DIGITAL SOLUTIONS SUPPORTING CONTINUUM OF CARE FOR FRAILTY PREVENTION IN OLD ADULTS
OMC webinarsOctober – November 2020
-
DisclaimerThe procurers are carrying out Open Market Consultations (OMC) with interested
suppliers, seeking to establish dialogue, collect feedback about the scope of the
procurement and its feasibility of development within the project timeline. This
presentation was used during the OMC webinars hosted by the eCare procurers
between October and November 2020.
Some of the information presented below is therefore tentative, subject to
changes and may differ from the information included in the Call for Tenders.
Please note that this is pre-tender engagement with the supply chain. It is not, in
any way, an assessment or evaluation of suppliers and does not give any
advantage in the forthcoming Call for Tenders.

in brief

The problem to be addressed: frailty in an ageing society
Population ageing
has profound
implications for the
planning and
delivery of health
and social care
The most
problematic
expression of
population ageing is
the clinical condition
of frailty
Although frailty is
highly prevalent no
country in Europe
has adopted a
systematic process
for its prevention
or monitoring
-
FALLSFrail older adults are likely to
experience recurrent falls.
1 OUT OF 41 out of 4 adults over 85 years of
age is living with frailty
LONELINESSFrailty does not occur in silos; it usually
comes along associated with other risk
factors such as loneliness
ANXIETYPeople with frailty are at risk of
developing conditions such as
anxiety and depression
-
to deliver disruptive digital solutions for
the prevention and comprehensive
management of frailty to encourage
independent living, wellbeing and to
relieve health and care services budget
pressure.

eCare should
▪ empower the patients and their healthcare professionals
▪ take the patients’ preferences and choices into account (individualised care) and
▪ allow to agree on goals and approaches, making the patient an equal partner in the care process

TO ADDRESS
UNMET
NEEDS
of the procurers relating to the
multiple dimensions of frailty
management (building blocks)
INTEGRATED
SOLUTIONSas there are individual solutions
for some of these dimensions,
but research by the procurers
showed that there is no
solution currently available on
the market which integrates
seamlessly all the dimensions
and is interoperable with the
procurers’ existing systems
Aim of the procurement
-
Frailty is understood in the eCare Project framework
as an age-related state of decreased physiological
reserves characterized by a weakened response to
stressors and an increased risk of poor clinical outcome.
[…] Independently of pathophysiological
conceptualization, it is assumed that frailty is a dynamic
process that leads to a spiral of decline in various
functional domains and that exacerbates risk of geriatric
syndromes.
