DESCRIPTIONThe CSI implants are designed to prevent excessive pronation of the subtalar joint.These joint implants are fabricated from typical biocompatable implant materials.The CSI is fabricated using, titanium alloy, ASTM F136.
INDICATIONS
1. Flat foot treatment in children and adolescents2. Congenital flat foot3. Unsuccessful long term orthopaedic treatment (shoes, insoles…)4. Tarsal coalitions5. Painful flat foot6. Supple deformity in posterior tibial tendon dysfunction7. Paralytic flat foot8. Subtalar instabilityCONTRAINDICATIONSGeneral contraindications for the use of these implants for joint reconstruction include:1. Significant bone demineralization.2. Inadequate neurovascular status.3. Inadequate skin or musculotendinous system.4. Inadequate bone stock.5. Psychologically unsuitable patient.6. Active sepsis.
WARNINGS (See also the Patient Counseling Information Section)Strenuous loading, excessive mobility, and articular instability all may lead to acceler-ated wear and eventual failure by loosening, fracture, or dislocation of the device.Patients should be made aware of the increased potential for device failure if exces-sive demands are made upon it.
Implants are mechanical devices that can be worn away, fatigued or broken. Animplant site may become infected, painful, swollen, or inflamed. The status of theadjacent bone and soft tissue may be inadequate to support the implant, or maydeteriorate in time resulting in instability, deformity, or both. The benefits from implantsurgery may not meet the patient’s expectations or may deteriorate with time, neces-sitating revision surgery to replace the implant or to carry out alternative procedures.

Revision surgeries with implants are not uncommon.
PRECAUTIONS
ADVERSE EVENTSPotential adverse events reported with joint prostheses include pain, loosening, frac-ture, dislocation, or infection. There have been some reports of patients with metalsensitivity reactions following joint replacement. Implantation of implant materialsmay result in foreign body reaction adjacent to the implant site. Injury to the surround-ing nerves, blood vessels, tendons, or soft tissues can occur as a consequence ofimplanting this device.
SURGICAL PROCEDURESA surgical procedure for use of this implant device is available. It is the responsibilityof the surgeon to be familiar with the procedure before use of these products. Inaddition, it is the responsibility of the surgeon to be familiar with relevant publicationsand consult with experienced associates regarding the procedure before use.
PATIENT COUNSELING INFORMATION (See also Warnings)Providing each patient scheduled for implant surgery with documented counseling ofpotential complications and alternatives, which may include non-implant proceduressuch as soft tissue reconstruction or arthrodesis, prior to surgery is necessary. Inaddition to the patient related information contained in the Warnings and AdverseEvents sections, the following information should be conveyed to the patient:
1) Adverse effects may necessitate reoperation, revision, or fusion of the involvedjoint. 2) While the expected life of joint replacement components is difficult toestimate, it is finite. These components are made of foreign materials which areplaced within the body for the potential restoration of mobility or reduction of pain.However, due to the many biological, mechanical and physiochemical factors whichaffect these devices, the components cannot be expected to withstand the activitylevel and loads of normal healthy bone for an unlimited period of time.
STERILIZATIONThis component has been sterilized by ethylene oxide or gamma radiation. The implantis provided sterile in an undamaged package. If either the implant or the packageappears damaged, expiration date has been exceeded, or if sterility is questioned forany reason, the implant should not be used. Do not resterilize.
Trial sizer components are available to avoid having to open the sterile package priorto prosthesis implantation. The implant should be removed from its sterile packageonly after the implant site has been prepared and properly sized.

