Introduction The profiles in this booklet represent information about licensed residential care facilities that accepted Medicaid payments in fiscal year 2005 and/or fiscal year 2006. Some facilities not listed here may accept Medicaid; conversely, some facilities that accepted Medicaid previously may chose not to accept Medicaid payments in the future. A list of all licensed residential care facilities is available at the DHHS website, Bureau of Health Facilities Administration. These profiles are designed to help individuals to locate residential care facilities that may meet their needs. The information presented here is provided solely as a service to individuals, it does not constitute an endorsement of any facility. It is intended as a guide only; accuracy is not guaranteed. Residential care facilities are licensed by the Bureau of Health Facilities Administration. There are two types: Residential Care Home Facilities and Supported Residential Care Home Facilities. Listed below are definitions of these types as provided on the NH Association of Residential Care Homes (NH ARCH):

Residential Care Home Facility: Residential Care Homes are homes and facilities where residents need and receive assistance with their activities of daily living. This includes assistance with bathing, personal hygiene, dressing, eating, walking, etc. Other services provided include preparation of special diets, supervision of taking medication and observation of physical and emotional health. Residential Care Homes are operated on a 24-hour basis to provide residential accommodations, personal care, social, occupational and recreational services. Supported Residential Care Home Facility: Supported Residential Care Homes offer the same services as a Residential Care Home. In addition to these services, Supported Residential Care Homes can care for those residents who have greater medical needs. Supported Residential Care Homes offer nursing intervention for those residents who need nursing services, but yet do not need the 24-hour services of a more expensive nursing home.

The information presented in this guide was gathered during a survey completed in 2006 under the Home Care Connections Grant.
