• Femme 32 ans• Découverte par la palpation d’une tuméfaction
cervicale droite, mobile lors des mouvements de déglutition, ferme, lisse, régulière, de 2 cm de diamètre, sans hypertrophie du reste de la glande thyroïde, sans adénopathie palpable
• Pas de disgrâce esthétique, pas de gêne fonctionnelle, pas de signe suggestif de dysfonction thyroïdienne.
• Grand-mère maternelle opérée d’un goitre apparemment bénin, mère surveillée pour un petit goitre

Caractérisation ganglionnaire
Trois critères fondamentaux de malignité opposent ganglion normal et adénopathie : – forme : index de Steinkamp < 2 en cas d’adénopathie ( rapport du plus grand / le plus petit des 3 diamètres) – structure : disparition du hile systématique dans les adénopathies – vascularisation : l’adénopathie perd le caractère central de sa vascularisation qui peut devenir diffuse, anarchique, mixte ou périphérique.

• What is the role of FNA biopsy ?– R5 FNA is the procedure of choice - Recommandation A– R6 Cystic nodules that reatedly yield non diagnostic aspirates need close
observation or surgical excision. Surgery should be more strongly considered if the cytologically non diagnostic nodule is solid – Recommandation A
– R7 If diagnostic of malignancy, surgery is recommended – Recommandation A– R8 At the present time, the use of specific moleculars markers for
indeterminate nodules is not recommended -Recommandation I– R9 If cytolgy is indeterminate (follicular lesion), a radionuclide thyroid scan
should be considered. If a functioing nodule is not seen, surgery should be considered – Recommandation B
