Activity: Field Records and Data Collection - Vicinity and Satellite Properties
Requested Modification: Bound field notebooks are not utilized for the collection of data duringthe investigation of Vicinity and Satellite Properties. The following data sheet procedures, asdiscussed at the July 31, 1991 kickoff meeting, are described below for the project records.
Property Assignments
Properties are assigned to the NDL field team by Malcolm Pirnie on a day to day basis.Properties are scheduled for investigation at 9:00 am and 1:00 pm unless the homeownerrequests sampling at another time.
Data Sheet Serial Numbers
Each data sheet used during the field investigations has a unique serial number, asfollows :

Data Sheet Assignment
Prior to a Phase I investigation, a data sheet package is distributed to the NDL team byMalcolm Pirnie. The data sheet package includes a Radon Data Sheet, Indoor and OutdoorGamma Survey Data Sheets, Exposure Rate Summary Sheet and Gamma Scan Map. Theprocedure for data sheet assignments will be as follows:

1) A property to be surveyed is recorded on the Serial Number Assignment Sheet byMalcolm Pirnie. For example: Block 22, Lot 7, 25 Lakeside Drive. Note that theSerial Number Assignment Sheet is in the custody of MPI staff at all time.
2) The Phase I data sheet package, as noted on the Serial Number Assignment Sheet,will be assigned to the NDL team by Malcolm Pirnie. During data sheet sign out,both NDL and MPI are required to initial the Serial Number Assignment Sheet.
Note that the data package for Phase I investigations consist of data sheet serialnumbers such as 3001 (map), 4001 (radon), 5001 (indoor gamma), 6001 (outdoorgamma) and 7001 (exposure summary). If a Phase II investigation is required at thisproperty, e.g., soil sampling, data sheet 8001 is assigned. The second Phase Iproperty data sheet package consists of serial numbers 3002, 4002, 5002, 6002 and7002. Phase II an III activities, if necessary, have data sheets with the serial numbersuffix 002.
3) Data sheets are utilized by NDL in the field and returned to MPI. The SerialNumber Assignment Sheet is again initialed and a Chain of Custody Sheet notebookis signed.
Data Sheet Completion
1) Data sheets are completed in pen. Any changes or corrections made to thedata sheets are initialed and dated.
2) All data sheets are signed and dated by NDL's team leader.
3) General conditions of the surveyed property (type and condition of structure, groundconditions, location relative to other structures such as old train tracks or brook) arenoted in the comments section.
4) Data sheets are returned to NDL if they are not completed correctly.
Rationale : Over 150 properties will be investigated during this project making data recording innotebooks impractical. As written, the Field Sampling Plan (FSP) is not consistent with the QualityAssurance Project Plan (QAPjP). The FSP contains examples of the data sheets while the QAPjPindicates that field data will be maintained in a bound notebook. Serial numbers on data sheetsallow Malcolm Pirnie to trace possession of data sheets at all times. Once the sheets arerelinquished to Malcolm Pirnie, copies are made and the originals are placed in the project fileunder lock and key. This reduces the possibility of data being lost or damaged in the field.
