• Systemic arterial pH is maintained between 7.35 and 7.45

• The kidneys regulate plasma [HCO3–]
– "reabsorption" of filtered HCO3–
– formation of titratable acid
– excretion of NH4+ in the urine.
• 40–60 mmol/d of secreted protons is represented in the urine as titratable acid and NH4+
• A produção de H+ é constante
• Objectivo: não perder bicarbonato e eliminar o H+ produzido!

Alcalose Metabólica
• Causas:
– Hipovolémia
– Diuréticos
– Vómitos
ALCALOSE METABÓLICA• Metabolic alkalosis is manifested by an elevated arterial pH, an increase in the serum
[HCO3–], and an increase in PaCO2
• It is often accompanied by hypochloremia and hypokalemia
• Tem uma vantagem: estamos sempre a produzir H+!
• Under normal circumstances, the kidneys have an impressive capacity to excrete HCO3–
• net gain of [HCO3–]
• loss of nonvolatile acid (usually HCl by vomiting) from the extracellular fluid.

• Volume deficiency, chloride deficiency, and K+ deficiency exist in combination with a reduced GFR, which augments distal tubuleH+ secretion;
• Hypokalemia exists because of autonomous hyperaldosteronism;

Symptoms
• Changes in central and peripheral nervous system function are similar to those of hypocalcemia
• Mental confusion, obtundation, and a predisposition to seizures, paresthesia, muscular cramping, tetany, aggravation of arrhythmias, and hypoxemia in chronic obstructive pulmonary disease.
• Related electrolyte abnormalities include hypokalemia andhypophosphatemia.

• Correct the underlying stimulus for HCO3– generation
• Dilute hydrochloric acid (0.1 N HCl) is also effective but can cause hemolysis, andmust be delivered centrally and slowly.
• Hemodialysis against a dialysate low in [HCO3–] and high in [Cl–] can be effectivewhen renal function is impaired.

– 1º passo: Calcular Anion Gap, porquê?• Isso divide em dois grupos de causas:
– HIPERCLORÉMICAS E NORMOCLORÉMICAS
– Facilitando o diagnóstico da causa

• AG = Na+ – (Cl– + HCO3–)
• Representa os aniões não mensuráveis no plasma (normalmente 10 a 12 mmol/L)
• Estes aniões incluem: proteínas aniónicas, fosfato, sulfato, aniões orgânicos

Distúrbios mistos
• Alcalose respiratória + acidose metabólica: doente com diabetes descompensada que desenvolve cetoacidose (acidose metabólica) que, num estado de ansiedade, inicia hiperventilação (alcalose metabólica).
• Acidose metabólica + alcalose metabólica + acidose respiratória: doente alcoólico com cetoacidose alcoólica, que vomitou (alcalose metabólica) e inicia quadro de hipoventilação por depressão do centro respiratório devido ao álcool (acidose respiratória).

Cetoacidose diabética
• Coma
• Analises:
– Acidose metabólica
– Hiperglicémia ou normoglicémico (insulina, fome, grávidas, inibSGLT2)
Tratamento Cetoacidose diabética
• NaCl
• Correção K
• Insulina IV lenta
• Se pH<6.9 --- Administrar Bic Sódio – gasimetrias seriadas
ACIDOSE METABÓLICA• increase in endogenous acid production (such as lactate and ketoacids), • loss of bicarbonate (as in diarrhea)• accumulation of endogenous acids (as in renal failure)
ACIDOSE METABÓLICATreatment (elevated Anion Gap)• Diabetic Ketoacidosis (DKA)
– insulin prevents production of ketones, bicarbonate therapy is rarely needed except with extreme acidemia (pH < 7.1),
– fluid resuscitation with isotonic saline
• Alcoholic Ketoacidosis (AKA)– IV administration of saline and glucose (5% dextrose in 0.9% NaCl). Hypophosphatemia, hypokalemia, and
hypomagnesemia may coexist and should be corrected.
• Salicylates– intravenous NaHCO3 is administered in amounts adequate to alkalinize the urine – Acetazolamide
• Alcohol-Induced Acidosis– saline or osmotic diuresis, thiamine and pyridoxine supplements, fomepizole or ethanol, and hemodialysis
• Renal failure– Sodium citrate (Shohl's solution) or NaHCO3 tablets (650-mg tablets contain 7.8 meq)
