INTRODUCTION RFQ #1906004 Davidson County is seeking a qualified firm to provide engineering design, and construction phase services to replace a culvert at the Lexington Middle School located at 100 West Hemstead Street, Lexington NC 27292. Services may include surveying, design, environmental permitting, consultation, meetings & presentations, and construction related services. The purpose of the Request for Qualifications (RFQ) is to generally describe the work desired by the County and to invite qualified persons or firms to submit written descriptions of their qualifications and proposed approach for providing that work. All firms submitting proposals must have demonstrated experience and expertise in storm water issues, permitting and construction administration services. Experience within the previous three (3) years is preferred. Davidson County is an Equal Opportunity Employer and Service Provider. A committee of individuals interested in the project may interview qualified firms responding to this RFQ in the pt quarter ofFY 2019-2020, and will select a firm to recommend to Board of County Commissioners for final approval and award of contract. SCOPE OF SERVICES

The engineering services required by Davidson County for this project shall generally follow the tasks outlined below. The final scope of services will be negotiated with the selected engineering firm.

Preliminary Design- includes Conducting Stakeholder meetings, finalizing Design and producing Design Development Documents,

Construction Documents - includes final drawings, specifications, and other documentation as necessary

Pre-Construction Services -Agency Approvals - submit for and obtain all necessary local, state and federal agency approvals

Bidding Services -advertise, pre-bid conference, bid opening, bid tabulation, engineer's recommendation, contract award and Board of Commissioners' contract award presentation

Construction Management- includes progress meetings, inspections, submittals, shop drawing review/approval and evaluations of other submittals, change orders, pay applications and project close­out

SCHEDULE OF PERFORMANCE

The schedule will be as mutually agreed at the acceptance of the proposal and execution of agreement with the selected firm.

QUALIFICATIONS-GENERAL CRITERIA

The selection committee may schedule interviews with engineering firms with extensive experience designing and inspecting the construction of storm water culverts and permitting and construction administration services. Past projects need to include some with scopes and costs comparable to or larger than this one. The projects should be within the past three (3) years.

QUALIFICATIONS-SPECIFIC CRITERIA

1. Resumes for project manager, all design team members and construction inspectors of project

2. Past experience/ projects designing public utility (water and sewer) and storm water management design and permitting and construction administration services, including projects costing in excess of $500,000 or more

EVALUATION CRITERIA

Proposals will be evaluated on the information provided in the proposals and contacts with references. Individual criteria will be assigned varying weights at the County's discretion to reflect relative importance. Selection as a qualified firm will be based on the following criteria:

1) 10% Availability and interest in providing services 2) 15% Related experience of the firm including timeliness of services and cost control 3) 35% Related experience of the proposed Project Team to be assigned to this project 4) 15% Project schedule and approach 5) 20% References for similar projects from similar clients 6) 5% Other information regarding the firm that may prove beneficial to the County

SUBMITTAL REQUIREMENTS

Proposals must be limited to a maximum of 20 pages single-sided (10 pages double-sided) with a minimum font size of 12. Advertising material should not be included in the proposal and will not be considered in the selection process. All proposals will become the property of Davidson County and will be evaluated to determine which firm is best qualified to meet the County's requirements.

ADDITIONAL SERVICES At the request of the Owner, the Engineer shall perform services in addition to those described above, and the Owner will compensate the Engineer as mutually agreed at the acceptance of the proposal and execution of the agreement. All services to be completed which are outside the scope of the executed agreement must be submitted on the form below and shall be approved before the additional work is initiated. The Scope Change Authorization Form below shall be included in the appendices of the professional services agreement with the selected firm.
