FREQUENCY

The test may only be performed during plant shutdown (modes 5 or 6) when

DISCUSSION

The manually operated switches involved in the initiating, blocking and/orresetting of protection and safeguards functions work in any or all of fourways. These are: 1) contacts directly in the circuitry of the equipment,2) contacts directly to the master relays in the SSPS, 3) contacts to theelectronic logic circuitry in the SSPS and 4) contacts directly to theunlatching coils of the slave relays, as follows:

1. The correct functioning gf the contacts directly in the circuitry ofequipment is determined by observing the actual operation of theequipment.

2. The correct functioning'of the contacts that are directly involved inmaster relay operation is determined in the following manner. Thereare two ways that a slave relay can be activated. One is through themaster relay with which it is associated and the other a parallelcircuit through the Safeguards Test Cabinet. As the slave relays areactually energized during the use of the Safeguards Test Cabinet, itis therefore only necessary to prove the manual switch circuitrythrough the master relay to or beyond the common points of the twoparallel circuits. This is done by monitoring the bottom coil(terminal 2) of the slave relays in question with the output relaycabinet in the "TEST" mode and the master relay selector in the "OFF"position.

3. The correct functioning of the contacts that introduce a signal to theelectronic logic is verified by manually introducing a signal from thecontrol room switches and determining that the proper potential isreceived at the logic input. Operation of the logic is tested usingthe semi-automatic tester in the SSPS (STP I-16A or I-16D) whichapplies the same input signals to the logic as the control roomswitches.

4. The correct functioning of the contacts that unlatch the slave relaysis verified by observing that the proper unlatching potential isapplied across the unlatching coil. 'The physical unlatching of thecoils is demonstrated during individual slave relay testing throughthe Safeguards Test Cabinet, or other testing.

As this test involves only the two basic logic conditions of "True orFalse" it is not necessary to use calibrated voltmeters. The values givenin the procedure are referenced to the logic state rather than to precisevoltages.

DOCUMENTATION AND REPORTING

1. Sumary Sheet

A summary sheet is provided which is intended to serve as a "shopfollower" for the test. This sheet shall be filled out each time thetest is performed, and shall be filed with the data sheet package forthe test. The sumary sheet has several basic functions:

a. To allow the technician and inspector (if any) to documentcertain important findings and/or the completion of certainimportant steps in the procedure. In particular, provision ismade on the form to document the completion of the importantsteps in the removal from service and reinstatement.

At the bottom of the form is a section entitled "Summary of MajorFindings." This should be filled out by the technician, and isintended to be a brief summary of important, unusual orunexpected items, such as breaker contact remains closed, switchreplaced, Plant Problem Report filed, etc. This is intended onlyto be a convenient summary to aid the reviewer of the test. Itis not intended to be a comprehensive test report. More detailedinformation would be found on the test data sheets, maintenancehistory cards, Plant Problem Reports or in other special reportsprepared for the test.

b. To assist the reviewer(s) of the test results by directing theirattention to the important findings. The tests are ordinarilyreviewed shortly after completion by a cognizant supervisor whoapproves the data sheets and maintenance cards.

2. Data Sheets

a. The verification of each required check will be entered on theprovided data sheets (Form 69-9439). Other items which should beincluded, as appropriate, are:

. 1) Logic functions which do not operate properly should becircled in red to assist the reviewer.

2) Any apparent discrepancy which does not have an obviousexplanation should be explained under "Remarks" to assistthe reviewer.

b. Following completion of the test, the data sheets will bereviewed for conformity with requirements and signed by theInstrument Supervisor or other knowledgeable supervisor.

c. The data sheets are filed in the surveillance test file.

3. Additional Reporting and,Documentation Requirements

a. If there are no malfunctions, complete the data sheets and submitthe summary sheet, data sheets and the dated IBM schedule cardfor STP I-16C to the Instrument Supervisor. He, after approvingthe data sheets, will forward the IBM schedule card to the TestCoordinator to indicate completion of the test.

b. If at any time during the test any logic function fails toperform properly, notify the Instrument Supervisor immediatelyand do not continue with the test until approval has beenobtained from the Instrument Supervisor. The InstrumentSupervisor shall promptly notify the Shift Foreman that thechannel is inoperable. He shall also notify the Power PlantEngineer at the earliest convenience. Initiate a Plant ProblemReport.

