We are committed to a proactive role in providing our employees, dentists and their pa-tients with protection from the dangers of cross-contamination. A charge of $1.75 is added to each case to help defray the costs of our comprehensive compliance program.

PRODuCT REMAKE POLICyAll New Image products carry a 100% satisfaction guarantee for the appropriate term of each product. This covers the full replacement cost of all materials and workmanship of the prosthetic produced according to the dentist’s writ-ten prescription. When a removable product fits the model produced from the dentists’ impression and does not fit intraorally, New Image shall not be responsible for the full cost of refabrication. In this case, if all models and product are returned to New Image, the charge for product remake will be 50% of the original price. If models are not returned to the laboratory, the cost of remake will be 100% of the original price.

In order to give you quality service, New Image needs adequate time for fabricating, inspecting and delivering your work. The schedule refers to full working days in lab Monday thru Friday. Pick-up and delivery days are not consid-ered working days. Pick-up and delivery service is available throughout metro Atlanta for a $4.00 fee per case. Out of town cases should be sent to us by either Priority Mail (at no charge) or United Parcel Service (UPS) airpack (Second Day Service - Blue - $7.95, Next Day Service - Red - $9.95). All out of town cases are returned via United Parcel Ser-vice (UPS) for a $6.00 per case fee.



New Image Guarantee

THIS PROGRAM IS DESIGNED TO ACCOMPLISH THE FOLLOWING:

1. Assist you with case presentations (How many times have you been asked, “How long will this restoration last?” or “Is this appliance guaranteed?”)

2. Assist you in keeping your restorative and removable appliance patients on a regular dental maintenance program. To keep the Warranty valid, the crown and bridge patient must adhere to a semi-annual dental maintenance program (cleaning and exam). The full or partial denture patient must have the pros-thesis relined as recommended by the dentist, in addition to the 6-month recall.

3. Increase the traffic or patient load for the dentist using this service as a tool for his recall program. This program is available on crown and bridge cases. It is self-explanatory to the patient as de-tails are on the back of the Warranty.*

3. The maintenance schedule on this Certifi-cate must be documented by the attending dentist each visit to validate this Warranty.

4. Full or partial dentures must be relined as recommended to the attending dentist by New Image Dental Laboratory, Inc.

5. Within the limitations of this Warranty, the prosthesis, written work authorization and the Warranty Card must be shipped together to New Image Dental Laboratory, Inc.
