Our Structure and Governance The Burns Trauma & Critical Care Research Centre (BTCCRC) was established in 2004 in response to the Bali bombings. The devastation and tragedy of this event focused national and international attention on burns victims and emphasized that there was a critical need to improve the early and on-going treatment and outcomes of burns and critically ill patients. The outstanding response from the critical care, burns and anaesthetics teams of the Royal Brisbane & Women’s Hospital (RBWH) highlighted the skill and expertise we possess in this area. Our Centre is directed by Prof Jeffrey Lipman, chaired by Prof André van Zundert, Professor of Anaesthesia and managed by research manager Dr Sia Athanasas. The stability of the BTCCRC exists by virtue of our major supporter and partner, the RBWH Foundation and our partners The University of Queensland (UQ), the Queensland Government and the Australian Defence Force. The research within the centre is continually expanding and is divided up into five core themes.

Critical Care - Management and outcome of critically ill, head injury, renally dysfunctional and long stay ICU patients.

Drug Dosing and Pharmacokinetics - Pharmacokinetics, drug distribution and antibiotic resistance in critically ill patients. We have

laboratories across two campuses, within the Antimicrobial Optimisation (AMO) Group at RBWH and within the Centre for Translational

Anti-infective Pharmacodynamics (CTAP) at PACE, PAH.

Burns - Management and outcome of burns patients encompassing prevention, acute injury, long term outcomes and skin culturing.

Anaesthesia - Evaluating new procedures and safety of procedures for anaesthetising critically ill and difficult to anaesthetise patients.

Military & Trauma Medicine - Managing pre-hospital care, trauma and rehabilitation medicine.

MISSION AND AIMS

Our Mission The BTCCRC was established to further advance and improve the level of treatment and care given to burns and critically ill patients through world-class research-based clinical practice. It began as a collaborative venture between the RBWH Burns and Intensive Care Units, UQ and has incorporated the Department of Anaesthesia. In recent years the centre has undertaken research into military medicine.

Our Aims The main aims of our research centre are to:

Improve both the survival and outcome of patients by conducting high quality research in the area of Burns, Trauma, Critical Care, Military Medicine and Anaesthesia in general.

Provide a research-driven evidence base that informs clinical practice and policy in the above research themes.

Establish local, national and international collaboration in these areas thereby increasing research opportunities.

Enhance the research training and knowledge base of postgraduate students and staff in the above areas.

50 Years of Internet – A milestone breakthrough to connect scientists all over the world Traditional research has changed drastically over the last 50 years with the introduction of the Internet, supercomputers which can handle massive data, modern search machines and smartphones. The Internet is easy to use and gradually took over the global communication landscape, including the emerging global phenomenon of mobile leapfrogging and its importance for research access and dissemination. It all started on 21 November 1969, when two minicomputers connected the University of California (UCLA) and Stanford Research Institute (SRI) in Palo Alto. This ARPANET (Advanced Research Projects Agency Network), funded by the US Department of Defence, initial ly meant to exploit new computer technology to meet military needs. However, it was also born out of frustration as there were only a limited number of powerful research computers available, with researchers geographically separated from them. More computers were connected and within no time the US East Coast was reached. Norway was the first country outside the USA to be connected to the ARPANET in 1972, with a transatlantic satellite link between the Norwegian Seismic Array (NORSAR), while a terrestrial circuit added a London IMP. Soon, the whole world would follow with an unprecedented network, providing almost unlimited exchange of data. Internet technology leapfrogged with ARPANET from being an idea to connect all communication networks together, providing a network of networks. The Internet was born. The term ‘Internet” was reflected in ‘Internet Transmission Control Program’, a short term for internetworking. Computer-scientist Tim Berners-Lee who worked for CERN (Switzerland) invented the World Wide Web in 1989, making it possible to link hypertext files into an information system that could be accessed from any node on the network using a web-browser and web-based applications (apps). A major technical improvement was the release of the first mobile phone with Internet connectivity in Finland in 1996, the beginning of the ‘smartphone’ era. The Internet would gradually take over the global communication landscape and have a revolutionary and immeasurable impact on people’s daily lives all over the globe e.g. on communication (e.g. mobile smartphones using a variety of input sensors, i.e. camera, voice, touch, GPS), social network (e.g. text messages, emails, photographs, movies, videos, music, podcast, mobile gaming, blogs, Facebook, Twitter, Instagram, WhatsApp), news, books and culture, business, commerce and manufacturing, technology, travel, finance (banking, transactions and insurance), weather, science, research and scholar activities, education, health (medicine, nutrition, fitness), to share content quickly, efficiently and in real-time. How much data is on the Internet and how is the size of the World Wide Web estimated?

