Community Health Needs Assessment (CHNA) Request for Proposal

October 2015 Children’s Hospital Los Angeles (CHLA) is requesting proposals from qualified consultants to conduct a Community Health Needs Assessment (CHNA) in compliance with California Health & Safety Code, Section 127350 and Section 501 (r) of the Internal Revenue Code.

1. Children’s Hospital Los Angeles Background

CHLA is a 501(c) (3) nonprofit hospital for pediatric care that has been named the best children’s hospital in California and ranked in the top ten nationally for clinical excellence. CHLA is the only freestanding Level 1 Pediatric Trauma Center in the Los Angeles County that has been approved by the County Department of Health Services as well as accredited by the Committee on Trauma of the American College of Surgeons.

2. Community Health Needs Assessment (CHNA) Background When the Patient Protection and Affordable Care Act was enacted by Congress on March 23, 2010, Section 501(r) was added to the Internal Revenue Code. Final regulations governing Section 501(r) were released on December 29, 2014. Section 501(r) stipulates that hospital organizations under 501(c) (3) status must adhere to the new regulations. In accordance with the previously noted regulations, all not-for-profit hospitals are required to complete the following: Conduct a community health needs assessment at least once every three years

a) Solicit and take into account input from the following groups for both the identification and prioritization of significant community health needs:

b) Identify resources needed to address the significant health needs c) Complete an evaluation of the impact of any and all actions that were taken to address significant

health needs since the previous assessment d) Document a separate CHNA for each individually licensed hospital facility that is adopted by the

hospital’s authorized governing body e) Develop an implementation strategy to address health needs identified by the CHNA.

3. Proposal Requirement Scope of Work

Consultants working with CHLA must ensure, at a minimum, completion of the following tasks and deliverables as part of their contract and scope of work:

a. The collection and synthesis of secondary data utilizing CHLA’s CHNA data categories, community indicators, and other relevant data comparisons, benchmarks and sources;

b. The collection and analysis of primary data from required subpopulations and synthesis of secondary indicator data findings with primary data;

c. Conduct PhotoVoice projects, focus groups, and surveys for the purpose of obtaining primary data specific to the needs of the CHNA.

d. The facilitation of a well-defined and documented process for the identification and prioritization of significant health and social needs facing children, adolescents, young adults and families in the target community as defined by CHLA;

e. Develop a detailed written CHNA Report and a CHLA report template, and; f. Effectively communicate with the advisory council concerning all key data as well as the

progress of the assessment.

4. Proposal Timeline The consultant will be expected to complete the following by April 15, 2016:

a) Conduct the CHNA and complete a written CHNA Report (including the Executive Summary) and; b) Develop and submit a copy of the Implementation Strategy and Plan

Compensation for services will be based upon the completion of all services to CHLA’s satisfaction and paid as set forth in a written agreement between the consultant and CHLA.

5. Consultant Accountabilities and Experience Consultants will be accountable for all aspects of the conduct of the CHNA as outlined in Section III, to include:

● Milestones, ● Deliverables, ● Timelines ● Accuracy and quality of all processes and products.

● Health Need Summaries– documentation of secondary data, primary data, and geographic and race/ethnicity disparities for each identified health need

• Clear communication with community stakeholders as well as the advisory council regarding all pertinent information

Define and Validate Priorities

Prioritize the identified health needs including a clear description of the process and criteria used to prioritize the health needs

Document and Communicate Results

● Draft a CHNA/CHNA Executive Summary

● Write a CHLA-specific CHNA Report

● Presentation of CHNA findings to key stakeholders

● Development of a CHLA Implementation Strategy and Plan

In addition to regular meetings with the respective CHLA hospital staff/leadership, the consultant(s) selected for the position must attend all scheduled in-person/online meetings, and regular technical assistance calls (as needed).

6. Consultant Selection Criteria Consultants will be selected for the position based upon the following criteria:

● Knowledge of the final CHNA requirements as outlined by the Federal Register of the Internal Revenue Service, Volume 79, No. 250, Part II

● Previous experience conducting CHNAs for hospitals ● Demonstrated ability to effectively communicate CHNA findings, implications, and implementation

plans to various stakeholder groups and the public

7. Proposal Instructions a. Interested consultants are invited to submit a proposal to Children’s Hospital Los Angeles

no later than Monday, October 12, 2015. No applications received after this date will be considered. Proposals must provide evidence of items listed in Section 6 (Consultant Selection Criteria), and provide the following information:

i. Organization/Consultant Information that must be provided includes,

1. a brief narrative outlining your consulting experience addressed in Section 5,

Note: You are encouraged to provide supporting documentation to your proposal (e.g. example CHNA reports and Implementation Strategy documents) that demonstrates your experience with this process.

ii. CV and/or summary of relevant experience for key staff that will be assigned to the CHNA project,

iii. A minimum of two letters of recommendation from previous/current clients that have or have had similar requirements, and their current contact information.

b. Work Plan, Deliverables, and a Timeline - In addition to the required Scope of Work outlined in Section 3, provide the work breakdown components, milestones of your proposed project plan that will meet the scope of work/timeline

c. Payment Schedule - Payment schedule will be based upon phase completion as determined by CHLA / completed project plan milestones

d. Budget Proposal

i. A detailed list of proposed project costs and expenses, including estimated staff, time and rates, broken down by phase/deliverable.

ii. Provide a narrative justification for each line item in the Budget Summary. For each key staff please include a narrative of their expected project role and responsibilities, and percentage FTE.

iii. Costs that are not directly accountable to the project (such as administration, personnel and security costs) may not exceed 10%.

8. Basis of Award CHLA reserves the right to reject any and all proposals, to waive any and all informalities, and to disregard all nonconforming, non-responsive, irregular or conditional proposals. CHLA further reserves the right to reject the proposal of any and all applicants if we believe that it would not be in the best interest of the hospital or community or because the applicant is found to be not responsible or fails to meet any other pertinent standard or criterion established by the hospital.

9. Hiring Process Proposals will be reviewed by an evaluation team composed of CHLA staff as well as one or more non-employees. Proposals will be evaluated in two phases:

a) A review and evaluation of proposal elements, including reference checks, and b) Following the evaluation of the proposals, CHLA will individually interview the prospective finalists.

Following an initial review of the submitted proposals the applicants may be asked to deliver a presentation outlining the key components of their proposal. The applicants will not be given questions to prepare for in advance of the interview. Failure to participate in the interview process shall result in an applicant’s disqualification from further consideration.
