Mid-Atlantic Associates, Inc. (Mid-Atlantic) submits for your review the results of our IAQ evaluation at the above facility. Our report summarizes the project background, our scope of work, results of our investigation and recommendations.

PROJECT BACKGROUND

Teachers and staff at The Ashley Academy for Cultural and Global Studies (the Academy) have reported illness and symptoms they associate with mold exposure and indoor air quality problems at the Academy. Based on these concerns the Winston-Salem/Forsyth County Schools administration (WSFCS) contracted with Mid-Atlantic to evaluate the indoor air quality at the Academy, document our findings through physical inspection and laboratory analysis and to prepare a report of the assessment with recommendations for improving the indoor environment at the Academy. The results of our investigation are presented herein.

SCOPE OF WORK

Our approved scope of work included four components:

1. Meet with Ashely Academy staff to discuss their observations, symptoms andconcurrence on the overall project investigation plan;

2. Conduct a physical reconnaissance of the facility to identify building envelope and air handling equipment conditions that may be contributory to the quality of the buildings indoor air;

3. Collect analytical data to support our findings, conclusions and recommendations;

and,

4. Provide a written report documenting our activities. FIELD INVESTIGATION Meetings With WSFCS Personnel Mid-Atlantic met with WSFCS administrative staff on February 28, 2018 to review their understanding of the complaints along with their observations of the building and building history. Based on our discussions, a meeting was held with Ashley Academy staff on March 6, 2018 to discuss their observations and solicit their recommendations regarding our preliminary proposed scope of work, which was provided for their review in advance of our meeting. Prior to our meeting, Mid-Atlantic conducted research into publicly available databases maintained by the North Carolina Department of Environmental Quality (NCDEQ) for identification of on-site or nearby environmental releases that, if present, could be a potential source of indoor air contamination. Based on our review of the NCDEQ databases, no such contamination was identified and thus ruled out as a component of the Academy’s IAQ complaints. Print-outs from the results of each database searched were provided to Ashley Academy teachers during our meeting. General complaints from staff included odors and musty smells, upper respiratory issues, headaches/migraines, sinus infections and aggravation of existing allergies. Staff was also asked for input on improvements they thought would help improve IAQ. Areas identified included leaks between walls and in the bathrooms, discolored ceiling tiles from roof leaks, musty smells from floor coverings and “spots” on some air handling units which they attributed to mold. Odors were generally attributed to moisture or water intrusion and seemed more prevalent in the spring and summer months. Mid-Atlantic utilized information from the March 6 meeting to modify and finalize the scope of work for the assessment.

Field Activity Summary Mid-Atlantic deployed to the school on March 14 and 22, 2018. The indoor air quality assessment was conducted with the objective of identifying conditions in the school buildings that could be modified to improve the general condition of the indoor environment to create a healthier, more comfortable environment for students, teachers and staff. For general guidance, Mid-Atlantic utilized the U. S. Environmental Protection Agency’s (US EPA) Indoor Air Quality Tools for Schools Action Kit. Mid-Atlantic conducted the activities described below in order to achieve the objective of the investigation:

• Conducted a walk through assessment of representative classrooms in each of the four classroom wings, in the media center wing, and in the gym/auditorium pod to identify indoor air contaminant sources, water intrusion conditions, and general building conditions. We inspected for mold and moisture issues, contaminant sources, cleaning practices and general cleanliness for allergen control. A walk through inspection was completed for classrooms 175, 174, and 170 on the 170 wing; for classrooms 163, 159 and 158 on the 160 wing; for classrooms 106 and 107 on the 100 wing; for classrooms 208 and 204 on the 200 wing; rooms 318 and 305 in the gymnasium building, and the media center. The areas inspected are depicted in Figure 1.

• Inspected air handling units and heating, ventilating and air conditioning

(HVAC) systems in representative classrooms in each of the four classroom wings, in the media center wing, and in the gym/auditorium pod. HVAC systems were inspected for the following:

o General cleanliness and condition; o Presence of visible mold growth on internal components and diffusers; o Outdoor air intake; o Condition and operation of condensate drains; and, o Presence of water damage.

