This section reports the numbers of prostheses implanted between 1 January and 31 December 2012 and entered into the NJR by February 2013.

The procedure counts in this section of the report may vary to the procedure counts in Part Two of the Annual Report. This section counts only those procedures where the implants being described have been used. Therefore, if a procedure has been submitted with missing implant information this will cause numbers to be different from Part Two.

1.1 Femoral head material and sizeTable 1 shows the relative use of different modular heads. There has been a 2% decrease in the use of metallic heads and a 2% increase in the use of ceramic heads compared with 2011. Since 2008 there has been a 12% decrease in the use of metallic heads.

Table 2 shows the frequency of the different femoral head sizes. The main change compared with 2011 is a decrease in the use of very large heads from 38mm

and above. This is a reversal of the trend from 2009 to 2011.

1.2 Brand sales of conventional total hips and resurfacing prostheses

Tables 3 to 8 show the most frequently used brands in England and Wales during 2012, as assessed by

their rate of entry into the NJR database. The level of assessment according to ODEP criteria is given in the final column. The tables have been split into two to show brand usage in primary procedures and revision procedures separately.

Table 8 provides a summary of procedures where brands of resurfacing acetabular cup were used in combination with a conventional stem and a large

diameter metal-on-metal femoral head. This type of combination has decreased from 2,494 procedures in 2009 to 35 procedures in 2012.

1.3 Combinations of cups and stems

Tables 9(a)� and 9(b)� detail the combinations of cup and stem brands used in primary and revision hip procedures respectively. Tables 9(a)� and 9(b)� also detail which combinations have a different stem and cup manufacturer – a practice known as ‘mixing and matching’. In previous years only conventional cup and stem combinations were listed but this year’s results show conventional cup and resurfacing cup and stem combinations. For 74,419 hip primary replacement procedures in the 2012 data, both the cup and

stem brand were known, and a total of 778 different combinations of cup and stem brands were recorded. The most common cup/stem combination was the Corail stem combined with the Pinnacle cup. Of the 74,419 primary procedures 12,112 (16%)� used ‘mixed and matched’ cup-stem combinations from different suppliers, approximately 1% less than in 2011. For revision hip procedures 521 cup-stem combinations were used. The most common cup/stem combination was the Stryker Exeter V40 stem combined with the Trabecular metal cementless cup from Zimmer.

1.4 Summary

The total number of brands of acetabular cups and femoral stems used in procedures recorded in the NJR has increased significantly since its inception. There were 101 brands of acetabular cup (excluding resurfacing cups)� used in 2012, a decrease from 2011 when 127 brands were used. There were 146 brands of femoral stem used in 2012, also a decrease from 2011 (162)�. In addition to conventional hip resurfacing procedures, there were 35 procedures

entered into the NJR where a resurfacing cup was used in conjunction with a large diameter modular head and a conventional modular stem, compared with 217 in 2011. Material selection for femoral heads used between 2003 and 2007 remained constant (76% metal and 24% ceramic)�, however, since 2008 there has been an increase in the use of ceramic heads (61% metal and 39% ceramic in 2012)�. There has been a decrease in the use of very large sizes of femoral heads (greater than 36mm)�.

The procedure counts in this section of the report may vary to the procedure counts in Part Two of the Annual Report. This section counts only those procedures where the implants being described have been used. Therefore, if a procedure has been submitted with missing implant information this will cause a mismatch with Part Two.

2.1 Prostheses entered into the NJR

62 brands of total condylar knee prostheses were used in primary procedures and 40 used in revision procedures. In addition, there were 19 brands of unicondylar prostheses, 7 brands of patello-femoral replacements and 15 brands of hinged knees. The number of brands for each prosthesis type has increased for total condylar and unicondylar compared to 2011.

Tables 10(a)� and 10(b)� show the brands of primary and revision total condylar knee replacement prosthesis recorded in NJR for 2012.

The majority of procedures involving cement used antibiotic-loaded rather than non-antibiotic cement. 88% of hip procedures and 96% of knee procedures and all ankle procedures using cement used antibiotic-loaded cements. The brands of all cements used in hip (Table 21a)�, knee (Table 21b)�, ankle procedures (Table 21c)�, elbow (Table 21d)� and shoulders (Table 21e)�, are shown below.
