severevisionlossinCaucasiansoverage65.

I am often asked “What is Macular Degeneration?” AMDisadegeneration/deteriorationofthemacula.Manyolderpeopledevelopmaculardegenerationaspartofthebody’snaturalagingprocess.Themaculaistheverycenterpartoftheretinathatallowsus tosee finedetailsclearlyandperformactivitiessuchasreadinganddriving.Whenthereisdiseaseofthemacula,thecentralvisioncanbeaffectedcausingblurryvision,darkareasand/ordistor-tion.AMDaffectsyourabilitytoseenearandfarandeverythinginbetween.AMDcanmakesomeactivitiessuchassewingorreadingdifficultorimpossible. AlthoughAMDdecreasesthecentralvision,itdoesnotaffecttheperipheralorsidevision,andtherefore,AMDalonedoesnotresultintotalblindness. Even inmoreadvancedcases,peoplecontinue tohavesomeusefulvisionandareoftenabletotakecareofthemselves.Inmanycases,AMD’simpactonthevisioncanbeminimal.

Types of Macular Degeneration “Dry”(atrophic)maculardegenerationisthemostcommonform.Itiscausedbyagingandthinningofthetissuesofthemacula.Visionlossisgenerallygradual.Depositsundertheretinacalleddrusenareacommonfeature.Drusenaloneusuallydonotcausevisionloss,butwhentheyincreaseinsizeand/ornumber,thisgenerallyindicatesanincreasedriskofdevelopingadvancedAMDandthusanincreasedriskofvisionloss. “Wet”(exudative)maculardegenerationaccountsforabout10%ofallAMDcases.Itresultswhenabnormalbloodvesselsformunder-neaththeretinaatthebackoftheeye.Thesenewabnormalbloodvesselsleakfluidorbloodandblurthecentralvision.Visionlossmayberapidandsevere.

What are the risk factors for Macular Degeneration? Theexactcausesofboth“dry”and“wet”AMDarenotcompletelyunderstood.GeneticsappearstobeamajorfactorinmorethanhalfofthecasesofAMD.Otherriskfactorsmayincludeadiethighinfat,cigarettesmoking,exposuretobrightsunlight,heartdisease,highcho-lesterolandhighbloodpressure.

How is Macular Degeneration Treated? NutritionalSupplements-Peoplewithmoderatestagesof“dry”AMDcanlowertheirriskofdevelopingadvancedstagesofAMDbytakingahigh-dosecombinationofvitaminA(betacarotene),vitaminC,vitaminEandzinc.Supplementsdonotappeartoprovideany

Bloodwasurgentlyneeded forwounded troopsaswar ragedacrossEurope in1940.Dr.Drewwaschosenbythe InternationalTransfusionAssociationtoorganizethe“BloodforBritain”project.The “Blood for Britain” project collected, processed, and trans-ported14,500unitsofplasma—allwithin fivemonths.Dr.Drew’sscientificresearchhelpedrevolutionizebloodplasmatransfusion,sothatpooledplasmacouldreadilybegivenonthebattlefield,whichdramaticallyimprovedopportunitiestosavelives. FearingtheUnitedStateswouldbedrawnintoWorldWarII,theU.S.armed forces requested thedevelopmentofasimilarbloodcollectionsystem.Dr.Drewestablishedaneffectiveplasmacollec-tionandpreservationorganization—amodelfortoday’svolunteerblooddonationprogram.Dr.Drewadvocatedanopportunity forAfricanAmericanstodonatetoadesegregatedbloodbank. Dr.DrewreturnedtoFreedmen’sHospitalasChiefofSurgery,andtoHowardUniversityasProfessorandChairoftheDepartmentofSurgery.There,Dr.Drewmadewhatheconsidered tobehisgreatestcontribution tomedicine: teachingandhelping tocertifyhundredsofblacksurgeons.In1944,hereceivedtheNAACPSprin-garnAwardforhiscontributionstohumanity. Marriedin1939,Dr.Drewandhiswife,Lenore,hadfourchildren:(Bebe(forbloodbank),Charlene,Rhea,andCharles).Dr.DrewdiedApril1,1950,inatragiccaraccident.Itwaswidelyrumoredthathediedfromlackofpropermedicaltreatmentduetothecolorofhisskin.Thisrumorhasbeenprovenincorrect,asfellowdoctorswhowerealsointheaccidenttestifiedtothefactthatDr.Drewdiedfrominjuriessustainedintheaccident. Without the efforts of Dr. Drew, blood collection and storagewouldnotbewhereitistoday,andsavingallthelivesasaresult.