PRECAUTIONS

Technical Specification 3.9.9 requires that the Containment Ventilationisolation system shall be operable while in refueling operations, duringCORE ALTERATIONS or movement of irradiated fuel within the containment.During this test the Containment Ventilation isolation system shall be made

inoperable to comply with the functional testing requirements of the manualinitiation per Tables 4.3-1 and 4.3-2. Therefore the action statement forspecification 3.9.9 shall be taken prior to performance of this test if inmode 6. That is:

With the Containment Ventilation isolation system inoperable, close each ofthe ventilation penetrations providing direct access from the containmentatmosphere to the outside atmosphere. The provision of Specification 3.0.3and 3.0.4 are not applicable.

PREREQUI SITES

All testing and/or maintenance must be performed under a validin-plant clearance. The clearance request should be submitted atleast 36 hours in advance to permit scheduling of related activities.NOTE: The clearance request should include prerequisite 2 and 3.

As part of this procedure it is necessary to deactivate the slaverelays on both trains A and 8 for the duration of the test, therefore,the Shift Foreman should be aware of this such that the status of theplant will permit this condition. See precautions.

If in Mode 5, have the CRD MG set input breakers (52-13D-7 and52-13E-3) opened and MOL tags hung.

All reactor trip breakers are available for test.Have available the following test equipment:

Three voltmeters. No ca1ibration is required for these meters as theyare used to verify a logic state rather than a precise value.

Establish communications between the reactor trip breakers, thecontrol room and the train being tested for the duration of the test.NOTE: This test will require a minimum of 2 instrument technicians.One operator is required for testing of the reactor trip breakers.

PROCEDURE

Removal From Service

NOTE 1: All steps refer to the SSPS Train A unless otherwise noted.When testing Train B the variables in parenthesis, where appropriate,must be used.

NOTE 2: Verification of the satisfactory completion of specifiedsteps in the removal from service and reinstatement is to be recordedon the summary sheet for this test (Form 69-9439). The technicianperforming the test shall legibly initial the verified space providedon the summary sheet adjacent to the step just performed labeled I&C.The verifying operations person shall legibly initial the verifiedspace provided on the summary sheet for which verification isrequired. Verification spaces labeled N/A do not require operationsperson's verification.

a. Preliminary Check as Follows:

1) Verify that the proper clearances have been obtained fromthe Shift Foreman and record the clearance number on thesummary sheet.

2) Obtain the final OK to perform the test from the ShiftForeman and obtain the keys to the SSPS, Train A and B.Initial the summary sheet.

3) Notify the Control Operator that the SSPS is about to betested. Initial the summary sheet.

4) Verify that both control rod drive M.G. sets are cleared andnot running. Initial the summary sheet.

b. Removal as follows:

1) In Train A(B) place a fused jumpter (2 Amps) between TB507-2and TB508-4 (in Logic Cabinet). Initial the summary sheet.

NOTE: This will bypass both the manual switch and generalwarning inputs to the UV trip coils.

2) In Trains A 8' Output Relay Cabinets transfer the MODESELECTOR switches to the "TEST" position, and hang a CAUTIONtag. Initial the summary sheet.

3) In Train A and B Logic Cabinets, put the INPUT ERROR INHIBITswitch in the "INHIBIT" position, and hang a CAUTION tag.Initial the summary sheet.

4) Check that the MASTER RELAY SELECTOR switches in both Trainsare in the "OFF" position. Initial the summary sheet.

5) Check that the UV COIL VOLTAGE in the Logic Cabinet isapproximately 48 volts. Initial the summary sheet.

2. Manual Reactor Trip

a. REACTOR TRIP switch on the control console to the shunt tripcoils.

