Scope of Presentation
• The pathology associated with percutaneous interventions
• Post mortems after surgery
• Post operative cardiac pathology
• Short and long term consequences of cardiac surgery
![Page 3: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/3.jpg)
Three Guiding Principles
• Find out exactly what was done and check that it was technically satisfactory. Take photographs with a proper camera.
• Exclude common complications such as haemorrhage, infection, stroke and thrombo-embolism
• Consider cardiovascular disease as the cause of death
![Page 4: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/4.jpg)
Percutaneous Interventions
• Central venous lines
• Gastric feeding tubes
• Pulmonary artery wedge catheters
• Chest drains
• Pacemakers
• Coils, stents and closure devices
![Page 5: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/5.jpg)
Central venous lines
• Make sure the line is secured to the skin
• Dissect downwards to the site of insertion, reflecting the skin carefully
• Check where the line is lying : in a vein, an artery or soft tissues?
• Some haemorrhage is common, tracks easily into loose connective tissues. May cause blood staining of effusions
![Page 6: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/6.jpg)

Percutaneous Feeding Tubes
• It is remarkable that most have absolutely no complications
• If complications occur they are usually very soon after insertion. Peritonitis is severe and widespread
• Limited reactions are uncommon
![Page 8: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/8.jpg)
Pulmonary Artery Catheters “Swan-Ganz”
• Measures right atrial, right ventricular and pulmonary artery pressures
• Also a pressure that is a function of left atrial filling pressure: “wedge pressure”
• A catheter is wedged in a small pulmonary artery. An inflated balloon isolates the tip from the proximal pulmonary artery
![Page 9: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/9.jpg)

Chest Drains
• Chest wall injury
• Lung contusions
• Injury to the heart or great vessels
Can be very difficult to identify the exact source of bleeding, almost impossible to dissect the intercostal arteries. Examine the chest wall very carefully
![Page 13: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/13.jpg)
Pacemakers
• Leads pass between tricuspid chordae and embed themselves in RV muscle
• Position of leads
– Single lead in RV
– Lead in RA and RV
– Lead in RA, RV and in a branch vein of the
coronary sinus (CRT, cardiac
resynchronisation therapy)
![Page 14: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/14.jpg)

Coils, Stents & Closure Devices
• Coils are inserted to induce thrombosis
• Fabric covered stents are inserted to prevent haemorrhage from rupture of an aneurysm or after liver procedures
• Metallic stents are inserted to maintain patency at the site of angioplasty
• Closure devices are used to repair ASDs and some acquired VSDs
![Page 19: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/19.jpg)

Common Complications of Coronary Angiography and Angioplasty
• Haemorrhage at site of femoral artery puncture. Radial artery increasingly used.
• New devices have reduced the size of puncture site haematomas
• Dissect the femoral artery and retain the artery at the site of puncture
![Page 22: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/22.jpg)
Serious Complications of Angioplasty and Stenting
• Dissection of aorta during placement of catheter in coronary orifice
• Perforation or rupture of artery by balloon or guide wire or aorta by intra aortic devices
• Stent thrombosis
![Page 23: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/23.jpg)

Coronary Stents
• All pathologists will do examinations on patients who have had angioplasty months or years previously
• Stents can be difficult to find
• Palpation will not distinguish a calcified plaque from a stent
• Proximal stents can be opened

Deaths after Interventions
• In the vast majority of cases recent interventions will not have caused death. If it has everyone will know about it.
• Careful dissection of the intervention site is the first and most important step
• Many interventions cause bleeding. Do not overestimate the importance of small haemorrhages
![Page 40: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/40.jpg)
Deaths after Interventions
• In your report describe the procedure in outline detail only
• Wherever possible retain the tissues related to the intervention
• Take copious histology
• Write a clear non judgemental report, quoting respected studies or reviews
![Page 41: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/41.jpg)

