35Gas leakage from abatement equipment due to poor work

Direct cause

After the exhaust gas abatement equipment was replaced, gas flowed from a leak at the pipe connection, causing a gas leak that the gas detector reported.

The O-ring on the connection flange was distorted.The airtightness of the connection was not checked before the equipment was used again.

After replacing the exhaust gas abatement equipment and before flowing the gas, check the state of the gas line, including checking the airtightness.

The container with the chemical solution tipped over.Appropriate protective equipment was not being used.

The chemical solution bottle was unstable.Since the working posture was not decided and the angle of the arm differed depending on the worker, the liquid splashed out of the range protected by the protective equipment.

Stabilize the chemical solution bottle by creating a fixed container for the bottle. Review protective equipment and the work procedure manual, and perform the work using an appropriate work posture.

The worker tried to lock the tube cutter because the blade was still sticking out, but a finger touched the blade.

The tube cutter was stored in the tool tray with its blade sticking out. The work procedure manual lacked a description of the risk.

Review the work procedure manual to keep the blade properly stored when not in use.Use a case that prevents the tube cutter from being stored if its blade is not properly stored.

Falls and collisions

When on a work platform, the worker didn't check the position of the steps and only assumed where the steps were when trying to get off the platform.

The steps were rotated 90 degrees while the work was being done. The worker was unclear about where the steps were located.

Use a platform with handrails on the steps.Assign someone as a guard to keep watch when any worker goes up or down the steps.Clarify the location of the steps in the work procedure manual.