The Internet's size is a moving target, creating a deluge of information. The total data stored by big online storage and service companies like Google, Amazon, Microsoft and Facebook on Internet increases daily and is currently (January 2020) estimated to be at least 1,200 petabytes (1.2 million terabytes). This figure excludes other big providers like Dropbox, Barracuda and SugarSync, to say nothing of massive servers in industry and academia. It is growing by leaps and bounds. The size of the indexed World Wide Web and that of Google are both >60 billion webpages (January 2020).

The mobile health industry and the digital assistant in research

Health Care took advantage of the Internet, especially with the development of mobile technologies, creating new opportunities to improve health and streamline communication between health care workers to exchange medical knowledge and research. Numerous knowledge sharing apps for medical professionals saw the light of day. Doctors happily use these apps to search topics they used to look up in books and files. But a library is usually not at hand when you need information right now. The Internet, accessed via a smartphone, provides a much better, faster, on-demand, often free, answer anytime anywhere, while the doctor need not go to a desktop or laptop computer. This freedom is embraced by the medical profession across all specialities. They all welcome the Internet as a valuable and immediate access tool to science and existing medical knowledge in an attempt to get an answer to their query, limiting the shear endless abundance of medical information that exists.

Need for regulations, accreditation and official oversight of apps in anaesthesia and intensive care.

Hundreds of thousands of health care apps are available at this moment, with numerous topics of care in anaesthesia and intensive care. However, anybody can produce a medical/anaesthesia app and sell it on the Internet. There is lack of oversight of these apps and no official regulation or accreditation rules exist. Clinicians and researchers are left out of well-controlled, up-to-date peer-reviewed apps and risk similar counterfeit actions of cybercriminals victimising the scholarly publishing world. Indeed, the new millennium saw a revolution in electronic “open-access” publishing, triggered by the technical options offered by the Internet and the low cost of setting up novel electronic journals. Similar to the scholarly world of publishing scientific articles, where various anomalous threats resulted in paid publishers, predatory and hijacking journals, which left duped authors without service, empty pockets and worse, no published article, as peer-review often is not delivered or unreliable with a lack of ethical guidelines during the submission process. This negative by-product of the Internet revolution has resulted in a massive influx of unscrupulous and dishonest publishing operators, driven by profit, promising rapid decisions and quick publishing using misleading marketing strategies and false claims, i.e. citation of fake impact factors, abusing real journal names, stealing web domains (web-swooping) and even using their logos. Informing our colleagues of these deceitful publishing practices is a must. We therefore need a regulatory organisation that checks medical apps using a peer-review process, labels and classifies them and marks these apps according to an internationally agreed app quality scoring and accreditation system. Publication of genuine scientific work, whether it is a scientific article or a medical app, should be considered alike as a mark of academic achievement. Anaesthetists and Intensivists all over the world use the Internet to get across scientific information and research data. The Internet, mobile smartphones, apps and email exchange have become standard tools in research. We now cannot image a world without them. We were again prolific in our publications this year, with staff and students publishing 127 peer-reviewed journal articles, 6 book chapters and 11 published abstracts. Once again…..a brilliant achievement! This year, we were awarded 29 grants, with our researchers securing just over $15.2 million in research funding. An outstanding effort! Research not only relies on funding bodies for vital research funds but also on our generous donors and sponsors whose financial support has helped boost our research endeavours. I would like to thank Dr Sheena Burnell for her wonderful gift of $10,000 to our research program in Anaesthesia and Mr John Fletcher of The University of Queensland in America, Inc. for his gift of $492. Professor Jeffrey Lipman MBBCh DA (SA) FFA (SA) FFA (CritCare) (SA) FCICM MD Director of BTCCRC; Professor, Anaesthesiology & Critical Care, UQ; Snr Staff Specialist, Dept of Intensive Care Medicine, RBWH; RBWH Research Ambassador Jeff is the Director of the BTCCRC, Professor of Anaesthesiology & Critical Care, UQ, Honorary Professor, Department of Anaesthesia and Intensive Care, Chinese University of Hong Kong, Honorary Professor of Nursing (QUT) and RBWH Research Ambassador. He has qualifications in anaesthesia and intensive care and has set up and run a number of Intensive Care and Trauma Units in South Africa before coming to Australia in 1997. He is the author of over 550 peer reviewed publications, 33 book chapters, has been invited to present at many national and international conferences and is a

