ON-SITE VALIDATION OF THE ENCODING AND UPDATING OF LEARNERS' DATA THRU

THE LEARNER INFORMATION SYSTEM

TO: OIC - Assistant Schools Division Superintendent CID Focal Person SGOD Focal Person School Heads (Private Elementary, JHS and SHS) All Others Concerned

1. To provide guidance and technical assistance to all concerned schools in the enrolment of learners

in the system and to ensure correctness, reliability and validity of data, the Schools Division of Imus

City will conduct an on-site validation to all private schools offering Kindergarten and Grade 1

classes from 07 August to 23 August 2017.

2. Technical assistance to be done shall include the following:

a. tally N"SO Birth Certificate from the School Form 1 data specifically the name (first,

middle, last name) and the date of birth;

b. review the age of learners enrolled in the Nursery, Kinder 1, Kinder 2 and Grade I levels;

and

c. ensure that DepEd Order No. 47, s. 2016 is followed accordingly in levelling the learners.

3. Hard copy of the learners' documents such as N"SO Birth Certificate, Form 138 and School Form 1

for SY 2017-2018 especially among 'Nursery, Kinder 1, Kinder 2 and Grade 1 learners should be

made available during the on-site validation.

4. Attached herewith is the schedule of the on-site validations for reference.