WARNUNGEN (Siehe auch den Abschnitt Patienteninformationen)Extreme Belastungen, große Mobilität und Gliedmaßeninstabilität können zu frühzeitigemVerschleiß und eventuellem Versagen der Prothese durch Lockern, Brechen oderVerrutschen führen. Der Patient sollte über das erhöhte Risiko eines Versagens derProthese bei Überbeanspruchung aufgeklärt werden.
Prothesen sind mechanische Hilfsmittel, die verschleißen oder brechen können. EineImplantationsstelle kann sich infizieren, schmerzen, anschwellen oder sich entzünden.Der Zustand des Knochens und des Weichgewebes kann unzureichend sein, um dieProthese aufzunehmen, oder es kann sich mit der Zeit verschlechtern und zu Instabilitätund Deformierung oder beidem führen. Solche Eingriffe im Zusammenhang mit Prothesensind nicht außergewöhnlich.
ACHTUNG
Nach dem Entfernen der sterilen Verpackung dieProthese bis zum Zeitpunkt der Implantation in ein Bad mit steriler Salzlösung legen.
Die Prothesen sollten ausschließlich mit stumpfen Instrumenten gefasst werden, umKratzer, Schnitte und Kerben auf der Prothese zu vermeiden.
Eine gründliche Vorbereitung der Implantationsstelle und die Wahl der richtigenProthesengröße erhöhen die Chance eines erfolgreichen Eingriffs. Zur Erleichterungder Knochenvorbereitung ist eine große Auswahl von Musterprothesen erhältlich. DieProthese in der richtigen Größe erst aus der sterilen Packung nehmen, wenn dieImplantationsstelle ausreichend vorbereitet wurde.
KOMPLIKATIONENBerichtete mögliche Komplikationen mit prothesen sind Schmerzen, Lockerung, Fraktur,Verrutschen oder Infektionen. Bei einigen Patienten traten Überempfindlichkeiten gegenMetall nach dem Gelenkersatz auf. Die Implantation von Materialien können zubstoßungsreaktionen gegen Fremdkörper im Umfeld der Implantationsstelle führen.
PATIENTEN INFORMATIONEN (Siehe auch Warnung)Jeder Patient, bei dem eine Implantation geplant ist, sollte vor der Operation fundiertüber mögliche Komplikationen und Alternativen aufgeklärt werden, zu denen auchVerfahren ohne Implantat gehören, wie eine Rekonstruktion der Weichteile oderGelenkversteifung. Zusätzlich zu den patientbezogenen Informationen in den AbschnittenWarnung und Komplikationen sollte der Patient über Folgendes informiert werden:
1) Komplikationen können eine neue Operation, eine neue Begutachtung oder Fusiondes betreffenden Gelenks erfordern.
STERILISATIONDieses Produkt wurde mit Ethylenoxid oder mithilfe von Gammastrahlen sterilisiert. Eswird steril in einer unbeschädigten Verpackung geliefert. Sollte entweder die Protheseoder die Verpackung beschädigt sein, das Verfalldatum überschritten oder die Sterilitätaus irgendeinem Grunde zweifelhaft sein, sollte die Prothese nicht verwendet werden.Nicht erneut sterilisieren.

CONTRE-INDICATIONSLes contre-indications générales d'utilisation de ces implants pour la articulaire comprennent :1. Déminéralisation osseuse importante.2. Etat neurovasculaire inadapté.3. MISES EN GARDE (Voir aussi la partie relative à l’information et conseils destinésau patient).Contraintes importantes, mobilité excessive et instabilité articulaire peuvent toutesaccélérer l’usure et aboutir à une éventuellement défaillance par détachement, frac-ture ou dislocation. Les patients doivent être conscients du risquepotentiel de la défaillance de l’appareil si ce dernier subit des contraintes excessives.
Les implants sont des dispositifs mécaniques qui peuvent s’user, se fatiguer ou secasser. Un site d'implantation peut s’infecter, devenir douloureux, se tuméfier ou.L’état de l’os adjacent et du tissu mou peuvent être inappropriés pour soutenir l’implant ou peuvent se détériorer avec le temps et aboutir à une instabilité, à une difformité ou aux deux. Les avantages de l’implantation chirurgicale peuvent nepas répondre aux attentes du patient ou peuvent s’altérer avec le temps et nécessiterainsi une chirurgie pour remplacer l’implant ou imposer la mise en oeuvre de procédures alternatives. Les interventions chirurgicales à l’aide d’implants ne sont pas exceptionnelles.
PRÉCAUTIONS
Les implants doivent être gardés dans une solution saline stérile aprèsqu’ils aient été retirés de leur emballage stérile avant implantation.
Il convient de manipuler les implants uniquement avec des instruments émousséspour éviter de rayer, de couper ou d’entailler le dispositif.
Une préparation méticuleuse du site d’implantation et le choix de la taille appropriée del’implant augmentent la chance de succès de l’implantation. Il existe une gammecomplète de gabarits pour aider à la préparation de l’os. On recommande de retirerl’implant de taille appropriée de son emballage stérile seulement après que le sited’implantation ait été préparé et correctement dimensionné.
EFFETS INDESIRABLESParmi les effets indésirables potentiels des prothèses articulaires on a signalé ladouleur, le détachement, la fracture, la dislocation ou l’infection. On a signalé quelquescas de patients qui ont manifesté des réactions de sensibilité au métal après
INFORMATION ET CONSEILS DESTINES AU PATIENT (Voir également lapartie Mises en garde)Il convient de fournir à tout patient programmé pour une implantation chirurgicale desinformations détaillées sur les complications et alternatives potentielles - qui peuventcomprendre des procédures non-implantatoires comme la reconstitution des tissusmous, l’arthrodèse. En plus de l’information relative au patient, contenue dans lesparties Mises en garde et Effets indésirables, il convient de faire savoir au patient que:
1) Les effets secondaires peuvent nécessiter une nouvelle opération, une correctionou une fusion de l’articulation concernée.
STERILISATIONCe composant a été stérilisé à l’oxyde d’éthylène ou aux rayons gamma. L’implant estfourni stérile dans un emballage intact. Si l’implant ou son emballage apparaissentendommagés, si la date de péremption est dépassée ou si la stérilité est mise en doutepour une raison quelconque, il faut proscrire l’emploi de cet implant. Ne pas re-stériliser. Il existe des gabarits d’essai pour vous éviter d’ouvrir un emballage stérileavant l’implantation de la prothèse. Il faut retirer l’implant de son emballage stérileseulement après que le site d’implantation ait été préparé et correctement dimensionné.