reviewer for a number of high quality international journals and granting committees, including the NHMRC. Jeff has been instrumental in developing the anaesthesiology & critical care component of the graduate medical program for Queensland and continues to lecture to medical and postgraduate students. He is site Principal Investigator for numerous multi-national and international trials. Research interests include all aspects of management in intensive care, resuscitation of burns and pharmacokinetics of antibiotic dosage. His research into antibiotic usage in acute situations has received international recognition whilst he has received multiple national and international awards. Jeff is involved in most research projects, oversees the direction of the research centre, mentors staff and supervises postgraduate students. In 2017, he was elected the RBWH Research Ambassador due to his extensive research track record and in this pivotal role, he has implemented systems to enable better integration of research at the Herston campus between RBWH, UQCCR, QIMR Berghoffer and QUT and better liaison between RBWH and its co-located Academic partners. He was recognized as one of the most cited authors in 2018 in his field by Clarivate Analyticss and in 2019 was top ranked scientist (in the top 1% most cited) in the fields of Clinical Medicine and Pharmacology and Toxicology. In 2019, the International Society of Antimicrobial Chemotherapy (ISAC) awarded Prof Lipman the merit title of ISAC-Fellow (FISAC) – a newly established honorary title of ISAC which recognises individual professionals who excel within their field or have delivered outstanding service to the society.

Professor André van Zundert MD, PhD, FRCA, EDRA, FANZCA Professor & Chair, Anaesthesiology & Critical Care; Chair BTCCRC, UQ; Snr Staff Specialist, Dept of Anaesthesia & Perioperative Medicine, RBWH; Professor of Anesthesiology at the Universities of Ghent (Belgium) and Maastricht (Netherlands) André has qualifications in anaesthesia, intensive care and trauma care, graduating as a medical doctor cum laude, at the University of Leuven (Belgium) in 1978 and with a PhD in obstetric anaesthesia from the University of Leyden (Netherlands) in 1985. He is the author of over 400 peer-reviewed publications, 60 book chapters and three standard textbooks, of which his book, the ‘Atlas Functional Anatomy for Regional Anaesthesia and Pain Medicine’, published in 2015 has enjoyed > 500,000 chapter downloads. In 2017, he authored a new book “Regional Nerve Blocks”, an illustrative book on virtually all

