multi-day visits.

8:00 am – 8:45 am: Population Health Management: Our Context, Structure This presentation will provide an overview of Partners’ population health program structure, including the reasons for population health strategies at Partners, the structure required to pay for population health and the design of the incentive program to encourage participation.

8:50 am – 9:30 am: Program Outcomes and Evaluation Data on program outcomes will be provided, along with a description of how the data is used to improve the program.