1) Have operator rack in the following breakers, in accordancewith OP B-6.

52/RTA (B) - Reactor Trip Train A(B).52/BYB (A) - Reactor Trip Bypass Train B(A).

2) Have operator close breakers 52/RTA (B) and 52/BYB (A).Record on the data sheet.

3) Verify an'nunciator "Reactor Protection Signals" (PK 04-12)is clear. Record on the data'sheet.

4) Continuously monitor that the UV COIL VOLTAGE remains at 48volts on Train A(B) while Step 2.a.5) is performed. Recordon the data sheet.

5) Turn the REACTOR TRIP switch on the control console to the"TRIP" position, verify that the "Reactor ProtectionSignals" annunciator (0412) is lit, then let the switchreturn to its neutral position. Record verification ofannunciator operation on the data sheet.

6) Verify that annunciator typewriter first printed out "ManualReactor Trip" in red then in black. Record on the datasheet.

7) Verify that the computer read out "Y00040 REAC MANUAL TRIPSW-1" first in red, then in black. Record on the datasheet.

8) Verify that breakers 52/RTA (B) and 52/BYB (A) are in thetripped position. Record on the data sheet.

9) Turn the REACTOR TRIP switch on the control console to the"RESET" position, then let it return to the neutralposition.

10) Verify that breaker 52/RTA (B) is closed. Record on thedata sheet.

6) Verify that computer read out "Y0005D REAC MANUAL TRIP SW-2"first in red then in black. Record on the data sheet.

7) Verify that breakers 52/RTA (B) and 52/BYB (A) are in thetripped position. Record on the data sheet.

c ~ REACTOR TRIP switch on the control console to the UV coils.

1) In Train A(B) transfer the jumper from between TB507-2 andTBSOB-4 to between TB507-4 and TB508-4. Record on the datasheet.

2) Verify that the UV COIL VOLTAGE in Train A(B) isapproximately 48 volts. Record on the data sheet.

3) Turn the REACTOR TRIP switch on the control console to the"TRIP" position and hold it in the "TRIP" position.

4) Verify that the UV COIL VOLTAGE in Train A(B) isapproximately zero volts. Record on the data sheet.

5) Let the REACTOR TRIP switch on the control console return tothe neutral position.

6) Verify that the UV COIL VOLTAGE in Train A(B) returned toapproximately 48 volts. Record on the data sheet.

d. REACTOR TRIP switch on VB-2 to the UV coils.

1) Turn the REACTOR TRIP switch on the vertical board VB-2 tothe "TRIP" position and hold it in the "TRIP" position.

2) Verify that the UV COIL VOLTAGE in Train A(B) isapproximately zero volts. Record on the data sheet.

3) Let the REACTOR TRIP switch on the vertical board VB-2return to the neutral position.

4) Verify that the UV COIL VOLTAGE in Train A(B) returned toapproximately 48 volts. Record on the data sheet.

e. Verification of P-4 (Reactor trip breaker trip contact interlockto ESFAS)Verification of P-4's accomplished in STP I-16E.

3. Containment Isolation Phase B

a ~ CONTAINMENT ISOLATION PHASE B ACTUATE TRAINS A AND B SWITCHES

1) In Train A(B) connect the negative lead of a vol tmeter toone of the terminals on TB642 or 656 (these are the twobottom TB's on the left side of the output cabinet and allterminals are common).

2) Set the voltmeter on DC to monitor +15 volts and zero volts.

3) With the positive probe verify that there is +15 volts DC onthe lower right coil (terminal 2) on the following slaverelays.

4) Turn both CONTAINMENT ISOLATION PHASE B ACTUATE TRAINS A ANDB switches (VBl) to the "ACTUATE" position simultaneouslyand hold them in the "ACTUATE" position.

5) With the positive probe of the voltmeter verify that thereis zero volts DC on the lower coil (terminal 2) on the sameslave relays as in Step 3.a.3). That is:

6) Release the two CONTAINMENT ISOLATION PHASE 8 ACTUATE TRAINA AND B switches.

