Template: Installer Declaration Business Appliances Method Purpose of this template Under the Installation of High Efficiency Appliances for Business sub-method (Business Appliances), Accredited Certificate Providers (ACPs) must arrange for the installer to complete and sign an Installer Declaration. This template may be used by ACPs to develop their own Installer Declaration. The purpose of the Installer Declaration is to confirm: the address where the equipment was installed the date of installation the make and model of the equipment installed, and that the equipment meets relevant ESS requirements, including that it was installed according to manufacturer’s guidelines, equipment/installation standards and legislation. The ACP must keep a copy of the completed Installer Declaration for each implementation as evidence to be checked at audit. Instructions for ACPs using this template To use this template, you will need to: Select the relevant installer declaration from the following pages Copy the wording from the installer declaration into your own document Format as appropriate. Template: Installer Declaration Business Appliances Method

Purpose of this templateUnder the Installation of High Efficiency Appliances for Business sub-method (Business Appliances), Accredited Certificate Providers (ACPs) must arrange for the installer to complete and sign an Installer Declaration. This template may be used by ACPs to develop their own Installer Declaration.

The purpose of the Installer Declaration is to confirm:

the address where the equipment was installed

the date of installation

the make and model of the equipment installed, and

that the equipment meets relevant ESS requirements, including that it was installed according to manufacturer’s guidelines, equipment/installation standards and legislation.

The ACP must keep a copy of the completed Installer Declaration for each implementation as evidence to be checked at audit.