DO NOT clean, re-sterilize or reuse as this may damage or compromise performance of the devices and may expose patient to risk of transmitting infectious diseases.
MRI WARNINGThis product has not been evaluated for safety and compatibility in the MR environ-ment. They have not been tested for heating, migration, or image artifact in the MR environment. The safety of the Tornier products in the MR environment is unknown. Scanning a patient who has this device may result in patient injury.

A complete range of trial sizers isavailable to aid in bone preparation. It is suggested that the proper size implant beremoved from its sterile package only after the implant site has been prepared andproperly sized.

2) Comme la longévité des composants de remplacement de l’articulation est difficile a estimer, elle est limitée. PRECAUCIONES

WAARSCHUWINGEN (Zie ook het gedeelte over informatie over counseling van depatiënt)Sterke belasting, overmatige mobiliteit en articulaire instabiliteit kunnen alle leiden totversnelde slijtage en uiteindelijk falen door loslating, fractuur of dislocatie van hetimplantaat. Patiënten moeten bewust gemaakt worden van de verhoogde kans opfalen van het implantaat als hier overmatige veel van gevergd wordt.

VOORZORGSMAATREGELEN
Voor implantaten mogen uitsluitend stompe instrumenten gebruikt worden, om krassen,snijden of knakken van het implantaat te voorkomen.
Zeer nauwgezette voorbereiding van de plaats van het implantaat en de keuze van dejuiste maat implantaat verhogen de kans op een succesvol resultaat. Een volledigassortiment proefcomponenten is verkrijgbaar ter ondersteuning van debotvoorbereiding. Geadviseerd wordt om het implantaat met de juiste maat pas uit desteriele verpakking te halen nadat de plaats van het implantaat is voorbereid en dejuiste maat heeft.
BIJWERKINGENDe mogelijke bijwerkingen die gemeld zijn bij het gebruik van protheses zijn pijn,loslating, fractuur, dislocatie en infectie. Er is ook een aantal meldingen van patiënten
CHIRURGISCHE PROCEDURESEen chirurgische procedure voor het gebruik van dit implantaat is beschikbaar. Het isde verantwoordelijkheid van de chirurg om op de hoogte te zijn van de procedurevoordat hij deze producten gebruikt. Het is ook zijn verantwoordelijkheid om op dehoogte te zijn van relevante publicaties en ervaren collega’s te raadplegen over deprocedure voordat hij deze verricht.
PATIËNTINFORMATIE (Zie ook Waarschuwingen)Het is noodzakelijk om alle patiënten die implantaatchirurgie zullen ondergaan vóór deoperatie te voorzien van informatie over de mogelijke complicaties en alternatieven.Alternatieven kunnen zijn: niet-implantaat procedures zoals reconstructie van hetzachte weefsel of artrodese. Naast de informatie die betrekking heeft op de patiëntzoals deze is opgenomen onder Waarschuwingen en Bijwerkingen moet de volgendeinformatie aan de patiënt worden meegedeeld:
1) Bijwerkingen kunnen heroperatie, revisie of fusie van het betrokken gewrichtnoodzakelijk maken.
STERILISATIEDeze component is gesteriliseerd met ethyleenoxide of gammastralen. Het implantaatwordt steriel geleverd in een onbeschadigde verpakking. Als het implantaat of deverpakking beschadigd lijken te zijn, als de uiterste gebruiksdatum is overschreden ofals om de een of andere reden twijfel bestaat over de steriliteit, mag het implantaat nietgebruikt worden. Niet opnieuw steriliseren.

