CHEMICAL MODIFIERS OF RADIATION INJURY
- PROTECTORS: Agents given prior to or at the time of irradiation to
prevent or reduce damage.
Amifostina. N-acetyl-L-cysteina. Diethydithiocarbamate. Tempol. SOD.
- MITIGATORS : Agents given during or shortly after a course of
radiation therapy, before symptoms of toxicity appear.
Pilocarpine. Erythropoietin. G-CSF. GM-CSF. Thrombopoietin.
rHuKGF (Palifermin). Prostaglandins. Benzidamina. Sulfasalazina.
- THERAPEUTICS: Agents given after symptoms of toxicity appear.
Oxígeno hiperbárico (osteoradionecrosis, cistitis y proctitis rádicas).
Pentoxifilina+Tocoferol (fibrosis). Interferon gamma.
Estatinas (toxicidad intestinal). Bevacizumab (radionecrosis cerebral).
Amifostine
Primer y único radio-protector aprobado por la FDA (Junio 1999)
Phosphorothioate (prodroga) que penetra y se activa
intracelularmente tras su desfosforilación por la fosfatasa alcalina.
En su forma desfosforilada difunde libremente en la célula
actuando como scavenger de los radicales libres.
Se concentra más rápidamente en los tejidos sanos que en los
tumorales:
· hipoperfusión tumoral
· acidosis tumoral
· menor expresión de fosfatasa alcalina en los tumores
p=0,008
Xerostomía aguda G2: 78% vs 51% (p<0,0001)
Xerostomía crónica G2: 57% vs 34% (p=0,002)
Producción media de saliva: 0,26 g vs 0,10 g (p=0,04)

Acción paracrina sobre el receptor FGFR2b expresado
principalmente en la membrana de las células epiteliales.
La expresión de FGF 7 es estimulada por noxas del tejido
epitelial. Papel importante en la reparación cicatricial.
Acción:
· Estimula la proliferación, migración y diferenciación de las células
epiteliales.
· Relacionado con la reparación del DNA
· Efecto protector contra ROS.

CONCLUSIONES
- Vida civil: La cito(radio)protección es un amplio y dinámico
campo de investigación (guerra nuclear, terrorismo nuclear,
conquista del espacio). Múltiples estudios preclínicos y clínicos en
curso.
- Medicina: Los avances en investigación para el uso civil de los
citoprotectore están repercutiendo y repercutirán en medicina.
Preocupación clínica de que los tratamientos puedan afectar al
control tumoral.
Búsqueda de factores diferenciales entre tejidos sanos y tejidos
tumorales: vías de administración, fisiológicos (pH, hipoxia,...),
genéticos (p53).
