About This Manual This manual explains the process for using our convenient and easy-to-use eReport tool for completing your premium audit report online. You’ll find information on how to complete the entry screens, upload supporting documents and save or print your submission.

We know will find this manual easy to follow allowing you to quickly complete your eReport without all the paperwork required of a mail audit, so you can focus on running your business.

Policyholder Premium Audit Communication

You will receive a Policyholder Premium Audit Report letter when it is time to complete the premium audit on your expired policy. The letter will include your secure access code and the link to the consumer website where you can access the eReport tool using your code.

eReport Secure Access Screen

Click the Premium Audit eReport tab to access the eReport page. Click the Access eReport button.

The Secure Access screen requires entry of the following information provided on your policyholder letter:

• Secure Access Code • Policy Number • Zip Code

eReport Dashboard

The eReport Dashboard presents the policy(ies) requiring completion of a premium audit report. Under the Action menu, click on Start Audit, as shown above.

If you are not able to complete your audit in one sitting, you may save your work as a draft, throughout the screens, and complete your audit when you have time, by clicking on Resume Audit.

A couple of other things to note on the eReport dashboard are:

• The Help and FAQ links at the top of the screen: o The Help link opens a window to display customer service contact information. o The FAQ link opens the Premium Audit page of Liberty Mutual’s external website containing general

information about premium audit as well as information about eReport.

Important Documents Screen

A reminder screen will be presented showing the business documents to have available, so you can complete the eReport as quickly and efficiently as possible.

Contact Information Screen

The Contact Information screen requires completion of all fields providing information about your authorized representative who can answer questions on behalf of the policyholder regarding the business operations and the audit information. If entry into a required field is omitted, an error message will be displayed requiring entry into that field before being allowed to continue to the next screen. Your contact information will be stored for future use in pre-populating this screen for future audits.

Note: It is especially important to enter the correct email information for the appropriate representative because all emails regarding the audit will be sent to this person whom we expect will be able to answer our audit questions.

General Information Screen

The General Information screen will be pre-populated with the policy address information on file. Entry is only required on this screen if updates are needed to correct address information.

Note: Since all mailed audit information will be sent to this address, it is very important that the policyholder verifies this information and makes corrections, as needed.

Company Information

The Company Information screen requires entry of information about your business operations. “Help” information is available next to each field to assist you in providing accurate information.

Owner and Officer Information Screen

The Owner and Officer screen requires entry of any owner or officer information associated with your business whether they are active, inactive, or unpaid. Again, the “help” information should assist you in quickly providing accurate information.

Payroll Information Screen

The Payroll Information screen requires entry of gross payroll by class code, excluding owner or officer of payroll. The information for this screen must be current payroll information that is within 15 days of the audit period.

Claims Verification Screen

The Claims Verification Screen will be pre-populated with your employee claim information on file. Entry is only required on this screen if updates are needed to correct the information. Once you have validated the claim information, you will need to check the verification box to confirm your agreement as to the accuracy of this information.

Premium Basis Screen

The Premium Basis screen is for entry of values for classes on a policy that have a premium basis other than payroll – i.e. sales, gallonage, etc.

Upload Documents Screen

The Upload Supporting Document screen is available for you to conveniently upload your supporting audit documentation. There is a 3MB per document limitation for the upload process to work successfully. If your documentation is greater than 3MB, it will be necessary for you to do multiple uploads of your documents.

Printing and Saving Once you have completed your online audit submission using eReport, you may wish to save or print a copy of what you’ve entered for your own records. The instructions that follow will walk you through saving and printing your submission report step by step. First, which internet browser are you using?

If possible, we recommend you use Google Chrome for this process. Click on the blue link below that represents the browser you’re using with eReport for instructions specific to that browser:

Print / Save eReport Audit Submission with Internet Explorer

1. Return to the eReport Dashboard

If you are not currently on the Premium Audit eReport Dashboard, click the eReport Dashboard link in the upper-right corner of the screen, just under the section showing the named insured, policy number and audit period.

2. Click the View Submission link

Once you’re on the dashboard, click the View Submission link under the Action section. This opens a report showing all your audit responses.

3. Open the Submission Report

A dialog box will appear at the bottom of your screen asking if you’d like to save the submission report. Click the Save button.

A second dialog box will appear at the bottom of your screen asking if you’d like to open the submission report. Click the Open button.

4. Print or Save the Submission Report

When the submission report opens in Adobe Acrobat, first click the File menu, then select Save As… save the file to your desired location or select Print to send the submission report to your printer.

1. Return to the eReport Dashboard

If you are not currently on the Premium Audit eReport Dashboard, click the eReport Dashboard link in the upper-right corner of the screen, just under the section showing the named insured, policy number and audit period.

2. Click the View Submission link

Once you’re on the dashboard, click the View Submission link under the Action section. This opens a report showing all your audit responses.

3. Open the Submission Report

A box will appear at the bottom of your screen with a small carat pointing downward. Click the carat to expand the options and select Open.

4. Print or Save the Submission Report

When the submission report opens in Adobe Acrobat, first click the File menu, then select Save As… save the file to your desired location or select Print to send the submission report to your printer.

Review Screen

The Review Screen provides you the opportunity to review all the information entered on each of the screens, and edit as needed, before you submit your eReport. In addition, you may add comments to support the information you provided for the audit. Finally, you’ll need to answer the few remaining questions at the bottom of the screen, acknowledge that the information is accurate, then sign and submit your eReport.

Successful Submission Screen

Upon successful submission of your eReport, a confirmation screen will be presented acknowledging such. At the bottom of the screen, you will notice a link to a short survey soliciting your feedback on the eReport process. Your feedback will allow us to make further enhancements to this tool, so it can fully meet your needs and provide a fast, easy, efficient and accurate way to complete and submit your audit.
