Prescriptions and Your Pocketbook: An AARP Pennsylvania Study1

Background

Prescription drugs are the fastest rising component of personal health expenditures in the United States. Spending on prescription drugs surged for the fourth consecutive year, climbing 17.1 percent in 2001. The average cost of a prescription increased to $49.84, a rise 10.1 percent between 2000 and 2001. Additionally, the number of prescriptions dispensed at pharmacies rose 6.8 percent to 3.1 billion.2 Pennsylvania residents use more prescriptions drugs and pay more for the prescriptions they fill. In Pennsylvania, the average price of a retail prescription was $50.45 in 2001.3 This is 1.2 percent above the national average price of a retail prescription. Moreover, the average number of prescriptions in Pennsylvania per capita is 12.6, compared to the national average of 10.9. This is 16 percent above the national average number of prescriptions. Pennsylvania is home to the nation’s second oldest population; the elderly make up 15.6 percent of the state’s population.4 Moreover, those who are 85 and older are the fastest growing segment of the population. Because use of prescription drugs tends to increase with age,5 the cost of prescription medication is especially salient to older Pennsylvanians. In Pennsylvania, the Pharmaceutical Assistance Contract for the Elderly (PACE) program helps low-income elderly with the costs of their prescription medications. In January 2002, the average PACE participant filled 4.8 prescriptions per month; this is a four percent increase from the previous year.6 The average cost per PACE

prescription claim is $40.06 in 2001; this represents a nine percent increase from the average PACE prescription cost in 2000. 7 This study explores the prescription drug landscape in Pennsylvania from a consumer perspective. To see what a consumer might encounter when trying to buy very common prescription medications, AARP commissioned a mystery shopping study. Professional market researchers called 587 pharmacies in five Pennsylvania counties (Allegheny, Blair, Lebanon, Lehigh, and Philadelphia) during June and July 2002. They asked for prices for a 30-day supply of Celebrex, Lipitor, Plavix, and Prilosec.8 These four drugs were chosen for the study because they are among the most widely used and costliest prescription drugs under Pennsylvania’s Pharmaceutical Assistance Contract for the Elderly (PACE) Program. The AARP Pennsylvania Prescription Drug Cost study examines:

• price variations for each drug across the state • urban versus non-urban drug prices • prices by type of store (chain vs. independent vs. mega-

discount and club) The report also contains appendices organized by county. These include graphic information on the prices for each drug within the county and store-by-store information on the prices provided to the mystery shopper at the time of data collection.

Findings Prescription drug prices vary widely across Pennsylvania. The costs of Prilosec, Plavix, Lipitor, and Celebrex vary widely across Pennsylvania. The graph illustrates the 10th, 50th, and 90th percentile of prices encountered for the prescription drugs. Ten percent of prices reported fall below the 10th percentile price on the graph, while 90 percent fall below the 90th percentile price. The median (or 50th percentile) is the exact middle price in the distribution. It is, therefore, the place where half of the prices are higher and half the prices are lower.

The price variation is most dramatic for Prilosec, where the 90th and 10th percentile prices vary by $26. The ranges between the 90th and 10th percentile prices found for Lipitor ($22), Celebrex ($21), and Plavix ($20) are more muted. The appendices contain graphs with the 90th, 50th, and 10th percentile price for each of the four drugs by county. Appendix A lists the prices for all medications collected by the mystery shoppers; the extreme highs and lows are reported for each drug although they are not depicted graphically. The consumer taking one or more of these drugs on a year-around basis can achieve hundreds of dollars in cost savings from “shopping around.” Pharmacies in urban areas have lower Plavix prices than those in non-urban areas; the prices of other drugs did not differ by urbanicity. One factor that might affect drug prices is whether the pharmacy is located in an urban or non-urban area. Urban pharmacies are likely to deal with a higher volume of medication and to have greater competition. For purposes of this study, urban areas are defined as the county of Philadelphia and the city of Pittsburgh. All other areas are considered non-urban areas. The price for Plavix is significantly different in non-urban and urban areas. The average price for Plavix in non-urban areas is $128, while the price in urban areas is $123. This $5 difference represents a 4% price difference between the average urban and non-urban prices for Plavix. Additional analyses did not find any significant differences between non-urban and urban areas for the three remaining prescription medications. Mega-discount and club stores frequently have lower prescription drug prices. A second way to examine prescription drugs prices is by type of store. Pharmacies and other providers were separated into three categories: chains, independents, and mega-discount and club stores. A chain store could be CVS or Rite Aid; an independent could be Duncansville Pharmacy or Fisher’s Pharmacy. Examples of mega-discount and club stores include K-mart and Sam’s Club. The graph illustrates the average price for each of the four prescription drugs at chain, independent, and mega-discount and club stores.

There are significant price differences by type of store for each of the four prescription medications. In three of four instances, prices for the mega-discount and club stores are lower. Also, independent drug stores are competitive with chain stores; for three of the four drugs independent providers had slightly lower than average costs relative to chain drug stores. Findings are discussed by prescription drug type. Celebrex: For Celebrex, mega-discount and club prices are significantly lower than either chain prices or independent prices. In addition, independent prices are significantly lower than chain prices. Lipitor: This pattern holds for Lipitor as well. Lipitor prices at mega-discount and club stores are lower than those at chain pharmacies and independents. Additionally, independent providers have lower Lipitor prices than chain pharmacies. Plavix: Prices for Plavix are significantly lower at independent stores than at chain pharmacies. Neither of these stores’ prices differs significantly from mega-discount and club stores. Prilosec: Prilosec prices are significantly lower at mega-discount and club stores than at independent or chain pharmacies. Independents and chains do not differ significantly from each other in average price.


