OverviewThis form is to be used to apply for a reference from the NSW Department of Planning and Environment to be provided to SafeWork NSW in connection with an application for a blasting explosives user’s licence (BEUL) for mining classes.

The guide to applying for a reference from the NSW Department of Planning and Environment - blasting explosives user’s licence (mining classes) (the guide) includes information about applying for and eligibility to be issued with a reference and the processes involved in assessing whether a person is eligible for a reference.

NOTE: This is not an application for a BEUL. BEULs are issued by SafeWork NSW under the Explosives Act 2003. The process for applying for a BEUL (including application forms) can be found on the SafeWork NSW website. SafeWork NSW requires an applicant for a mining BEUL to lodge a reference from the NSW Department of Planning and Environment to demonstrate the applicant’s qualifications and experience.

InstructionsYou must read the guide before completing this application form. Please contact your local NSW Department of Planning and Environment, Resources Regulator office if you require assistance in completing this form (see contact details in section 9 below).

Your written application must be accompanied by supporting documents to evidence that you satisfy each of the requirements outlined in this form.

Sections 1-5, 7 and 8 of the application can be completed electronically however section 6 must be completed in your own handwriting. All sections can be submitted by email, mail, delivered or fax (see section 8 for your local office).

A NSW Resources Regulator inspector or Resources Regulator officer will verify the details of your application and arrange for an assessment of your skills and knowledge. This assessment can take the form of a practical, oral and/or written evaluation, prior to you being issued with a reference.
