Introduction

In December 2011, Public Health Ontario (PHO) in collaboration with the Ministry of Health and Long-

Term Care initiated a voluntary surveillance program to assess the epidemiology of carbapenamase-

producing Enterobacteriaceae (CPE) and to help inform infection prevention and control policies and

procedures in Ontario. Formerly referred to as carbapenem-resistant Enterobacteriaceae (CRE), the

terminology has been changed to CPE to more accurately reflect the mode of resistance.

In order to facilitate the voluntary CPE surveillance process and ensure the security of personal health

information in Public Health Ontario’s custody, PHO developed a CPE online surveillance form. Infection

control practitioners will use the form to submit patient information to PHO instead of completing the pdf

copy and sending it through the confidential fax number.

The purpose of this document is to provide reference and guidance in submitting patient information

through the CPE online surveillance form.

GETTING STARTED

A user must have the following in order to access the CPE online surveillance form:

Windows based PC (unsupported using Mac)

Microsoft Internet Explorer 9 or higher

Google Chrome

Note: Browsers such as Firefox or Safari may be compatible but are not supported. Tablets and mobile

phones are welcome to use but are also currently not supported.

How to access the CPE online surveillance form

The CPE online surveillance form is accessible in the PHO website and does not require registration.

Alternatively, it can be accessed through the CPE resources website or by selecting Data Reports from

Data & Analytics navigation bar:

From Data Reports, go to Carbapenemase-producing Enterobacteriaceae and select ‘More’.

Select the CPE online surveillance form in the upper right navigation bar.

Notes and features

The CPE online surveillance form allows users to send patient information to PHO in a secured and

efficient manner. Below sections provide a description of the data requirements, features, as well as

instructions on how to complete the form.

INFORMATION AND CONSENT PAGE

Prior to entering patient information, users are requested to read the information letter.

DATA NOTES

The online surveillance form is divided into sections. Fields marked with asterisk (*) are required and

those in grey font are automatically populated based on the data entererd by the user. The form

includes patient information and specimen details to facilitate the identification of the patient (if

clarification is required) and to link the patient information with the laboratory record in the Public

Health Ontario Laboratory.

Comments: Users are encouraged to record any other relevant clinical or laboratory details regarding

the positive isolation of a CPE from this patient in this area.

FUNCTIONALITY

The CPE online surveillance form has built-in data validations in place. For example, the user will not be

able to submit, if required fields are missing or future dates were entered in any of the fields. The form

also contains various functionalities to facilitate data entry. Below are some of these functionalities.

Print

The following icons can be clicked to enable printing of the document:

Or, by clicking the Print button at the end of the form.

Alternatively, users can select File in the ribbon and then Print.

Dates

Users have the option to enter the date in the following format: dd-mmm-yyyy or use the date clicker to

access the calendar:

The calendar’s default date is the current date. Depending on the date, it will show the range if the

arrows are selected. In the case of date of birth, the range of years starts from the current date and

scrolls up to 1920.

Electronic Copy

To save an electronic copy, users can select Click to download:

Choose a folder then select Save.

Alternatively, the Print Screen function can also be used to take a screenshot of the document.

Submitting Data

To submit the page, users can click Submit at the end of the form:

Once clicked, users will be asked whether or not to proceed with the CPE submission. This was put in

place in case users opt to print or save a copy first prior to submitting and leaving page.

The following message will pop up when the form was successfully submitted:

Otherwise, error messages in red font will pop up. This usually happens when required fields were not

completed and/or dates into the future were incorrectly entered by the user.
