Synopsis of the incident:

Vessel arrived at Merak, Indonesia on End Dec 2010 at 1148 lt and berthed Dover terminal at 14:36LT on the same day to discharge the charterer’s cargo. Subsequently the weather has deteriorated and vessel encountered gusty winds with heavy swell. The attempt to un berth the vessel was unsuccessful in rough weather condition. Eventually two forward mooring ropes were parted as vessel was surging against the shore fenders. One shore fender was damaged and vessel sustained dent damage on her shell plate.

Design of terminal: The Dover terminal located at Lat: 05-57.65 S, Long: 105-59.70 E. The terminal is open to sea and no breakwater provided to protect the ship’s from seasonal weather.

Ship’s status during incident:Vessel was heading South, with her port side alongside to terminal. 6 Mooring ropes of 2-2-2 & 3-1-2 formation was used at forward and aft station respectively.Tidal current: NE’ly(about 0.8kts),Wind force: 30-35kts, W’lySwell direction W’ly (about 3mters)Starboard Anchor was used during berthing and 3 shackles was paid out.Draft: F: 6-40M, A:7-30M, Displacement: 9500 MT

Terminal restriction for heavy weather.

-Officially there is no restriction for heavy weather,-But as per SSSCL agreed as follows:-*Stop cargo: 25kts. -*Disconnect: 30kts.- *Un berth: 35kts.

Mooring arrangement:• 6 nos megaflex ropes were in use both fwd and aft ( 2-2-2 & 3-1-2 formation F & A respectively)•The ropes dia was 44 mm with MBL 38 T•Four nos ropes were on mooring drums whereas, two made fast on bitts both F & A.• One headline and one breast line was parted and subsequently replaced.

Damage to Ship:• 2 nos mooring ropes were parted.• Indented port side shell plate between Frame 98 & 103 at load water line with approx dimension L3500 mm x B 3500 x D 150 mm at strake E & H, including internal members and scantlings.Damage to shore:• 1 spool type shore fender at breasting dolphin no.2 was damaged. Damage noted to be on the hyper cell rubber part. However, no damage to the concrete structure was evident.

Causal factors:

• Terminal design less than adequate. No break water to protect the ships from seasonal rough weather.• Port logistics was less than adequate (In adequate supply of powerful tugs upon request).• Compliance to port information and guidance provided to ship by the company was less than adequate

Lessons Learned:

• Obtain weather forecast prior berthing and closely monitor while alongside. Un berth if necessary in advance rather than waiting for last minute.• Keep additional mooring ropes standby. Double up moorings if vessel require to stay alongside during bad weather & in case unable to un berth in time.• Liaise with agent, vessel’s operator effectively to ensure prompt assistance, support from shore.• Master shall request operator to supply additional stand by tug when deemed necessary.

Case 2: Hull dent by shore fender

Synopsis of the Incident:On 21st December 2010 at 0754 LT, vessel berthed port side alongside to Vopak #1 terminal, Merak.Her draft was F -5.80 M and A - 7.10 M ,with a displacement of 9830 MT. During deck round, duty officer observed that vessel's hull plating in way of slop tank (S), between Frame # 38 & # 41 was slightly dented.

Investigation revealed following:• Some frontal pad was missing from the fender and metal of frontal frame was exposed.• The wind was 15 kts and gusting to 20-22 kts• Vessel was surging moderately.•Crew did not verify the fender condition adequately during berthing.•The tyre fenders from vessel were used to reinforce the missing frontal pads and protect the ship’s hull coming in contact metal frontal frame.

Lessons learned: • The fender condition shall be verified prior berthing.• Temporary measures such as portable fenders to be placed when shore fendering arrangment is inadequate.• Hourly checks to be carried out diligently.