AVVERTENZE (si rimanda al paragrafo relativo alle Informazioni per la Consulenzaal Paziente)I carichi estenuanti, una mobilità eccessiva e l’instabilità articolare costituiscono fattoriin grado di accelerare l’usura e l’eventuale danno del dispositivo attraverso un processodi allentamento, frattura o dislocazione. I pazienti devono essere avvertiti del fatto cheil rischio di danni al dispositivo aumenta in caso di utilizzo poco moderato dello stesso.
Le protesi sono dei dispositivi meccanici soggetti a logoramento, affaticamento erotture. La sede di una protesi è soggetta al rischio di infezioni, dolore, edema oinfiammazione. Le condizioni dell’osso e del tessuto molle adiacenti potrebbero essereinadeguate a sostenere la protesi, oppure andare incontro a deterioramento graduale,con conseguente instabilità e/o deformità. E’ possibile che i benefici apportati dallaprotesi non rispondano alle aspettative del paziente o perdano la loro qualità nel tempo;in tal caso si renderebbe indispensabile eseguire un intervento chirurgico di revisione,atto a sostituire la protesi o ad eseguire delle procedure alternative. Gli interventichirurgici di revisione di protesi non sono rari.
PRECAUZIONI
Le protesi vanno maneggiate esclusivamente con l’ausilio di strumenti smussati, perevitare di graffiarle.
Una preparazione accurata del sito di impianto ed una corretta scelta delle dimensionidella protesi aumentano le probabilità di successo dell’intervento. Una gamma completadi dispositivi di prova è disponibile per la predisposizione dimensionale dell’osso. Siconsiglia di estrarre la protesi delle giuste dimensioni dalla sua confezione sterilesoltanto dopo aver predisposto e dimensionato adeguatamente il sito di impianto.

INFORMAZIONI PER LA CONSULENZA AL PAZIENTE (si rimanda alleAvvertenze)A ciascun paziente per il quale si è programmato un intervento di impianto di protesioccorre fornire una consulenza documentata in relazione alle possibili complicazioniche potranno insorgere e alle alternative, che includono un intervento senza l’impiantodi protesi; quest’ultimo può consistere nella ricostruzione dei tessuti molli onell’immobilizzazione dell’articolazione. Oltre alle informazioni per il paziente riportate aiparagrafi “Avvertenze” e “Eventi negativi”, è indispensabile fornire al paziente leseguenti informazioni:1) l’eventuale insorgenza di eventi negativi può rendere necessario un secondo interventochirurgico, una revisione o la fusione dell’articolazione in questione;2) La durata dei componenti sostitutivi dell’articolazione è difficile da prevedere ma, inogni caso, avrà un termine. Questi componenti sono realizzati in materiali non fisiologicie collocati all’interno dell’organismo per il potenziale ripristino della mobilità o per lariduzione del dolore. Ciononostante, per via dei diversi fattori di natura biologica,meccanica e fisico-chimica che influenzano questi dispositivi, non ci si può aspettareche i componenti siano resistenti, per un tempo illimitato, al livello di attività e allesollecitazioni da carico ai quali sono sottoposte le normali ossa sane.
STERILIZZAZIONEQuesto articolo è stato sterilizzato con ossido di etilene o con radiazioni gamma. Laprotesi è fornita in condizioni sterili all’interno di una confezione integra. PROCEDIMIENTOS QUIRÚRGICOSHay disponible un procedimiento quirúrgico para el uso de este dispositivo de implantación.Es la responsabilidad del quirófano familiarizarse con el procedimiento antes de utilizarestos productos. Antes de usar dicho dispositivo, el quirófano es, además, responsablede familiarizarse con las publicaciones pertinentes y mantener contacto con asociadosexperimentados respecto al procedimiento.