Five Particular Problems
• Identifying the source of post operative bleeding
• Diagnosis of post operative myocardial infarction
• Pneumonia and acute lung injury
• Intestinal necrosis
• Explaining post operative renal impairment
![Page 43: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/43.jpg)
Deaths after Surgery in England
• 2332 in hospital deaths in 2002
• 102 of these had an autopsy within 30 days of an operation, 92 records traced
• 65% complete pre and post mortem agreement
• 20% major and 15% minor disagreement
• 39% infectious, 22% thromboembolic, 22% ischaemic heart disease
• Disagreements 35% orthopaedics, 33% neurosurgery, 27% general surgery & 6% cardiothoracic
Ann R Coll Surg 2005;87:106
![Page 44: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/44.jpg)
ICUs,Hôpital Bichat, Paris
• 3 year prospective study
• 1492 patients, 315 deaths, 167 autopsies
• The 171 missed diagnoses included 21 cancers, 12 strokes, 11 MIs, 10 PEs and 9 cases of endocarditis
• Modern diagnostic techniques did not seem to reduce the rate of missed diagnoses
Combes et al Arch Int Med 2004; 164: 389
![Page 45: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/45.jpg)
Surgery and the Cardiac Patient
• Each year 27 million patients have surgery under anaesthesia in the USA
• Of these 8 million are at risk of cardiovascular morbidity
• 50,000 have a peri-operative infarct at a cost of US$ 20 billion
• Prevention involves revascularisation, pre-operative beta blockers and intra-operative monitoring
Fleisher and Eagle N Engl J Med 2001;345:1677-82
![Page 46: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/46.jpg)
Post Mortems after Surgery
• If possible, read the clinical records in detail
• Identify and list all incisions, access sites, canulae, drains and pacing wires
• Examine all surgical anastomoses and operative sites. 250 ml haemorrhage common, 500 ml not unusual.
• Inspect costal angles, subphrenic, paracolic and pelvic spaces
• Pus is always important!
![Page 47: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/47.jpg)
Post Operative Haemorrhage
• Is more common in emergency operations, especially if the patient was acidotic or in shock
• Although it is very difficult to identify the exact site of bleeding the major region of haemorrhage can usually be found
• Ask a haematologist to comment on the coagulation profile and platelet function
![Page 48: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/48.jpg)
Ischaemia after Vascular Surgery
• 185 patients, 70% male, mean age 66yrs
• 66 had transient ischaemic events with ST segment depression
• 12 (6.5%) had a myocardial infarct
• Mean duration of ischaemia was 225 mins. Most had a post operative tachycardia
• All infarcts were non Q wave/NSTEMIs, diagnosed biochemically
Landesberg et al JACC 2001;37:1839-45
![Page 49: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/49.jpg)
PeriOperative Ischaemic Evaluation (POISE study)
• 8351 patients, 190 centres, 23 countries
• Four post operative cardiac biomarkers and a range of clinical, ECG and imaging
• 415 (5.0%) had a perioperative MI
• Only 34.7% had ischaemic symptoms, only 12.3% had Q waves
• 30 day mortality 11.6% in those with MIs but 2.2% in remainder Devereaux et al Annals of Internal Medicine 2011;154:523
![Page 50: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/50.jpg)
Risk Factors for Perioperative MI
• Post operative increase heart rate
• History of stroke
• Major vascular or orthopaedic surgery
• Creatinine > 175µmol/l
• Increasing age
• Emergency or urgent surgery
• Serious bleeding (≥ 2 units transfused)
![Page 51: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/51.jpg)
Myocardial Infarction after Surgery
• The infarcts are often small and usually close to an area of healed infarction
• Coronary thrombosis is rare
• The infarcts are usually haemorrhagic, possibly because of reperfusion when heart rate declines
• Take both coronary artery and myocardial histology
![Page 52: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/52.jpg)

Mortality after by pass Surgery
• 1158 males and 215 females followed for 12 years. Most angina free!
• 266 males had died by 12 years compared to an expected 231
• Female mortality was twice the predicted
• Age, smoking, diabetes, raised BMI and previous myocardial infarction were risk factors
Ketonen et al Int J Cardiol 2008;24:72-79
![Page 55: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/55.jpg)
Autopsies long after Cardiac Surgery
• Make sure you recognise the median sternotomy scar
• Look out for the internal mammary graft as you raise the sternum! Clip it!
• Opening the pericardium is difficult
• Identify the grafts, especially distally.

Complications of Valve Surgery
• Peak incidence of early prosthetic valve endocarditis is 2 months but late disease can occur at any time
• Valve thrombosis is devastating and is more common in liver disease
• Bioprosthetic valves degenerate but modern bi leaflet prosthetic valves are very durable

Autopsies within 30 days of Cardiac Surgery
• There is public access to the results of cardiac surgery, by centre and operator
• Patients are given detailed pre operative information
• There is an inevitable mortality, greater in high risk patients and in females
• There is therefore less “political” need for routine autopsies in these patients
![Page 66: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/66.jpg)

What are the Important Acute Complications?
• Haemorrhage, especially in emergency surgery for aortic dissection
• Acute by pass graft thrombosis
• Perioperative myocardial infarction
• Prosthetic valve endocarditis or thrombosis
• Multi organ failure
• Neuropathological changes, especially stroke

Multiorgan Failure after Surgery
• Biochemical evidence of acidosis
• Pulmonary oedema or acute lung injury
• Severe hepatic congestion or fatty change
• Patchy small bowel necrosis
• Patchy pale discolouration of renal cortices
• Multiple blocks from all relevant organs
![Page 75: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/75.jpg)
Pneumonia & Acute Lung Injury
• A difficult distinction, especially as pneumonia is a cause of ALI.
• In ALI lungs are often >1000g and all lobes appear abnormal
• Most patients have survived several weeks in ITU and show chronic changes
• Lung histology is essential for accurate diagnosis
![Page 76: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/76.jpg)

Intestinal Necrosis
• Probably the earliest post mortem feature of multi-organ failure
• Should be suspected in any patient with acidosis
• Genuine small bowel necrosis has a confluent pattern. Autolysis is patchy
• Histology is of limited value
![Page 80: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/80.jpg)

Post Operative Renal Failure
• The majority of cases are pre renal, usually the result of cardiac failure
• Cortical and tubular necrosis are rare and require detailed histology for diagnosis
• Any patient > 65 years may have kidneys weighing 120g and cortices of 3mm
• The appearances of diabetic kidneys are variable, especially in early disease
![Page 82: Post Mortems after Medical InterventionsPost Mortems after Surgery and Percutaneous Interventions Patrick J Gallagher ... • 65% complete pre and post mortem agreement ... modern](https://reader030.fdocuments.us/reader030/viewer/2022040819/5e6597c6cb64e76b11571711/html5/thumbnails/82.jpg)
Conclusions
• Important unsuspected changes will be identified in at least 15% of cases
• Document these with photographs and histology
• Acute myocardial infarction is the most important challenge for the pathologist
• Have the courage to accept that you will not explain death in up to 15% of case
