All serial numbers must be submitted online. Upon each submission a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Incentives must be submitted online by February 28, 2015.

Replaced Table Sale and Transfer Agreement In exchange for the trade-in rebate amount as payment from Midmark Corporation, I hereby agree to sell and transfer my old table, as owned and/or used in my medical or clinical practice, to Midmark Corporation. I hereby acknowledge and certify one of the following disposal actions regarding the above table that has been purchased by Midmark Corporation:

I. it was picked up by Midmark’s selling distributor

II. it was or will be donated within ten (10) business days of the date of this agreement to a charitable organization that is a qualified tax-exempt organization under section 501(c)(3) of the Internal Revenue Code;

III. it was or will be destroyed within ten business days of the date of this agreement through its complete disposal or delivery to: a trash disposal facility or service or a destructive recycling facility (for the purpose of recycling into raw materials); or it was or will be destroyed within ten business days of the date of this agreement through its complete disposal or

IV. it will otherwise be immediately and completely taken out of use in or for any clinical purpose whatsoever.