Throughout time, therehavealwaysbeendoctorswhoweregroundbreaking in their field,andoneof thosedoctorswasDr.CharlesDrew.

Dr.CharlesDrewwasanAfricanAmericandoctorandresearcherwhoseknowledge,visionandleadershiphelpedrealizethepossibil-ityofbloodcollectionandstorage;hisdiscoveriesinthe1930sand40sservedasamodelforthefutureofbloodbanking.Dr.DrewwasborninWashingtonDC.Heattendedapublicschoolinthenation’scapital,andgraduated in1922 fromPaulLawrenceDunbarHighSchool.He continuedhis education atAmherstCollege inMas-sachusetts,wherehewrestledwith the rigorsof academicsandservingascaptainofthefootballteam. GraduatingfromAmherst,DrewacceptedapositionatMorganStateCollege,teachingchemistryandbiology,aswellascoachingfootball.HismedicaleducationbeganduringtheGreatDepressionatMcGillUniversitySchoolofMedicineinQuebec,Canada.ThoughDrewworkedasawaiterwhileastudentatMcGill,hegraduatedsecond inhisclassof137.ElectedtoAlphaOmegaAlpha,DrewlaterbecamevicepresidentoftheMedicalHonorSociety.In1933,heearnedMDandCM(mastersofsurgery)degrees. Following internships at Royal Victoria Hospital and MontrealGeneral,Dr.Drewwasappointedtoaresidencyinsurgery(1936-1938)atHowardUniversity’sFreedmen’sHospital.Amedicalresi-dencyatNewYorkCity’sPresbyterianHospitalenabledhimtodopost-graduatework atColumbiaCollegeofPhysicians andSur-geons. There,hemethismentorandcollaborator,Dr.JohnA.Scudder.Dr.Drewgraduated fromColumbiawith thedissertation,“BankedBlood:AStudyinBloodPreservation.”HewasthefirstAfricanAmer-icantoreceivetheMcScDegree(doctorofscienceinmedicine).

Apprehension,trepidation,fear.AllarecommonfeelingsparentsfacewhencomingtotheRonaldMcDonaldHouseofIowaCityforthefirsttime.ForGinaTenPas,itwasnodifferent.

“ItwasascorchinghotJulydaywhenIfirstlaideyesontheRon-aldMcDonaldHouse.Iwas39weekspregnantandI,alongwithmyhusbandandtwoyoungchildren,drovefromDesMoinestocheckintotheRonaldMcDonaldHouseinIowaCity,”saidGina. TheTenPas’unbornsonwasscheduledtomakehisarrivalthenextday.However,itwasn’tgoingtobeatypicaldeliveryasJohnPeterhadbeendiagnosedwithHypoplasticLeftHeartSyndrome,orincommonterms,theleftsideofhisheartwasseverelyunderdeveloped.Withinthefirstfewdaysofhislife,JohnPeterwouldundergothemostcom-plicatedheartsurgeryinpediatrics.

“IwasfilledwithapprehensionandworrythefirstnightattheRonaldMcDonaldHouse.Weweredownrightscaredforourlittleboyandtheeffectsthiswholeexperiencewouldundoubtedlyhaveonourothertwochildrenwhoweretooyoungtounderstandthegravityofthesituation,”addedGina. ExperiencesliketheTenPas’arenotuncommonforfamiliestravelingtotheRonaldMcDonaldHouseofIowaCity,a31-roomfacilitylocatedjustthreeblocksawayfromtheUniversityofIowaChildren’sHospital.FamiliesoftencometotheRonaldMcDonaldHouseexperiencingagreatdealofstressanduncertaintybecauseoftheirchild’scondition. Evenotheramenitieslikethechildren’splayroom, the downstairs game room,fully-stocked pantry and kitchens, freelaundry,wirelessinternet,library,andfreeparkingpassesplaya largerole inmak-ingthepathfamiliestakeeasier.Thelarge