• Monitored and data logged indoor temperature, relative humidity and carbon dioxide concentrations using TSI Q-Trak 7575 indoor air quality monitors in rooms 106, 158, 204 and 305 for six to seven hours.

• Inspected four boilers in the school and monitored carbon monoxide (CO) gas

using a QRAE 3 monitor in active sampling mode in indoor spaces adjacent to the boiler rooms.

• Collected 16 lift tape samples for microscopic analysis of mold growth by a

North Carolina-accredited laboratory. Samples were collected to identify mold growth where it was suspected but could not be confirmed by unaided visual inspection.

• Reviewed pest management practices for the school.

• Interviewed a shift manager for the custodial contracting company, Supreme

Maintenance Organization (SMO), at the school. We reviewed cleaning practices, cleaning products and cleaning equipment used by the provider.

FINDINGS Building and HVAC Systems Inspection Mid-Atlantic inspected classrooms and common areas throughout the school. In general the school buildings were clean and well maintained. It is clear from the maintenance logs provided by WSFCS staff that roof leaks do occur periodically and require repair and cleaning. We did not observe, nor were we informed of any active roof leaks, including staining or other visible evidence of leaks during our assessment. Carpets including area rugs had been removed from all of the classrooms we inspected, with the exception of several small area rugs. Hard tile floors were in place in classrooms. Some carpet and rugs were reported to have been removed during this school year after they were water damaged by roof leaks or other water leaks. Classrooms on the 100, 200, 160 and 170 wings of the main building have operable horizontal opening windows (Photograph 1). Several teachers reported that they are permitted to open windows for ventilation or temperature control, and we observed open windows in several classrooms. There were no screens in the windows and there were no water leaks observed around window housing.

We observed visible mold growth on only one surface in one of the inspected classrooms which suggests moisture control indoors is adequate during the heating season. It appears that if there were moisture problems that caused mold growth in the cooling season, that mold contamination has been cleaned. Several teachers did report that they observed mold growing on furniture and other materials after roof leaks or at the start of the school year and that the contamination had been removed. We did not observe any ponding of water or other moisture problems on the exterior of the building. We did not identify any chemical contaminant sources in the building that are likely to cause illness or symptoms for building occupants when being used as observed. Heating, Ventilating and Air Conditioning Systems Inspection Classrooms on the 100, 160, and 170 wings of the Academy are heated by hot water radiant heaters. Cooling in those rooms is provided by ceiling mounted, ductless “crack” cooling units that recirculate room air, but do not provide outdoor air ventilation. Staff from WSFCS opened drop panels on nine of the crack units in nine classrooms so that the internal components of the units could be inspected by Mid-Atlantic. On every unit we inspected there was some water damage that had been caused by dripping water from condensation on chilled water lines leaking into the unit housing. (Photographs 2 and 3). Additionally, there are maintenance record reports of leaks that have occurred and were repaired in the condensate collection pans and lines in several units. It appears water damage from those leaks was cleaned when condensate lines were cleared. There was some soiling visible on air handler insulation, blowers, cooling coils and other surfaces on all units. (Photographs 4 and 5). WSFCS staff reported that each crack unit was cleaned by an outside contractor prior to the beginning of the 2017 school year which was apparent by our inspection of the units. There is a central air handling and cooling unit paired with variable air volume (VAV) boxes mounted above the dropped ceiling in the 200 wing of the building. Heating, cooling and outdoor air ventilation are provided by these systems. The central unit housing and cooling coils appeared to be clean and in good repair, as did two VAV boxes that were partially disassembled and inspected.