existing nerve blocks used in anaesthesia. He has been invited to present at hundreds of international conferences and is a reviewer for a number of highly regarded international journals, including Editor-in-Chief of the International Journal of Regional Anaesthesia. Under his leadership, he supported anaesthesia education by producing the bimonthly ‘Anaesthesia Research Review’, sent to all anaesthetists in Australia. He has held several positions (Secretary-General, President and Honorary Member) in Anaesthesia Societies in Europe and was President of several international and world congresses. André has been instrumental in developing the joint RBWH-UQ ‘Centre for Excellence and Innovation in Anaesthesia’ (CEIA) at RBWH, which underpins the importance of teaching, training via simulation, testing anaesthesia equipment, quality care (audits and incident reporting) and research. His aim to boost research in anaesthesia within the department and to collaborate with other departments and colleagues from all over Australia and beyond has been achieved through his leadership. André is a strong believer in ‘Teach the teacher’ and in ‘Problem oriented education’. His research interest is in airway management, regional anaesthesia and obstetrics, for which he received international recognition and 2 Royal Distinctions (from HRH Queen Beatrix, The Netherlands & HRH King Albert II, Belgium). He was a founding member of The European Society of Anaesthesiology and founder of the Pain Society (Netherlands). Since his arrival in Brisbane, he was appointed as co-convenor of the annual ANZCA conference and the ANZCA Special Interest Group in Difficult Airway Management. Furthermore, he is a member of the ANZCA Research Committee and ANZCA Research Grant Committee and has been greatly acknowledged for his contributions to: 1) Pain Relief in Childbirth (the suggested technique is still practiced all over the world in the labour ward) 2) providing safe and effective airway management using videolaryngoscopy and 3) Pain therapy. For the CEIA, André received the UQ 2014 Innovation Award. He holds several professorial appointments at UQ, QUT, Victoria University and 3 professorial appointment overseas, at the Universities of Ghent (Belgium), Maastricht (Netherlands) and Udayana (Indonesia). In 2019, Andre is recognised as one of the Top Ten World Experts in Airway Management (USA). Dr Michael Rudd MBBS FACS OAM PhD

Co-Director BTCCRC, UQ; Acute Trauma Surgeon, RBWH Michael is the Co-Director of the BTCCRC and the Director of the Adult Burns Unit at RBWH. He has fellowships in Burns and General Surgery. Michael has completed a PhD in transplantation immunobiology. His research interests are in all aspects of burn management, burn pathophysiology and immunobiology. His clinical and research background make him uniquely qualified to work as a burns surgeon and scientist at both the bedside and in the laboratory.

Our laboratory research is closely allied with our clinical research. The five (5) major themes researched within the centre are:

We conduct a large number of multi-centred clinical trials across the five research themes, involving national and international

collaborators and sites ensuring that our research impacts on a global scale.

