POSTER 094 The Ameloblastic Fibro-Odontoma in the Mandible: A Case Report and Histopathologic Study Yong-Kwan Choi, Korea (Choi YK; Han SJ; Kim CH; Lee JH; Kim KW) Statement of the Problem: This is a case report of a rare mixed odontogenic tumor, an ameloblastic fibro- odontoma, in the posterior of right mandible. A 2-year- old male patient was referred to our department for a large tumorous lesion on his right mandible. The radiol- ogy examination revealed a large radiopaque and radi- olucent lesion with an impacted and unerupted tooth, #44, #45, #46 #85. After surgical enucleation and curet- tage of the mass, the tumor was confirmed to be an ameloblastic fibro-odontoma. The mass was composed of the dental organs and an island of odontogenic epi- thelium embedded in a cell-rich mesenchymal stroma. An immunohistochemical study was carried out. The results showed positive CK7 staining, and weekly posi- tive staining for Bcl-2 and Ki-67 staining. However, there was no CEA, CK8, CK12 and CK16 staining observed. Materials and Methods: Fibroblasts were taken from the patients with multiple unerupted teeth and primary cultured. The total RNA was extracted and examined using a microarray assay. Normal dermal fibroblasts and gingival fibroblasts were used as the control. The results of the microarray were confirmed by quantitative RT- PCR. Method of Data Analysis: n/a Results: The microarray data are as follows. The genes showing up-regulation by more than 2-fold were FGF-5 (fold ratio; 3.721), FGF-11 (fold ratio; 3.085), and BMP-4 (fold ratio; 5.345). The genes shown down-regulation by more than 2-fold were BMP type II receptor (fold ratio; 0.227), Caspase 7 (fold ratio; 0.470), FGF-7 (fold ratio; 0.383), MMP-3 (fold ratio; 0.182), MMP-12 (fold ratio; 0.469), RUNX1 (fold ratio; 0.131). The results of quanti- tative RT-PCR were in accordance with the microarray data. Conclusion: These results suggest that FGF-5 mediates the overgrowth of fibroblasts, and that the down-regula- tion of the MMP-312 activity might be important to the non-eruption of teeth. POSTER 096 A Case of Undifferentiated Carcinoma Arising in the Right Side of the Palate Toshiya Sano, DDS, PhD, 9-1 Motomachi, Suita City, Osaka Fu, 564-0031, Japan (Akamatsu H; Shima M) Statement of the Problem: Undifferentiated carcino- mas relatively rarely develop in the head and neck re- gion, but their prognosis is poor, causing distant metas- tasis in the early stage. We encountered a patient with an undifferentiated carcinoma that developed in the right palate region, and markedly responded to radiotherapy and chemotherapy. Materials and Methods: The patient was a 77-year-old male who visited our department for a chief complaint of swelling of the right palate region on December 26, 2005. Method of Data Analysis: As histopathological find- ings, poorly differentiated atypical cells were noted, and EMA was positive on immunostaining. Based on these findings, undifferentiated cancer was diagnosed. No re- gional lymph node and systemic metastasis was noted on CT, MRI, Ga and bone scintigraphy. Results: As preoperative treatment, radiotherapy at 40 Gy and chemotherapy with one course of TS-1 100 mg/body x 14 days CDDP 10 mg/body x 5 days and one course of TS-1 100 mg/body x 14 days were per- formed. Surgery was planned, but not performed be- cause of the strong request of the patient. Additional irradiation at 30 Gy and chemotherapy with one course of TS-1 100 mg/body x 14 days CDDP 10 mg/body x 5 days markedly reduced the tumor, and the patient was discharged. The patient received 5 courses of chemo- therapy with TS-1 100 mg/body x 14 days at the outpa- tient clinic as of December 2006, and no recurrence was noted. The patient is under course observation, and we will closely follow him. Conclusion: Radiotherapy and chemotherapy were markedly effective for undifferentiated cancer.
