REGION IX EDUCATION COOPERATIVE (REC IX) 237 SERVICE ROAD RUIDOSO, NM 88345 575.257.2368 RELATED SERVICES/DIAGNOSTIC/CONSULTANT REQUEST FOR PROPOSALS 2016 -2017 Needed Services: Provision of related/diagnostic/consultant services (Physical Therapy – 2 day per week, Speech Language Therapist – 2 days per week) for Participating School District/Communities: Capitan, Carrizozo, Tularosa and RECIX. Services will include: 1. Direct therapy(ies) as specified by student’s IEP or IFSP A. Schedule and frequency of services will be arranged by Contractor consistent with IEP's/IFSP’s and school district or RECIX calendars 2. Educational evaluations as specified by LEA’s administration following requests by Student Assistance Teams or specified by Developmental Services Program 3. Consultation/training as specified by school districts or RECIX 4. Participation in evaluation/diagnosis as requested by appropriate administrative staff of school districts or RECIX and will include: A. Instrument administration/interpretation B. Comprehensive written report including evaluation/diagnosis summary, interpretation, parent input, collaboration with other service providers, and recommendations C. All evaluations/diagnosis will be in compliance with Standards of Excellence for NM Schools and local district policy. D. Evaluation/Diagnosis for an Early Childhood will be in compliance with DOH Policy and Procedures and RECIX 5. Collaboration/coordination of all personnel providing services to those children determined in need of services by IEP/IFSP committee and/or Student Assistance Team 6. Conferencing with staff and parents of students receiving services 7. Participation in students’ IEP/IFSP committee meetings 8. Other services deemed necessary by IEP/IFSP committee or appropriate administrative staff of LEA 9. Contractor will determine appropriate manner of providing such services so long as services are performed in accordance with the professional and ethical standards of the profession, are consistent with state and federal regulations, and meet the scheduling constraints of the participating school districts or agency Contract Qualifications: Contractor must meet all licensure/certification requirements as established by the State Public Education Department and State of New Mexico and must present proof of such licensure/certification. Contractor will have to provide proof of insurance and references. Contractor will need to provide their own testing kits and materials or pay a fee to REC IX for the use of testing kits and materials Contract Length: Services will be provided for the 2016 - 2017 school year and will be aligned with REC IX calendar. Contract Amount: Contractor must submit complete compensation requirements in proposal and will attach a cost proposal sheet for each position (Physical Therapy – 2 day per week, Speech Language Therapist – 2 days per week) with the following data: Hourly rate for direct services or indirect services; travel rate for mileage or time; and any other costs such as supervision of a candidate who is a PTA. Contractor will agree to execute the form of professional services agreement appended hereto. Proposal Submission: Contractor must address all contract specifications in his/her proposal submission. All proposals must be received no later than 4:00 p.m., Tuesday, April 12, 2016. RFP must contain the following and be in order as listed below: 1. Cover Letter 2. Proof of Liability Insurance 3.
