall be in J hooks or cable tray. Once in work areas it may be secured to the wall directly above the suspended ceiling or to the concrete ceiling structure every three feet by cable hangers. Cable ties must be trimmed off cleanly at a locking hole. Cables shall be secured at every corner. Cables shall be run in a uniform fashion and shall not be woven among other utilities. Existing cable pathways shall be utilized or created with the consultation of the District Director of Technology.

2.9.1.2. Terminations. Field terminations shall be enclosed in a TracJack® Surface Mount Box, 2-Port, Fog White. Ortronics field jacks shall be Clarity 6 TracJack® FogWhite and the MDF & IDF jacks shall be Clarity 6 Panel Jack®, Rear Load, T568A/B, BLACK. Ortronics angled Patch Panels are required when necessary. Verify with the District during Pre-proposal meeting.

2.9.1.2. Service Loops. Service loops of fifty four (54) inches shall be placed above each ceiling at the field cable location and a minimum of ten (10) feet or more at each MDF or IDF. Existing service loop pathways shall be followed. Any cable without the above specified service loop will be required to be replaced and installed correctly. Deviation from this will result is the cable being removed and replaced.

2.9.2. ANSI/EIA/TIA. All internal building network infrastructures shall adhere to ANSI/EIA/TIA 568B (Commercial Building Telecommunications Cabling

Standards), ANSI/EIA/TIA 569 (Commercial Building Standard for Telecommunications Pathways and Spaces), and National Electric Code standards.

2.9.3. Penetrations. Penetrations through floor and fire-rated walls shall utilize intermediate metallic conduit (IMC) or galvanized rigid conduit (GRC) sleeves and shall be fire-stopped after installation and testing, utilizing a fire-stopping assembly approved for that application.

2.9.4. Labeling. Labeling shall conform to ANSI/TIA/EIA-606 standards. In addition, provide the following:

2.9.4.1. Outlet labeling. Label each outlet with permanent self-adhesive label with minimum 3/16 in. high characters. Labeling shall also conform with the District labeling schema.

2.9.4.2. Cable Labeling. Labeling shall also conform with the District labeling schema. Label each cable with permanent self-adhesive label with minimum 1/8 in. high characters in the following locations:

2.9.4.2.1. Field End Labeling. Inside receptacle box at the work area.

2.9.4.2.2. MDF/IDF Labeling. Behind the communication closet patch panel or punch block.

2.9.4.3. Use labels on face of data patch panels. Provide facility assignment records in a protective cover at each telecommunications closet location that is specific to the facilities terminated therein.

2.9.4.4. The labeling for data drops should include the IDF letter, then the patch panel number and the two or three digit number of the port on the patch panel.

2.9.4.5. Camera Labeling. All interior cameras shall be affixed with the corresponding manufacturer identification sticker and a permanent self-adhesive label with minimum 3/4 in. high characters conforming to the district labeling schema. Please verify labels and locations on camera housing with the District Director of Technology.

Example: Interior camera 1 label: 17-01 & A107 (17 = Year, 1 = Camera) (A = Closet, 107 = Patch Panel number)

2.9.5. Testing. Testing shall conform to TIA/EIA TSB-67 Transmission Performance Specifications for Field.

2.9.6. Testing UTP. Testing of Unshielded Twisted Cabling Systems as well as ANSI/TIA/EIA-568-A-1 Propagation.

2.9.7. Delay and Delay Skew Specification. Delay and Delay Skew Specification for 100 ohm 4-pair cable. Testing shall be accomplished using level II field testers.

2.9.8. Testing Failures. Test each pair and shield of each cable for opens, shorts, grounds, and pair reversal. Correct grounded and reversed pairs. Examine open and shorted pairs to determine if problem is caused by improper termination. If termination is proper, tag bad pairs at both ends and note on termination sheets.

2.9.9. Cable Replacement. Perform testing of copper cables with tester meeting TIA/EIA TSB-67 and ANSI/TIA/EIA- 568-A-1 requirements. If copper cables contain any bad pairs, remove and replace the entire cable.

SECTION 3 - ADDITIONAL REQUIREMENTS

The proposed solution will:

3.1. Financial. Provide quotes detailing your solution for the District. The quote should include all necessary hardware, cabling, and software needed to complete a full installation. The vendor is responsible for due diligence and a site survey before submitting a Proposal. If additional information is needed to figure this amount, please contact the Director of Technology.