backyardwithaplayground,patiodeck,andwoodedareamaketheHouseawelcomingplaceforsiblings,too. “Westrivetokeepfamiliestogetherasmuchaspossible,”saidSamBurke,FamilyServicesCoordinator.“Because it’sbeenshownthatkidshealfasteriftheirparentsandfamiliesarenearbyandinvolvedintheirmedicaltreatment.” TheTenPas’stayedsix fullweeksasJohnPeter’ssurgeryandrecoveryhaditsupsanddowns.Theirinitialapprehensionandworryfadedasmuchasitcould,andtheTenPas’foundthemselvescon-nectingwiththeotherfamilies. “Ifeltasthoughwehadjustjoined‘theclub’…Iwascomfortedtoseethesefamiliespushingforwardandthrivingdespitetheirdifficultsituation.Thewarmthfromtheirsmilesremindedmethatthisdidn’thavetobeahorrible,lonelyexperience.Wewereinthistogether,”addedGina. JohnPeterisnowactinglikeanormalbabyandisnextheartsurgeryisquicklyapproaching.Hisoldersister,Abigail,andbrother,Caleb,arelookingforwardtocomingtotheir“otherhome,”theRonaldMcDonaldHouseofIowaCity. “ThankstotheHouse,ourchildrenhavefondmemoriesoftheirbaby brother being born,” added Gina. “The Ronald McDonaldHousehelpedholdournewfamilyoffivetogetherwhenweneededitthemost.”

Expanding Its Reach Thecareandconcernforthehealthandwell-beingof children isn’t solely limited to the Ronald McDonaldHouseofIowaCity.Infact,thereachofRonaldMcDonaldHouseCharitiesofEasternIowa(RMHCofEasternIowa)stretchesbeyondthewallsoftheHouse. Over thepast fouryears,RMHCofEastern Iowa’sgrantsprogramhasimpacted7,900EasternIowachildrenbysupportingover30areanon-profitswhoseprojectsorprogramsbenefitthehealthandwell-beingofchildren.In2011,RMHCofEasternIowaawarded$37,000to10dif-ferentnon-profitorganizations. RMHCofEasternIowa,remarks“theprogramfitswellwiththemissionofthisorganizationbycreating,findingandsupportingorganizationsthatdirectlyimprovethehealthandwell-beingofchildren.Theorgani-zationsimpactedbythegrantsprogramhelpRMHCofEasternIowaextend itsmission farbeyondtheRonaldMcDonaldHouse in IowaCity.” Alsosince2008,RMHCofEastern Iowahasofferedgraduatinghighschoolseniorstheopportunitytoreceiveascholarshiptoaidwithcollegetuition.AmberHeinrich,asenioratIowaStateUniversity,wasthefirstrecipientofamulti-yearscholarship. “Thescholarshiphashelpedmereducemycollegefinancialburdenthatmoststudentsencounter,”saidAmber.Beyondthefinancialhelp,thescholarshiphasbeensignificanttoAmberbecauseithastaughtherlifelessonofwhyvolunteeringissoimportant. “Iknewthatvolunteeringcouldmakeadifference,but Ihonestlydidn’tknowhowmuchofadifferenceuntilIstartedvolunteeringattheRonaldMcDonaldHouse.Thepeoplewhostaytherejustappreciatethelittlestthings,likehomemadebrowniesorplayingwiththekidsintheplayroomforalittlebit.Youtrulyseehowmuchofadifferenceyoucanmakeinaperson’slifejustbydoinganicedeedorvolunteeringinyourcommunity,”addedAmber.

publictransportation,withotherfamilymembersandeveninyourownhomethatcancauseyoutogetsick.Flusea-sontypicallybeginsinNovemberandcanlastuntilMay.Itisestimatedthatasmanyas20%oftheU.S.populationwillgettheflueachyear. The elderly, newborns and peo-plewithcertainchronicillnessesareparticularlysusceptible tocomplica-tionswiththeflu.Determining ifyouhavecaughtacoldortheflucanbetricky,butcertainsymptomscandif-ferentiatebetween theconditions. Ifsymptomsseemliketheflu,contactadoctorrightaway,asatestcanbedonetoconfirmtheillness.

Symptoms can include:•Nauseaandvomiting.•Fever with the flu can last 3 or 4

days. Fever with a cold is rare inolderchildrenandadults.

•Headachescanappearsuddenlyorbe severe when accompanied bytheflu.Headacheisrarewithacold.

•Muscle aches canbe severewiththeflu.

•Tiredness and weakness is oftenextremewiththefluandcanlast2ormoreweeks.Mildtirednessandweaknessisassociatedwithacold.

time—waytooyoungforprostatecancer.Butthebloodworkhadindicatedotherwise. Whenthefirstbiopsycamebacknegative,wehadwantedtobe-lieveweweresafe.But theexperiencehadbeensodisturbingwecouldn’tshakeit.EspeciallyKeith.Everthepragmatist,hehadwantedtobepreparedjustincase,andunderstandallhisoptions.Sohe’dstarteddoingsomeresearchonprostatecancerandtreatmentchoice. From thebeginning, the twoofushad lookedat theseoptionsfromdifferentperspectives.IhadthoughtanypotentialcancershouldbetotallyremovedfromKeith’sbody,andthus,Ionlyhadconsideredsurgery. Ourmarriagewasstrongandsupportive,ourhealthgood.Neverdidwethinkthatcancerwouldenterourlivesandcauseustostartarguingsovehemently.Wheneverwediscussedthepossibilityofcan-cerorimpotence,welostoursenseofhumor,digressingquicklyintotouchysarcasmthatmadeitimpossibletomaintainacivildiscussion.