Mid-Atlantic inspected the air handling system for rooms 304 and 305 in the gymnasium wing of the building. This recirculating air system provides heating and cooling for those rooms. The heat exchange coils and air handler housing appeared to be clean and in good repair. Indoor Air Monitoring Data logging indoor air monitors recorded temperature, relative humidity and carbon dioxide concentrations in rooms 106, 158, 204 and 305 from approximately 8:30 AM to 3:30 PM on March 14, 2018. Results of temperature logging for each room are shown in Figures 2 through 5. March 14, 2018 was a heating day in Winston-Salem with minimum and maximum outdoor air temperatures of 30 and 39 degrees F. Indoor temperature control was consistent in each classroom (i.e., little fluctuation in indoor temperature throughout the day). Temperatures were elevated, approximately 78 to 82 degrees F, in rooms 204 and 158. Results of relative humidity monitoring are shown in Figures 6 through 9. Relative humidity measurements were less than 30% in all rooms during the monitoring period. Results of carbon dioxide monitoring are shown in Figures 10 through 13. Carbon dioxide concentrations reached peak concentrations of approximately 2,600 parts per million (ppm) in room 106; 1,500 ppm in room 158; 1,500 ppm in room 204; and 1,400 ppm in room 304/305. The difference in peak carbon dioxide concentrations measurements between rooms106 and 158 is likely a result of differences in room occupancy rather than of building characteristics. The crack units in these rooms do not provide outdoor air ventilation and there is no mechanical provision for outdoor air ventilation in those and other rooms served by the crack cooling units and radiant heaters. All of the classrooms we saw on these wings had operable windows, which can under some conditions be used to provide outdoor air ventilation. Figures 14 through 17 depict all parameters combined on one graph for each room sampled. It appears that the VAV system on the 200 wing of the building and the systems in the gymnasium building can be adjusted to increase the outdoor air ventilation rate provided by those systems.

Tape Lift (Mold) Sampling Locations of tape sampling for fungal (mold) growth are summarized in Table 1. Laboratory analytical results of tape lift sampling are included in Appendix A. Samples were collected to confirm that what appeared to be visible mold on surfaces was, in fact, active or current mold growth. Former or currently active mold growth was identified in all of the 16 samples collected. In the crack cooling units on the 100, 160 and 170 wings of the building, there was extensive visible mold growth on various surfaces inside the air handling units including on air handler insulation, on blowers, and on cooling coils. Areas of visible mold growth were limited to small areas on the cooling coil and insulation of the air handlers in the 200 and 300 wings of the building. Mold growth was not observed on walls, furnishings, books, ceiling tiles or other surfaces throughout the areas of the school that were inspected with the exception of sparse mold growth on a bookshelf in room 174 (Photograph 6), and a stained area of ceiling tile adjacent to the crack cooling unit discharge in room 174. Note that there was similar diffuse staining of ceiling tiles in other rooms served by the crack cooling units. Pest Control Practices Staff from WSFCS report that their pest control is limited to the use of baits for insects and rodents where needed. There is no are application of pesticides in use at the school. Cleaning Practices Staff for the cleaning contractor for the school, SMO, report that floors are swept and spot mopped daily throughout the school. Area wide mopping is done at least weekly. Vacuums are used in the media center and on the few area rugs in the school. Mid-Atlantic observed that there is one “True HEPA Filtration” upright vacuum in use as well as several exposed bag upright units. (Photograph 7). Cleaning products observed on a cleaning cart were limited to surfactants and non-solvent based cleaners. Boiler Inspection and Carbon Monoxide Monitoring Four boilers at the school were inspected and carbon monoxide monitoring was conducted on March 22, 2018. The boiler exhaust systems appeared to be in good repair

(Photographs 8, 9, and 10) and exhaust outlets were not in close proximity to operable windows or outdoor air intakes. Staff from WSFC report that boiler exhaust is analyzed annually including CO measurement, with an operational goal of less than 60 ppm CO in boiler exhaust. Results of indoor carbon monoxide monitoring are shown in Table 2. One of two boilers in the cafeteria boiler room was operating during CO monitoring. The boiler in the gymnasium operated for 5 minutes prior to monitoring, and the boiler in the 200 wing was not operating during sampling although it had operated during the day. Carbon monoxide was not detected during monitoring in the boiler rooms, or in rooms near and adjacent to the boiler rooms in the school. CONCLUSIONS The Ashley Academy School buildings appear to be generally clean, uncluttered and well maintained. Water intrusion and resulting damage that has been reported has been repaired and thoroughly cleaned. Wall to wall carpeting is limited to the media center and the use of area rugs is limited to small rugs in just a few classrooms. All of these conditions are favorable for control of allergens including mold and dust mite allergen in the indoor environment. The HVAC systems in the 200 classroom wing, and in the gymnasium building appear to function properly and are adequately maintained. Though there was some mold growth identified in VAV boxes and air handlers serving these areas, the growth appeared to be limited to small areas where mold is commonly found in cooling systems. The crack cooling units appear to have a long history of water damage from condensate leaks and high humidity inside the units. Extensive visible mold growth, or the remnants of growth that had been cleaned, were apparent on various components in every one of these units we inspected and sampled. While it is apparent that each unit was thoroughly cleaned at the beginning of this school year, removal of mold from porous or semi-porous surfaces, like the insulation inside the units, is difficult and often cannot be achieved. Additionally, there are many surfaces inside the units that are difficult to access for cleaning.

