Why did we develop our own team plan?

• The journey for developing person centred planning in Gwynedd began in March 2005. During this time we became increasing aware that the development of a person centred team plan would give us the foundations on which to work.

• We believed that it would give us a better understanding of our working relationships and what support each of us would need both individually and as a team.

• It would identify each individual members’ strengths and gifts in order to utilise these skills and support each other to develop them to their full potential.

• We needed to share our learning and encourage agencies, departments and the care sector to work in the same way. We therefore decided that the ‘7 Day Training’ model would provide Managers with the tools and skills to develop Person Centred Team Plans.

How did we develop our team plan?

Over the past year we have been developing our team plan during our team meetings. We have used the person centred thinking tools to gather information to put into our plan:Like & admireReputations exerciseGood day/Bad dayTeam donut

By using these exercises we began to pull out what was important to each of us and what good support looks like for us.

The things that are most important to Angela :

• That I arrive at work and arranged meetings on time

• That I have a coffee when I arrive at the office

• That I have my diary, laptop and work bag with me at all times

• That my diary is always up to date• That the photocopier and printer is

working • That I have up-to-date information about

planned meetings, schedules training and presentations in order to organised my work by creating a work schedule

• That I have at least one hour in the morning to sort out my e-mails and organise my work

• That others respect and value the work that I do.

• That others respect that I have strong values

• That I present work that is of the highest standard possible

• That people are open and honest with me

What you need to know and do to support Vicky :

• If I’m tired, let me get on in peace and quiet so I can get home and relax!

• When I am busy, offer me some help if you can, if not, don’t interrupt me, just let me get on with what I need to do.

• If you want me to do something, please give me plenty of notice so that I can plan the work and time properly

• I need to have my laptop, mobile phone, and car in good working order.

• If you need to contact me, by e-mail or mobile is best as I am often out of the office for long periods of time.

• If I have upset you, done something wrong or offended you in anyway, please tell me in a gentle way. I dislike conflict and arguments and would never deliberately want to upset anyone.

• Sometimes I open my mouth without engaging my brain – I like to think and talk my ideas through with other people

• That there is a clear understanding between myself & the psychologists we share the office with as to when we need to be in the office.

• Be honest with me.• To have my Tom Tom with me as I travel a lot.• To have regular team meetings to update my diary

& support each other.

What next?

• Continue to develop our own plan using the person centred thinking tools.

• Encouraging other agencies and schools to develop person centred team plans.

• Introducing Person Centred Teams to the Elderly and Physical Disabilities Services

• Develop Person Centred Supervision in Gwynedd.

• Introduce Person Centred Team Plans and Positive, Productive Meetings to the CSIW (Care Standards Inspectorate for Wales)