2) Drug Dosing and Pharmacokinetics

Pharmacokinetics, drug distribution and antibiotic resistance in critically ill patients. Critically ill patients often have varying physiological needs to an average patient and often require different dosages of antibiotics and other drugs, particularly if they are on renal replacement therapies. There is also a controversy over whether bolus dosing or continuous infusion of β-lactams is more effective in antibiotic delivery in critically ill patients. Prof Lipman has pioneered work that demonstrates that patients in intensive care are not receiving adequate doses of antibiotics to achieve optimal levels to kill pathogens. This in turn leads to poorer health outcomes for patients and antibiotic resistance. Antibiotic resistant infections commonly occur in long stay, debilitated patients and contribute to increased risk of death, longer ICU and hospital stay, further weakness and long term dysfunction. This problem also contributes to patients being unable to be discharged from ICU or hospital and so exacerbating the shortage of acute hospital beds and increases in surgery waiting time. Management and outcome of burns patients encompassing prevention, acute injury and long term outcomes Thermal injury is a devastating injury, not only for the immediate concern of risk of loss of life and extreme pain, but in the long term cosmetic and functional issues. As a means of improving the treatment and outcomes of these patients, the Burns Unit at RBWH, one of the largest in Australia, has instituted a large number of studies investigating skin production in the lab, initial cardiac function, management of pain, antibiotic efficacy, nutrition issues, exercise post burns, and prevention of scaring. 5) Military & Trauma Medicine Trauma is the leading cause of death and disability amongst the young, active population in Australia. Our trauma teams are engaged in research investigating all aspects of trauma care trials of medications to stop bleeding in pre-hospital care, to determining the microenvironment dynamics in wounds, to longitudinal health outcomes. The Centre’s activity throughout the year has continued to focus on championing change as evidenced by over fifty publications resulting from our work during 2019, translating our dosing regimens into internationally followed guidelines and by extending some our mechanistic studies into controlled trials. We continued our collaboration with our industry partners and our administering institution - The University of Queensland – in the delivery of educational and capacity building seminars focusing on pharmacokinetic modelling and optimisation of antimicrobial use. We aimed to achieve this in multiple health care contexts, from acute care of adult and paediatric critically ill patients to Hospital in the Home (HITH) and Outpatient Parenteral Antimicrobial Therapy (OPAT). In February 2019, our centre delivered OPAT/HITH seminar at the Garvan Institute of Medical Research in Sydney. Over one hundred delegates from a variety of disciplines, including pharmacy, medicine, nursing, microbiology, pharmaceutical industry, publishing, as well as policy makers representing NSW State Government attended. Key areas of research need in this context were identified as a result of this productive meeting and the Centre has subsequently engaged in the process of developing grant applications to address some of these important questions. We cultivated regional collaboration across Asia Pacific with the delivery of the Antimicrobial Resistance & Optimisation Seminar in Ho Chi Minh City, Vietnam. Our clinician researchers collaborated with Ton Duc Thang University in bringing together Asia-Pacific leaders in antimicrobial resistance research in Ho Chi Minh City, Vietnam. We linked with the University of Sydney to deliver this important regional collaboration. In early July 2019, we again collaborated with the University of Nîmes, France in the delivery of a seminar focused on the topic of antimicrobial optimisation. Topics discussed included the emerging resistance in Europe, new and old drugs for problematic pathogens, guideline development in the era of resistance and antimicrobial stewardship optimisation in different settings. This meeting attracted a range of practitioners involved in complex drug dosing including clinical pharmacists, infectious diseases physicians, intensive care physicians, transplant physicians and clinical pharmacologists. A major achievement of the year has been the Centre’s partnership with The University of Queensland and several other nationa l institutions and international partners on the delivery of the 3rd National Antimicrobial Resistance (AMR) Forum, held in Brisbane on November 8, 2019. In addition to the CRE REDUCE, the key partners included the Queensland State-wide Antimicrobial Stewardship Program, the UQ Primary Care Clinical Unit, the UQ School of Veterinary Science, the UQ School of Dentistry, the UQ Institute of Molecular Bioscience and the National Centre for Antimicrobial Stewardship, based in Melbourne. Over two hundred physicians, nurses, pharmacists, veterinarians, dentists, policy makers, and others, were attracted to this meeting under a multidisciplinary umbrella of effective action against antimicrobial resistance in human and animal medicine. On the days prior and post Forum, we convened the Forum associated workshops, including on antimicrobial optimisation in general practice, antimicrobial stewardship implementation and the development and use of antimicrobial dosing guidelines in veterinary practice. The Centre continued its work in rapid diagnostics and dosing software, integrating the research administration work between the institutions involved in this flagship research program, including Queensland Pathology, the Princess Alexandra Hospital, Queensland Children’s Hospital, Royal Brisbane and Women’s Hospital and The Prince Charles Hospital. The Centre hosted and enjoyed the contribution of multiple visiting researchers throughout the year, including from Universidad Autónoma de San Luis Potosí, Mexico, the Amsterdam University Medical Centre, the Netherlands and the University of Pittsburgh, Pennsylvania. We are very grateful to our funding body NHMRC, for enabling our research and our work around building research capacity, which will benefit us all in the future.