Outdoor air ventilation rates, particularly on the 100, 160 and 170 wings served by the crack cooling units are lower than is recommended. ANSI/ASHRAE Standard 62.1-2016 “Ventilation for Acceptable Indoor Air Quality” recommends an outdoor air ventilation rate of 15 cubic feet per minute per person in an occupied space, including school buildings, to control odors and indoor air contaminants in indoor environments. Under these recommended conditions, carbon dioxide that is generated by building occupants will reach steady state concentrations of approximately 1,000 to 1,200 ppm. Higher concentrations of CO2 indoors are not hazardous; however, indoor spaces become odorous and stuffy under these conditions and building occupants are more likely to report dissatisfaction with the indoor air quality as a result. The four gas-fired boilers in the school appear to be well maintained and we found no evidence of carbon monoxide exposure indoors from boiler effluent. Note that monitoring was done for a short time on a single day. Continued diligence in maintenance of the boilers is necessary to ensure that CO does not enter the school building RECOMMENDATIONS Mid-Atlantic offers the following recommendations to improve indoor air environmental quality and reduce allergen exposure indoors at Ashley Academy School. Recommendations are offered in order of descending importance.

1. Consider replacement of the crack cooling units in the 100, 160, and 170 wings. These units are a likely source of mold exposure when they are active. Replacement units should provide conditioned outdoor air ventilation and should be easily cleanable. If they are not replaced, the crack units should be thoroughly cleaned at least annually. Both sides of cooling coils, air handler housing, condensate pans and drains, blowers, diffusers and any other accessible surface should be cleaned. If units are not to be replaced consideration should be given to replacing air handler housing insulation inside the units.

2. Avoid the use of area rugs and carpeting in classrooms. Hard surface floors are

easier to clean and will not serve as a sink for settled allergen, nor as a growth substrate for mold and dust mites, particularly where moisture problems occur.

3. Replace ceiling tiles that are in line with, and less than four feet from, supply diffusers on the crack cooling units.

4. Where possible, increase outdoor air ventilation rates in air handlers in the

gymnasium building, and on the 200 classroom wing. Air handlers and VAV boxes on the 200 wing should be cleaned. Internal surfaces to be cleaned include heat exchange coils, insulation, condensate collection pans and lines. Similar cleaning should be done for the air handler that serves rooms 304 and 305. Other air handling systems in the school should be inspected and cleaned as needed.

5. Outdoor air ventilation may be provided by opening classroom windows during the

heating season if security and energy management concerns allow. Opening widows during the cooling season, however, is likely to create humidity and moisture problems and resulting mold growth indoors.

6. Use data loggers to measure trends in relative humidity in the classrooms served

by crack cooling systems. Indoor relative humidity should be maintained below 60% in indoor spaces.

7. The cleaning contractor for the school should use vacuum cleaners with high

efficiency exhaust filtration in order to minimize the aerosolization of allergens during vacuuming.

Recommendations for Teachers and Staff of Ashley Academy The following recommendations are offered for teachers and staff at the school in order of descending importance.

1. Remain diligent in identify and reporting any moisture problems, particularly water intrusion in the building.

2. Watch for and report mold growth on books, furniture and other surfaces, as well

as recurrent moldy musty odors indoors.

3. Minimize clutter in classrooms and other spaces to make them more easily cleanable. Remove unneeded furnishing, books, newspapers and other materials where possible.

4. Avoid the use of air freshener aerosols (sprays), solids or other odor masking

agents to control odors indoors.

5. Do not allow furry pets into classrooms. A significant portion of adult and child populations are allergic to fur bearing animals.