7) Repeat Step 3.a.3) to verify the master relays havedeenergized. Record on the data sheet.

b. CONTAINMENT SPRAY TRAIN A(B) RESET pushbutton

1) In the Logic Cabinet turn the FUNCTION SELECTORS LOGIC Aswitch to position "23" and LOGIC B switch to position "18".(Hi-Hi Contain. Press., Spray Actuation and Phase B).

2) Simultaneously depress any two of the four MANUAL INPUTFUNCTION TEST pushbuttons and maintain them in the depressedposition through Step 3.c.7).

1. With a voltmeter set to monitor 125 volts AC verify zero volts ACacross the reset (upper) coil (terminals 3 8 4) of slave relays:

m. Release the CHANNEL I and CHANNEL II, MANUAL INPUT FUNCTION TESTpushbuttons.

n. Return the LOGIC A switch to the "OFF" position and the LOGIC Band C switches to the position "24". Record on the data sheet.

o. Remove the negative lead of the voltmeter that was connected toone of the terminals of TB642 or 656. Record on the data sheet.

Verify the MULTIPLEXER TEST switch for Train A and Train B is inthe "NORMAL" position. Initial the summary sheet.

c. Verify that the switches on the Logic Test Panel for Train A andTrain B are set as follows:

1) LOGIC A at "OFF"

2) LOGIC B at "24"

3) LOGIC C at "24.".

4) LOGIC D at "24"

5) PERMISSIVES at "OFF"

6) MEMORIES at "OFF"

7) BLOCKING FUNCTION TEST at either position

8) MULTIPLEXER TEST switch in "NORMAL"

Initial the appropriate column on the summary sheet.

d. On the Output Relay Test Panel for Train A and Train B verifythat the MASTER RELAY SELECTOR is in the "OFF" position. Initialthe sumIary sheet.

e. Transfer the MODE SELECTOR switch in the Output Test Panel forTrain A and Train B to the "OPERATE" position and observe theirrespective green lights are lit. Initial the summary sheet.

NOTE: Manual Safeguards actuation induced by operation ofswitches is now possible. Automatic actuation on Safeguardschannels input is impossible as the inputs are inhibited.

NOTE: The preferred position during the Modes 5 and 6 when notmoving fuel, or core alterations is in "TEST". This makes theSSPS incapable of causing safeguards actuation.

*In in "TEST," hang a caution tag on the MODE SELECTOR switchstating that S. I. blocks must be reinstated before placing in"OPERATE." This may be done by performing Steps 1 thru 10 ofSTP-16D3.

CAUTION: If moving fuel or during core alterations theContainment Vent Isolation must be operable (Requires SSPSOPERABLE). Therefore the following conditions MUST be met:

Return both Train A and Train B INPUT ERROR INHIBIT switches tothe "NORMAL" position. Remove CAUTION tag. Initial the summarysheet.

In Train A(B) remove the jumper from between TB507-4 and 508-4.Record removal of jumper. Initial the summary sheet.

In Train A(B) verify the UV Coil Voltage at 0 volts. Initial thesummary sheet.

Verify "GENERAL WARNING" lights are out on both Trains A&B if theMODE SELECTOR switches are returned to the "OPERATE" positions.Initial the sumary sheet.

Close and lock all'.SSPS cabinet doors. Initial the summarysheet.

Report OFF to Shift Foreman and have Operations reestablishnormal plant conditions relative to containment ventilationisolation and rack out the reactor trip breaker 52/RTA(B) andbypass breaker 52/BYB(A) in accordance with Operating ProcedureB-6.

Inform the Shift Foreman of the following SSPS switch positionsand return key for Solid State Protection. Initial the summarysheet.

I) MULTIPLEXER TEST switches - "NORMAL" (both trains)

2) INPUT ERROR INHIBIT switches - "NORMAL" (both trains)

3) MODE SELECTOR - as per his instruction (both trains)

Sign data sheets and turn them over to the Instrument Supervisorin charge for review and filing.
