Section 1 IntroductionThis directory provides a list of Aetna MedicareHMO and PPO network providers. To get detailedinformation about your health care coverage, pleasesee your Evidence of Coverage.If you are enrolled in a standard Aetna MedicareHealth Maintenance Organization (HMO) plan,you will have to choose one of our network providersthat are listed in this directory to be your Primary Care{Physician/Provider} (PCP). The term PCP will beused throughout this directory. Generally, you mustget your health care coverage from your PCP. YourPCP will issue referrals to participating specialists andfacilities for certain services. For some services, yourPCP is required to obtain prior authorization fromAetna. You must use network providers except forurgent care or out-of-area urgent care/renal dialysis.Except for those benefits described in your plandocuments as direct-access benefits, or in anemergency, you will need to obtain a referral fromyour PCP before seeking covered non-emergencyspecialty or hospital care. Check your Evidence ofCoverage for details.If you are enrolled in an Aetna Medicare (HMO)Open Access plan, you are not required to choose aprimary care physician (PCP) and are free to visitnetwork providers without a referral to receive coveredservices. You must use network providers except forurgent care or out-of-area urgent care/renal dialysis.If you are enrolled in an Aetna MedicarePreferred Provider Organization (PPO) plan, youhave the flexibility to choose either network providersat lower out-of-pocket costs, or out-of-networkproviders at a higher cost sharing level. With theexception of emergencies or urgent care, it may costPPO members more to get care from out-of-networkproviders. You can use any Aetna Medicare PPOnetwork PCP and pay the plan's lower primary doctorvisit copay. Although you are not required to select aPCP, you are encouraged to do so.The network providers listed in this directory haveagreed to provide you with your health care coverage.You may go to any of our network providers listed inthis directory; however, some services may require a

referral. If you have been going to one networkprovider, you are not required to continue going tothat same provider. In some cases, you may getcovered services from non-network providers. In caseswhere non-contracting providers submit a bill directlyto you, you should not pay the bill, but submit it toAetna Medicare for processing and determination ofyour financial liability, if any.Certain healthcare services, such as hospitalization oroutpatient surgery, require precertification with Aetna.This means the service must be approved by Aetnabefore it will be covered under the plan. Check yourEvidence of Coverage for a complete list of servicesthat require this approval. When you are to obtainservices requiring precertification from a participatingprovider, the provider is responsible to precertify thoseservices prior to treatment. If your plan covers out-of-network benefits and you may self-refer for coveredservices, it is your responsibility to contact Aetna toprecertify those services which require precertification.If you need emergency care, you are covered 24 hoursa day, 7 days a week, anywhere in the world. Whetheryou are in or out of an Aetna Medicare service area,we simply ask that you follow the guidelines belowwhen you believe you need emergency care. Call the local emergency hotline (ex. 911) or go to

the nearest emergency facility. If a delay would notbe detrimental to your health, call your PCP. Notifyyour PCP as soon as possible after receivingtreatment.

If you are admitted to an inpatient facility, you or afamily member or friend on your behalf shouldnotify your PCP or Aetna as soon as possible.

If you are traveling outside of the Aetna Medicare(HMO) (PPO) plan service area, you are covered foremergency and urgently needed care. Urgent caremay be obtained from a private practice physician, awalk-in clinic, an urgent care center or an emergencyfacility. If, after reviewing information submitted to usby the provider that supplied care, the nature of theurgent or emergency problem does not qualify forcoverage, it may be necessary to provide us withadditional information.

Follow-up Care after EmergenciesAll follow-up care should be coordinated by your PCP.Follow-up care with non-participating providers is onlycovered with a referral from your PCP and priorauthorization from Aetna. Whether you were treatedinside or outside your Aetna Medicare service area, ifyour plan requires referrals, you must obtain a referralbefore any follow-up care can be covered. If your plandoes not require referrals you should contact Aetna atthe number on your ID card before care is received atnon-network facilities. Suture removal, cast removal,X-rays and clinic and emergency room revisits aresome examples of follow-up care.PPO plans: All in-network and out-of-network follow-up care will be covered under the terms andconditions of your plan.Emergency care can always be obtained in or out ofthe service area from the nearest available provider.When in the service area HMO plan members mustuse plan providers for urgent care. When out of theservice area urgent care may be obtained from thefirst available provider. In addition, when out of theservice area you can obtain dialysis treatment fromany qualified dialysis provider. If HMO plan membersobtain routine care from out-of-network providersneither Medicare nor Aetna Medicare will beresponsible for the costs.

Si est viajando fuera del rea de servicio del plan(HMO) (PPO) de Aetna Medicare, recibir coberturapor la atencin mdica de emergencia y de urgencianecesaria. La atencin mdica de urgencia puederecibirse de un mdico de prctica privada, de unaclnica de acceso libre o de un centro de atencinmdica de urgencia o de emergencia.
