The Pledge Card

• Most companies use the standard United Way pledge card. If your organization has requested it, the pledge cards can be preprinted with donor information.

• Pre-printed pledge cards require a two week turnaround. We require a spreadsheet with:

• Prefix

• First name, last name and middle initial

• Unique identifier

• In order to minimize duplicates accounts in our database and errors, the more information (address, email) the more we have to match the donor if they already exist in our database.

• Please use black or blue ink only.

• Please encourage email address capture.

Annual Gift

Five ways to give:* Payroll deduction * Cash/check *Credit/debit card *Bill me *Stocks

For payroll deduction:

* Be sure to include amount per pay in Step 3, not entire gift. Mark number of pay periods.

Remember:

* Credit card - correct number of digits and expiration date

* Frequency of billing

* Stocks can still be pledged using the form.

Three ways to designate a gift:

A - Undirected gift to United Way

B - Focus Area

C - United Way program, affinity group, or local 501(c)3 health and human service

Other designations:

• United Way programs and services are now listed online. They should be written on the front of the pledge card “write in” using the appropriate space.

• United Way has an open designation policy. We accept any designation to a local 501(c)3 health and human service agency.

• If this option is chosen, we request a city and state to help identify the organization.

Payroll Envelope

• If your company information is not included on a label, please update where indicated.

• Complete all information: number of payroll donors, amount of payroll gift.

• Please complete the verification questions.

• United Way must have a signature from company representative on the envelope front before it can be opened and processed.

Special note

• Please make a copy of all payroll pledge cards.

• Donors should also make a copy of their pledge cards.

• Tax implications

• United Way of Greater Toledo does not provide tax receipts for gifts made through payroll deduction.

• The IRS requires a copy of the pledge card for income tax purposes.

• Please make a copy of the envelope for your records.

Non-payroll Envelopes

• These envelopes (green ink) are for all non-payroll gifts:

• Corporate gift

• Special events

• Employee gifts of cash, check, credit/debit, or direct bill

• If payment is enclosed, the total must be written in that column.

• There must also be an amount in the pledged column – it can be greater than payment enclosed.

• Subtotal line is for the employee gifts.

• Total gifts is for all three categories.

• Please get a signature from your CEO if a corporate gift is included.
