ENDODONTIC CASES AND ITS FACILITIES AT COMMUNITY HEALTH CENTERS IN WEST JAVA PROVINCE INDONESIA

ABSTRACT

Dental caries has been a government’s attention in dental health issues now days. An invasive caries could be started as a port de ‘entry of root canals infections which classified in ICD-DA as endodontic cases. The aim of this study is described the amount of endodontic cases at Community Health Centers in West Java Province and its facilities. The study conducted by collecting secondary data derived from Recording and Reporting System Integrated Health Center (SP2TP) 2015. The data reported to City/District Public Health Service Office in city/district of whole West Java Province. The result showed that West Java Province Health Service Office data collection system was inadequate to link with all district/city, only 37 % have adequate health data system. About 10 districts/city 0.012 % of population needs endodontic treatment and 30.2 % from population can take endodontic treatment in Community Health Center, the rest can get services at the hospitals, but there were 7.74% population do not have access at all.

INTRODUCTION

Dental health condition in Indonesia is still an issue, National Health Survey in 1995 shows that toothache ranks 6th community diseases complaint.1 The next survey conducted in 2004, The Household Health Survey (SKRT) explained that 39% population have dental problems,2 in 2007 survey this figure dropped to 23,4%.3

Basic Health Research 2007 (Riskesdas) results explained that DMF-T index for Indonesia is 4,85 with D-T index 1,22; M-T 3,86; and F-T 0,08. The main cause of this problem is caries, Riskesdas data showing 43,4% population with dental problems heve teeth with caries active while ever had dental caries is 67,2%.3

Dental health issues that have been govenrment’s attention limited in dental caries. Though there is a continuation of caries, that is an infection of root canals which in Classification of Diseases to Dentistry and Stomatology (ICD-DA) named as endodontic

cases.4 Endodontic cases not be a concern can be illustrated by the exclusion of the pulp treatment action as one of the prosedures covered by Badan Penyelenggara Jaminan Sosial Kesehatan (BPJS Kesehatan) insurance in primer health services. Need referral to hospital to treatment of pulp, even though so mnay hospitals in Indonesia does not have endodontist. 5

This study will describe the amount of endodontic cases in Jawa Barat Province Community Health Centers and its care fasilities which available in the city or distric.

METHODS

The riset was conducted by collecting secondary data derived from Recording and Reporting System Integrated Health Center (SP2TP) 2015 which reported to City/District Public Health Service Office in city/distric the whole of West Java Province.

Data were taken by dental programs officers of City/District Health Service Office in province meeting. In that meeting participants were given the chance to convey endodontic problems in their city/district.

RESULTS

Evidently City/District not all The Health Service Office have a good data collection system, from 18 district and 9 city in West Java Province, only 10 (37 %) have adequate health data system. Endodontic cases in 10 city district/city in West Java Province shown in Table 1. Total cases in all district/city is 1,666.8 cases / year, the highest cases is in Bogor district as 8.036 cases / year, and the lowest cases is in Pangandaran district as 27 cases / year.

In Recording and Reporting System Integrated Health Center (SP2TP), there are

no endodontic treatment data. To give overview about endodontic treatment in district/city, to dental programs officers of City/District Health Service Office raised questions about presence or absence of endodontic rates and endodontic materials/instruments procurement in Community Health Center. If no that, will be no endodontic treatment in

Community Health Center and the patients should be reffered to hospital. The next question asked to know Existence Endodontist at all hospitals in district/city area, the results shown in Table 2. Table 2 : Endodontic Treatment Support

From 10 districs/city that have data, there are 5 (50%) districs/city have Endodontis. Two districs/city have Endodontics in government and private hospitals, two districs/city only in government hospital, and one districs/city only in private hospital.

DISCUSSION

This research shown total cases in 10 district/city West Java Province is 16,668 case / year. Based on data from the statistics bureau (BPS), total population in this district/city is 13,388,0736. Compared with amount of endodontic cases, meaningful 0.012 % of population needs endodontic treatment.

From 10 districts/city, only 3 that have endodontic rates and endodontic materials/instuments procurement in Public Health Center, namely Kota Bekasi, Kota Cimahi, and Kabupaten Majalengka. This means that only in three cities, endodontic treatment can be done at Public Health Center level. Total population in these 3 district/city is 4,042,521, if compared with the population in 10 district/city it means 30,2 % from population can take endodontic treatment in Public Health Center.

In Indonesian health insurance system with provided by BPJS Kesehatan, endodontic treatment are not covered in Public Health Center5, thus the absence of endodontic treatment at Public Health Center can not be regarded as a fault. In this system, patient with endodontic problem should be referral to the hospital.

The problem is, that not all hospitals have endodontist, from 10 district/city only 5 have an endodontist. The good condition only in Kota Bekasi and Kota Cimahi because endodontic treatment available at Public Health Center and there are endodontist in the hospitals. There are no endodontist in 5 distrisc/city hospital, namely Kabupaten Bogor,

Kabupaten Kuningan, Kabupaten Majalengka, Kabupaten Pangandaran, and Kabupaten Sumedang. The worst condition because there are no endodontic treatment both in Public Health Service nor in hospitas, there are in Kabupaten Pangandaran and Kabupaten Kuningan. The population in this district is 1,035,5896, if compared with the population in 10 district/city it means 7,74% population do not have access to endodontic treatment in they own district/city.

CONCLUSSION

The West Java Province Health Service Office do not have a good data collection system, from 18 district and 9 city in West Java Province, only 10 (37 %) have adequate health data system.
