Allegation Summary:1. The named resident was assessed as a fall risk and the resident required a for ambulation. When interviewed thenamed resident stated fell when attempted to ambulate without requesting assistance from staff. said started to hurt two hours after the fall and continued to hurt until taken to the hospital for strain over 12-hours afterthe fall. When interviewed, the provider stated he did not immediately notify the named residents physician or representative.He stated he did not make the required notifications until he noticed bruising on the named residents thefollowing day.