3.4. Warranty. Warranty terms on the camera hardware and any associated software should be clearly defined and must include a parts warranty to cover all components for a minimum of 3 years.

3.5. Delivery and Installation. Any delivery and installation charges should be clearly defined as separate line items.

3.6. Documentation. ALL test results shall be submitted as electronic and hard copy to the District.

3.6.1. Cable Documentation. All cable test results shall be submitted to the manufacturer for cable warranty as well as to the District once the warranty is received from the cabling manufacturer. A 50% retention on cable and cabling labor shall be withheld until such time as manufacturer warranty documents are received by the District.

3.6.2. CAD. All structured cabling documentation shall be provided to the district in a CAD format and 18” x 24” paper copies. All cable locations shall be denoted on the As-Built drawings using the District labeling. All Cameras will be indicated on the drawings with corresponding number in conjunction with field labeling.

3.6.3. Additional Documentation. Please see Appendix D for camera inventory documentation to be submitted to the District in electronic (Microsoft Excel; .xlsx) and on 11” x 17” paper format.

3.6.4. Final Documentation and Submissions. Documentation shall be submitted to the District in an organized and professional fashion. Electronic submissions shall be contained and organized on a single flash drive and submitted to the district. Paper copies shall be contained and organized in labeled three ring binders and submitted to the District. As-Built prints shall be stapled and bound and submitted to the District.

Section 4 - Proposal Process

The proposed solution will:

4.1. Proposal Evaluation. The proposals will be evaluated by the District. The District, at its sole discretion, will determine whether to hold discussions with the Proposers who are in a “competitive range” or to recommend the Contract be awarded without discussion on the basis of the proposal submitted. The evaluation criterion include the following:

• Bidder's total proposed price • Quality of previous work done for the District.• Bidder's support/service • Bidder's warranty/maintenance • Bidder's qualifications/experience

4.2. Proper Submission of Proposal. The proposal must be submitted and received by the District by (Noon) 12:00 P.M. Wednesday, March 1, 2017. It is the responsibility of the proposer to ensure proper and timely delivery of all required material. Faxed, e-mailed or late submissions will not be considered.

4.3. Comprehensiveness of Scope of Services Provided. The District’s evaluation of the overall capabilities of the proposer to meet the required service levels described in this RFP.

4.4. Overall Project Costs and Fees for Equipment Procurement. The amount of the standard charges and overall proposed contract pricing will be a determining factor in award of the agreement. All equipment, hardware, parts, and accessories

in the proposal MUST be itemized line by line, part by part. Failure to complete this task will result is the Proposal being disqualified. Change orders are not expected and are should not be expected to be approved by the District due to errors or omissions on the part of the Proposer. Inflation of the Proposal to cover errors and omissions on the part of the Proposer shall not score favorable in the scoring rubric Appendix C. The Project Cost and Fees Submission Example is in Appendix E.

4.5. Service Enhancements. The firm or organization’s efforts to understand the District’s needs and goals.

4.6. Other Factors. Any other factors the District believes would be in the best interest of the District to consider, which were not previously described.

Section 5 - Final Comments

5.1. The District reserves the right to reject any and all proposals, cancel all or part of this RFP, and waive any minor irregularities and to request additional information from proposing firms or organizations. By requesting proposals, the District is in no way obligated to award a contract or pay expenses of the proposing firms or organizations in connection with the preparation or submission of a proposal.

5.2. The District’s decision to award a contract will be based on many factors including but not limited to service, cost, equipment specifications, system design and innovation. No single factor, such as cost, will determine the final decision to award.

5.3. The successful Proposer will be required to secure and maintain appropriate insurance coverage acceptable to the District. Proof of such coverage, in the form of a broker-issued certificate, must be received by the District prior to the beginning contract date.

5.4. The District truly appreciates the effort all the firms, organizations and proposers and their respective staffs have put forth in responding to the IP Security and Surveillance Camera System Request for Proposal.

5.5. The successful Proposer shall comply with all applicable laws, rules, regulations and policies in performing the work outlined in this proposal.

5.6. All proposals become the property of the District and will not be returned to the Proposer. At the conclusion of the selection process, the contents of the Proposals will be placed in the public domain and be open for inspection by interested parties. Trade secrets or proprietary information that are recognized as such and are protected by law may be withheld upon the written request of Proposer.
