Foreword Iodine Deficiency Disorders (IDDs) are a worldwide major public health
problem. According to Govt. of India, more than 1.5 billion people all over the world
are at risk of IDD. Therefore, in 1962, Government of India launched National Goiter
Control Program and in 1992, it is renamed as National Iodine Deficiency Disorders
Control Program.
Iodine Deficiency may lead to Goiter, still births, abortions, congenital
anomalies increased infant mortality, mental deficiency, deaf mutism, squint &
dwarfism.
Iodine Deficiency Disorders with the exception of few are permanent and
incurable. However, all these disorders can be easily prevented before they occur. The
simplest, most effective and inexpensive method to prevent the broad spectrum of
IDDs is to consume iodated salt daily.
The main goal of this program is to bring the prevalence of IDD to below
5% by 2017 and to ensure 100% consumption of adequately iodated salt at the
household level.
To know the prevalence of Iodine Deficiency Disorders in the State of
Punjab in 2012-13, four districts namely Faridkot, Sri Muktsar Sahib, Shaheed Bhagat Singh
Nagar & Hoshiarpur have been successfully surveyed by State IDD Cell, Punjab as per
Govt. of India guidelines. State IDD Cell team, District Health Officers, School Health
Officers, Epidemiologists & Rural Medical Officers were trained by IDD Survey Team,
Ministry of Health & Family Welfare, Govt. of India regarding technique of IDD Survey
in collaboration with School of Public Health, PGIMER Chandigarh.
IDD Survey Report (2012-13) has been prepared by State IDD Cell, Punjab
on the basis of IDD Survey conducted by identified three teams in each district
consisting of District Epidemiologist (for Goiter examination), Block Level Medical
Officer/Rural Medical Officer, Block Level LHV/ANM, Block Level BEE/SI and ASHA/ Link
worker.
I congratulate State IDD Cell team, District IDD Survey teams for their
hard work and efforts in successfully completing the IDD Survey (2012-13) and also
thank Dr. Sonu Goel, Assistant Professor, School of Public Health, PGIMER Chandigarh
for his co-operation.
I hope that State IDD Cell will continue making efforts in future to bring
down the prevalence of Iodine Deficiency Disorders in the State of Punjab.