Conclusions Mystery shopping studies uncover both the typical consumer experience and the possible variation in consumer experiences when shopping for particular products. This study provides a snap shot of the experiences that a consumer might have when inquiring about the cost of various prescription drugs. The mystery shoppers found wide variation in prescription drug prices provided by pharmacies around Pennsylvania. Prices vary by store and by type of store where the medication is purchased. Often, the mega-discount stores and clubs have lower prices than either independent pharmacies or chain pharmacies. Additionally, many independent pharmacies report slightly lower than average prices relative to chain stores. For Plavix, drug prices also varied by locality, with lower prices being found in urban areas. The key messages to consumers are:

• Take a little extra time and call three or more providers to determine who offers the best prices. Because older consumers take a number of prescription medications on a monthly basis, the cost savings over a year from choosing a pharmacy wisely can be quite dramatic.

• It may pay to check with mega-discount stores, if they are available in your area, as well as independent and chain pharmacies. The mega-discount stores appear to offer consumers considerable savings.

• When prescriptions are renewed every few months, make the calls for price comparisons again. Prescription drugs prices change day-to-day, week-to-week and month-to-month. The pharmacy with the better prices may very well have changed.

• Store representatives are not equally knowledgeable about prescription medications and their dosages. Some of the price variation can be attributed to the different store representatives and their level of knowledge about medications. If you are unsure about the information provided to you, wait a short time and call back.

Because these four prescription drugs are for chronic medical conditions, most consumers will be taking these medications for a very long time. Saving a few dollars with every prescription will, over time, add up to big savings. Thus, it pays for consumers to shop wisely.


Methodology

In June and July 2002, pharmacies and other providers of prescription medications were contacted in five Pennsylvania counties – Allegheny, Blair, Lebanon, Lehigh and Philadelphia. Providers were contacted by telephone and asked for price quotes on five prescription medications. The five medications are: Celebrex (200 mg), Lipitor (10 mg), Plavix (75 mg), Fosamax (70 mg), and Prilosec (20 mg). The dosages represent a 30-day drug supply. Additionally, providers were asked if they have a prescription delivery service, its costs, and whether they have any discount programs or services. The list of pharmacies and providers was downloaded from the Internet using ReferenceUSA, an Internet based reference service. ReferenceUSA is a subscription service available to libraries, educational institutions and government agencies. The initial list contained 779 entries. After telephone verification, the list was culled to a total of 587 open and operating pharmacies and providers. As noted in the introduction, drug prices for Fosamax were collected. However, when results were reviewed, the level of variation for Fosamax prices was unique. Fosamax comes in five different dosages. It is a recommended medication for five different illnesses – four types of osteoporosis and Paget’s Disease. From the first day of telephone surveying, there was confusion about the dosage amount being requested. Because of its different dosage amounts as well as the range of illnesses Fosamax can treat, it is not surprising that recommendations for its use vary. Pharmacists sometimes recommended that the shopper explore a different regime with her doctor. Given the wide price variation and confusion about dosage that was reported anecdotally by the mystery shoppers, it was decided not to report results for the Fosamax drug prices. The data collection was conducted by FGI, Inc., a professional marketing research firm. An outside vendor was selected to ensure the validity and reliability of the data. FGI, Inc. formally monitored 15 percent of the interviews for verification and quality control. Additionally, the project supervisor listened to all interviews as they were conducted so questions and problems could be quickly addressed.

Appendix A

County Specific Tables with Alphabetical Listing of Provider, Address, Phone Number and Prices for Four Common Drugs


The following pages list the county-specific providers – alphabetically – with their addresses, phone numbers and the prices for Celebrex, Lipitor, Plavix, and Prilosec. These prices are quotes obtained by the professional market researchers between June and July 2002. Occasionally, there are missing or incomplete data. In these instances, the pharmacists or store representatives did not provide the requested information; the symbol g is used to identify the providers who did not supply price quotes. In other instances, the price quotes obtained were discount prices based on provider discount programs or senior discounts. Discount prices are delineated by . These prices are not reported because the size of and eligibility for the discount could not be verified. Anecdotal evidence from the market researchers’ interviews found that not all store representatives were equally knowledgeable about the prescription medications and their dosages. In some instances, interviewers spoke with pharmacists and in other cases they spoke with clerks or store representatives. Some of the price variation can be attributed to the different store representatives and their level of knowledge about medications.

AARP Pharmacy Mystery Shop Worksheet 732-1054

Drug Pricing The objective of this research is to determine how much variation occurs from pharmacy to pharmacy in the pricing of specific prescription drugs that are frequently prescribed to elderly people who may not have health insurance other than Medicare. You will be masquerading as the daughter/son/husband/wife [depending upon your own age] of someone elderly who has been prescribed all FIVE of the medications that we are researching, and you are attempting to find out how much these medicines will cost for a one month supply without medical insurance. Because this is not a survey, you will not be using a script, per se, and you are allowed to improvise if necessary, although it probably will not be. The pharmacist or pharmacist’s assistant will most likely simply provide you with the costs of the medications. I would recommend using the same approach every time [i.e. always be the daughter or always be the husband, etc….] and I would also recommend making up a name and age for the person you are calling about just in case it comes up [as long as the age is 65+, past retirement age, you should be fine]. If you use the same approach every time, you will be consistent in your calls, and after a few calls it will seem very comfortable to you and it’s less likely that you would give away that you’re calling from a market research firm. You must never give this away. Keep the call conversational. You will be provided with a bullet point checklist of questions to ask, and an answer sheet that you will use to record the information you collect. Be sure to write clearly, and to verify the information as you receive it, because it will be data entered by someone in our data services department. You will have a comments field at the end of your answer sheet to include anything unusual that happens during the call, or any additional information that you receive that does not relate specifically to the answer categories on your sheet.
