Title of the project 2. Purpose(is it part of degree / diploma/Ph.D) 3. Details of the research plan indicating overall aims& objectives of the research, hypothesis and importance of the research proposal. Present knowledge and relevant references. Give here the following details: Study design- indicating the total number of cases/samples/animals to be studied. Mode of selection of subjects especially in experiments involving human beings. Equipments and other materials to be used. Methodology/techniques to be employed for evaluating the results including statistical methods. Any potential to obtain patents etc. Duration of Research Project (Period which may be needed for collecting the data & for analyzing the data). Performa, questionnaires, follow up card etc. Patient information sheet and informed consent form in local language. For any drug/device trial, all relevant pre-clinical animal data & clinical trial data from other countries, if available. Statement describing compensation for study subjects for participation and /or study related injuries. Any regulatory clearances required. An agreement to report any serious side effects or adverse drug reactions to IEC. Statement of conflicts of interest, if any. Any other information relevant to the study. 4. If any, preliminary work already done by the Investigator on this subject.e.g. selection of subjects, standardization of methods, with results etc. 5. Links with other departments,institutions for multi-centric projects, collaborations etc. including obtaining of permission for the same. 6. Facilities in terms of equipment, etc., available at the institution for the proposed project. 7. Source of funding and financial requirements for the project. 8. Check list signed by principal investigator, guide (in case of students) & statistician.
