QSENWhat does QSEN stand for? Quality and Safety Education for Nurses
History Started October 2005 Funded by The Robert Wood Johnson
Foundation In 3 phases
QSEN Phase 1: Six competencies were
indentified. Five competencies from the Institute of Medicine (IOM) and safety
Phase 2: Pilot schools integrated the six competencies into their nursing programs. Website for QSEN was launched at www.qsen.org
Phase 3: Faculty expertise was developed to teach competencies
QSEN
To prepare future nurses so they will have knowledge, skills, and attitudes (KSAs) needed to continually improve quality and safety in healthcare systems (QSEN, 2012).
QSENHow do we accomplish this? Six competencies were defined in Phase
1 Patient-centered care Informatics Teamwork and Collaboration Evidence-based Practice (EBP) Quality Improvement (QI) Safety
PATIENT-CENTERED CARE“Recognize the patient or designee as the source of control and full partner in providing compassionate and coordinated care based on respect for patient’s preferences, values and needs.” (QSEN, 2012)
• Care conferences• Resident and Family Council• Rounding with a Purpose
INFORMATICS“Use information and technology to communicate, manage knowledge, mitigate error, and support decision making.” (QSEN, 2012)
• CPOE• Cerner• Health Medx
TEAM WORK AND COLLABORATION
“Function effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care” (QSEN, 2012)
• SBAR• Daily check-in• CENA shift to shift reporting tool
EVIDENCE-BASED PRACTICE
“Integrate best current evidence with clinical expertise and patient/family preference and values for delivery of optimal health care” (QSEN, 2012)
• Mandatory Influenza vaccinations for all staff
QUALITY IMPROVEMENT
“USE OF DATA TO MONITOR THE OUTCOMES OF CARE PROCESSES AND
USE IMPROVEMENT METHODS TO DESIGN AND TEST CHANGES TO CONTINUOUSLY IMPROVE THE QUALITY AND SAFETY OF HEALTH CARE SYSTEMS.” (QSEN, 2012)
Collection of data Policy's and Procedures
SAFETY “Minimizes risk of harm to patients and providers through both system effectiveness and individual performance.” (QSEN, 2012)
Use of technology to improve safety. Standardized practices to increase safety
and decrease potential harm to patients.
QSEN In my role as a staff
nurse on the acute and long term setting, I use QSEN practice standards every time I work. When I receive and give report I am practicing hand off communication . When I pass meds, change dressings, obtain lab specimens, assess patients, use bar code scanning, and read back physician's verbal orders, I am following policies and procedures set by nursing practice standards.

QSEN
In my role as a MDS Coordinator for the Rehab and Nursing Center I mostly use the patient-centered care QSEN competency in what I do. I assess the Resident, assist in coordinating care between all other members of the interdisciplinary team, and involve family/significant others in their plan of care with respect of the Resident’s needs, preferences, and values.