Introduction
Surveys conducted by Indian Council of Medical Research, Directorate
General of Health Services, State Health Directorates and other Research
institutions have revealed that out of 324 districts (28 States & 7 UTs) Surveyed
so far, 263 districts are endemic and no State/UT is free from Iodine Deficiency
Disorders.
It has been estimated by the Ministry of Health & Family Welfare that
about 70 million people are suffering from various Iodine Deficiency Disorders in
the country. These disorders include Goiter, still births, abortions, congenital
anomalies, increased infant mortality, mental deficiency, deaf mutism, squint &
dwarfism.
Iodine Deficiency Disorders with the exception of a few are permanent and
incurable. However, all these disorders can be easily prevented before they
occur. The simplest, most effective and inexpensive method to prevent the broad
spectrum of IDDs is to consume iodized salt daily.
In the State of Punjab, only three districts have earlier been surveyed by
the Central IDD Survey team of the Directorate General of Health Services, Govt.
of India
1. Hoshiarpur
2. Gurdaspur
3. Ropar
IDD Control Cell- An IDD Cell has been set up in the State under the
supervision of State Programme Officer. The posts of Technical Officer,
Statistical Assistant and LDC Typist have been filled up.
State IDD Lab Punjab- A State IDD lab has been set up at the State Food Lab,
Sector-11, Chandigarh under the supervision of State Programme Officer. The
posts of Lab Technician and Lab Assistant have been filled up.
IDD Survey- During year 2011-12, IDD Survey has been completed in
Jalandhar, Kapurthala, Amritsar, Tarn-Taran and Gurdaspur.
Distribution of Iodized Salt- The State does not have any facility for Iodization
of salt. The same is being procured from Rajasthan and Gujarat. Iodized salt
has not been included under Public Distribution System.
IEC Activities- These activities have so far been conducted through
Newspaper advertisements, Radio spots regarding prevention of IDDs through
regular consumption of Iodized salt.
Training- A one day training was organized by the IDD Cell of the Directorate
of Health Services, Punjab on 19th November 2013 to train District Health
Officers, Epidemiologists, School Health Officers & RMOs regarding IDD
Survey techniques. Dr. sonu Goel, Assistant Professor, School of Public Health,
PGI assisted during IDD Survey.
OBJECTIVES
General Objective
To assess the current status of IDD in the four chosen districts of Punjab.
Specific Objective
To assess the prevalence of clinical forms of IDD among 6-12 year children in 4
selected districts
To estimate random urinary excretion levels in a sub-sample of 6-12 year
children covered for clinical examination, and
To assess the use of iodized salt by community by on spot testing and analytical
estimation of iodine in salt samples.
IDD Survey Methodology
For year 2012-13, IDD Survey in 4 districts Faridkot, Sri Muktsar Sahib, SBS Nagar
& Hoshiarpur was conducted based on the guidelines issued by the Ministry of
Health & Family Welfare, Government of India using the method of Probability
Proportionate to Size (PPS) covering 6-12 years old children.
For the purpose of IDD Survey, first of all the State IDD Cell collected following
data from district:
– Total population of district
– Block/Village wise population
– Urban - ward wise population in urban city area
– District Literacy rate (male/female-literacy rate)
Based on above data from district, a total of 30 clusters were identified as per
PPS sampling for IDD Survey
Identification of clusters
A sample of 30 villages/wards was selected. The sample size was determined as
follows:-
Sample size = 30 villages
Sample interval = Total population of the district/30 = K
Selection of random start from 1 to K = R
Selected random numbers = R, R+K, R+2K, R+3K……R+29K
After identification & approval of identified 30 clusters in a district from Govt. of
India, State IDD Cell shared the list of identified 30 clusters with the district.
Capacity Building of District Staff
Capacity building visit was made by State IDD Cell team in all four districts
identified for IDD Survey.
District Preparation:
In each identified district, three teams were made to carry out Survey in 30
clusters (10 clusters per team). Each team consisted of four members
(compulsory) and one ASHA/Link Worker from identified cluster. Team
composition in each district was as follows:
– District Epidemiologist (for Goiter examination)
– Block Level RMO/MO (for Goiter examination)
– Block Level LHV/ANM
– Block Level BEE/SI
– ASHA/ Link worker from identified cluster
Each team in a district was provided with Honorarium/ Mobility budget support.
CLINICAL EXAMINATION
A Sample size of 90 children (45 boys & 45 girls) between age group of 6-
12 years from the school and community from each identified cluster was
taken.
From each identified district, 2700 children were covered for Goiter
examination.
Collection of Salt & Urine Sample- As per GoI guidelines
Every 5th child examined for Goiter was covered for Salt sample by visiting
the houses.
Every 10th child examined for Goiter Survey was covered for urine sample.
A total of 18 salt samples & 9 urine samples were collected from each
cluster.
In all, 540 Salt Samples & 270 Urine samples collected from each district.
Techniques of Examination
From the clinical point of view the diagnosis is entirely based on the inspection
and palpation of the neck by trained Medical Personnel.
Classification of Goiter
Grade 0- Neither Palpable nor visible—No Goiter
Grade 1- Goiter palpable but not visible when the neck is in normal position
Grade 2- A swelling in the neck that is visible when the neck is in a normal
position.
Results of IDD Survey in Faridkot
An analysis of data of district Faridkot reveals that nearly 12.7% of the children of
age group 6-12 years are affected with Iodine Deficiency. The prevalence of
Goiter among boys is higher than in girls.
The prevalence was found to be higher among children of 12 years age group
and similar among other age groups. (Please refer to Tables no. 1, 2 & 3).
Iodine content in Salt Samples: A total of 540 salt samples from district Faridkot
were analyzed for Iodine content in the State IDD Lab Punjab using titration
method. 486 (90%) salt samples were found adequately iodized and 54 salt
samples (10%) contained Iodine content less than 15 parts per million (Table 4).
Urinary Iodine Estimations: A total of 270 urine samples from district Faridkot
were analyzed for Iodine Estimations in the State IDD Lab, Punjab. All Urine
samples contained Iodine content more than 100 μg/l. (Table 5)

Results of IDD Survey in Sri Muktsar Sahib
An analysis of data of district Sri Muktsar Sahib reveals that nearly 8.7% of the
children of age group 6-12 years are affected with Iodine Deficiency. The
prevalence of Goiter among girls is higher than in boys.
The prevalence was highest in children with age group of 8-9 years and lowest in
children with age group of 10-11 years. (Please refer to Table no. 6, 7 & 8).
Iodine content in Salt Samples: A total of 540 salt samples from district Sri
Muktsar Sahib were analyzed for Iodine content in the State IDD Lab, Punjab
using titration method. 497 (92%) salt samples were found adequately iodized
and 43 salt samples (8%) contained Iodine content less than 15 ppm (Table 9).
Urinary Iodine Estimations: A total of 270 urine samples from district Kapurthala
were analyzed for Iodine Estimations in the State IDD Lab, Punjab. All Urine
samples contained Iodine content more than 100 μg/l (Table 10).

Results of IDD Survey in Shaheed Bhagat Singh Nagar
An analysis of data of district SBS Nagar reveals that nearly 2.4% of the children
of age group 6-12 years are affected with Iodine Deficiency. The prevalence of
Goiter among boys was found to be slightly higher than in girls.
The prevalence was highest in children with age group of 10 to 11 years and
lowest in children with age group of 6-7 years and 8-9 years. (Please refer to
Table no. 11, 12 & 13).
Iodine content in Salt Samples: A total of 540 salt samples from district SBS
Nagar were collected & analyzed for Iodine content in the State IDD Lab, Punjab
using titration method. 534 (98.9%) salt samples were found adequately iodized
and 6 salt samples (1.11%) contained Iodine content less than 15 ppm (Table 14).
Urinary Iodine Estimations: A total of 270 urine samples from district Amritsar
were collected & analyzed for Iodine Estimations in the State IDD Lab, Punjab. All
Urine samples contained Iodine content more than 100 μg/l (Table 15).