InJanuary1995,whenitwastimeforKeith’sthirdprostatescreen-ing,wewerebothtired.Wehadbeenliving inakindofpurgatory,enduringtorturouswaitingbetweenyearlyappointment,andalwayswonderingifthistimethecancerwouldbeconfirmed.Thestressanduncertaintyhadtakenatollonus,andwetalkedaboutmakingthisourlastappointmentever. Dr.BarrettmadeafewnotesinKeith’schart,andthenlookedatus.Theresults,hesaid,werealarming.NotonlyhadKeith’sPSAlevelrisenoncemore,butDr.BarrettcouldnowfeelanoduleonKeith’sprostate.Sureenough,hesuggestedthedreadedthirdbiopsy. Keithrefused.Emphatically.Thephysicianreviewedtheimportantfactorsofprostatecancerforbothofus,emphasizingthehighdeathrate.“Pleasereconsider,”addedDr.Barrett. Wedidn’tstaylongafterthat.Keithquicklyassuredthephysicianhewould thinkabout “everything,”andpromised tocallwithin theweekwithhisdecision. The day after the appointment, our daughter phoned from herhomeinWashingtonState.ShecouldtellthatIwasupset.“What’swrong,Mom?”sheaskedinaworriedvoice. Ibrokedownimmediately.“It’syourDad.They’realmostpositivehehascancer,andhesayshewon’thaveathirdbiopsy!Sayshe’ssickofthewholething!Thenthere’streatment.Yourfather’ssoafraidofbecomingimpotent;he’dratherdiethanhavesurgery!Whatifhedoeshavecancer?”Imanagedtogasp.“Hisconcernoverkeepinghissexlifeisgoingtokillhim!” “Mom,doyouremembertheconversationyouhadwithStevenandmewhenwewereteenagersabouthowweshouldfeelaboutsex?Itseemedsoimportanttoyouthatwelearntotrulyappreciateit.Yousaidweshouldthinkofsexasachancetogiveandreceiveplea-surethroughourbodies.”Bethcontinued.“Irememberyousaying,‘Neverbeashamedorembarrassedaboutyoursexdrive.Treasureitasabeautifulgift.’Doyourememberthattalk,Mom?” OfcourseIrememberedthattalkalmost10yearsearlier.Talkingtothechildrenlikethathadbeensuchaturningpointinmyownlife. Ihadbeenraisedtobea“goodgirl”wheresexwasconcerned.Myconservative,religiousparentshadinstilledinmeabeliefthatsexwaswrongbeforemarriage,afterwhich,itbecamemiraculouslyallright.Consequently,whenKeithandImarriedattheageof21Iwasshyandhesitantinmysexualbehavior,andcertainlyinexperienced. Keithwasdifferent.Hewasliberal,daring,andanxioustoexperi-ment.Keiththoughtofsexasanaturalpatofbeinghuman,andsome-thingweshouldneverhesitatetoenjoy.Ourtwoverydifferentviewsaboutsexhadsometimescausedproblemsinourmarriage.UntilthedayIhadhad“thattalk”withthechildren.

Ithadbeensuchanawakeningtohearmyself.WhatarevelationithadbeentodiscoverthatIhadmentallyadoptedKeith’sbeliefs,buthadneveractuallyputthemintopractice. “Yes,Iremember,Beth.Asamatteroffact,itwasafterthattalkwithyouandSteventhatIdecidedtobemoreopenaboutsexmyself.” IthoughtabouthowproudIwasfordoingthat.Forchanging.Forbeingmoreopentoexperimenting.Fornotholdingbacksomuch.Inthelasttenyears,oursexlifehadbecomemuchmoresatisfyingtobothofus,mostlybecauseIhadbeenwillingtochange.Sexwasmoreexcit-ingnowthanithadeverbeen.Andweexpectedittoonlygetbetter. WhenIgotoffthephone,Ithoughtagainabouthowimportantsexreallywasinourmarriage.Forthefirsttime,IfeltsomeempathyforKeith’sfears—andhesitantlyacknowledgedtomyselfthatitmightbehardtogiveupwhatwenowhad. Butstill,Icouldn’tgosofarastoagreethatKeithshouldn’thavethisbiopsy—orsurgeryifheneededit!
