What is Independence Care System (ICS)?
ICS is a nonprofit managed long-term organization supporting seniors and adults with physical disabilities and chronic conditions to live at home and participate fully in community life.
What are the Characteristics of the ICS Care System?• Responsive: What’s important to you and
your family is important to us• Coordinated: We work with you to get the
health care and social services you need• Expert: A highly skilled, dedicated,
diverse staff, advocating for you and with you to be as independent as you can be
• Empowering: Giving you the knowledge and skills you need to make informed decisions

What Makes ICS Different?

In addition to providing all services mandated by the NYS Department of Health, ICS also provides a number of additional services that meet the unique needs of its membership.

Specialized services
What are some of ICS’s Specialized Programs and Services? • A community-based wheelchair
evaluation, repair and maintenance workshop
• Wound Care & Prevention Program• A Women’s Health Program (ICS
members can obtain mammography and OB-GYN care from multiple HHC facilities)
• Teams of nurses and social workers specialized in working with people with dementia; spinal cord injuries; and multiple sclerosis (MS)
What are some of ICS’s Specialized Programs and Services? (Con’t.)
• The first MLTC plan to have Consumer-Directed Personal Assistance Service (allows members to hire, train, and supervise their own personal care aid)
• Member advocacy program staffed by prominent Disability Rights Advocates
• Social programing run out of our Bronx and Downtown Brooklyn Offices
• Medicaid Eligibility and Surplus/Trust Coordinators
What are the Eligibility Requirements for Enrolling into ICS’s Medicaid Managed Long-Term Care Plan?
An individual must:• Have a physical disability or chronic illness that will last
longer than 120 days and require assistance with activities of daily living
• Receive or be eligible for Medicaid• Be 18 years or older• Live in the Bronx, Brooklyn, Manhattan, or Queens• Be able to stay safely at home
Activities of Daily Living Include…
• Bathing• Walking• Dressing• Utilizing the toilet• Preparing food• Going to medical appointments• Remembering what medications to take
State nurse assessments are done to determine if prospective members need support in the above areas. If so, they are potentially eligible for ICS services.

Referring someone to ICS is quick and easy. Go to the ICS website: www.icsny.org and just click on the “Get Started” tab at the top of the page to fill out the web referral form.
